2 Please note: Lambeth Council is collecting the information on this form (which includes personal details) for the purpose of administrating the Expression of Interest and Application Forms for the Lambeth Tennis Network licences. 4 Coaching Scheme Minimum Criteria Eligibility Please note: in order to ensure that you can demonstrate your eligibility for a Lambeth Tennis Network Licence agreement please make sure Section 2 is fully completed Any evidence that is submitted with this LTN Licence application will be retained by Lambeth Council All documentation must be valid and current (i.e. valid within three years of attainment) Note: Please do not send originals, please send copies only. Part 2 Qualification requirements LTN Licenced Coaches To register as an LTN Licence Coach you should currently hold as a minimum the LTA level 3 (DCA) Licenced Coach qualifications, or equivalent. This will ensure that you have a current DBS (CRB) certificate, First Aid and Child Protection awareness and Insurance. I wish to register as an LTN Licenced coach and I can provide copies of the following; Please ensure that you provide: Copies of the relevant qualifications in accordance with the LTA Coach Education structure (or equivalent) A valid DBS (CRB) check Evidence of having attended First Aid and Child Protection courses; Current Insurance certificate. Please note that if you cannot provide this evidence then you will need to obtain an LTA level 3 or equivalent as minimum criteria to becoming an LTN Licenced Coach. LTN Assistant Coaches You will be required to show evidence that your Assistant or Assistants are qualified in accordance with the LTA coach Education structure. All coaches you wish to register as LTN Licenced Assistant Coaches should as a minimum be LTA Registered and currently hold the Level 1 or 2 Assistant Coach qualification. 4

5 This will ensure that your Assistant Coaches have a valid CRB check, First Aid and Current insurance certificate. Do you wish to register LTN Licenced Assistant Coaches? Yes No If they are not registered please ensure that you provide: Copies of the relevant qualifications in accordance with the LTA Coach Education structure (or equivalent) A valid DBS (CRB) check Evidence of having attended First Aid and Child Protection courses; Current Insurance certificate. Should this change please ensure that you register the coach under the Licensing Agreement; that they hold the LTA Level 1 as a minimum; and submit the relevant documentation to the Lambeth Council Officer administering the licensing scheme. Documentation Required Please attach appropriate copies as indicated Existing Financial debts Do you have, or aware of any outstanding debts with Tennis facilities within the London Borough of Lambeth or under any previous sports and recreation agreements with Lambeth Council Yes To be eligible as an LTN Licenced Tennis coach you need to finalise any outstanding payment. Lambeth Tennis Network Coaching Classification Please select one of the options below to indicate which LTN Licence you wish to apply for: Option A Lambeth Tennis Network Programme Coach As a coach you are predominantly interested in coaching LTA developmental programmes. You may be eligible for exemption of court charges for the delivery of these programmes. You will be charged a licensing fee of per annum You will pay the general Adult court rate. 5

6 You agree to participate in the LTN voluntary programme (on average up to (2hrs per week). I wish to register as a Lambeth Tennis Network Programme Coach If ticked, please go to Part 3. Option B Lambeth Tennis Network Non Programme Coach As a coach are predominantly interested in private coaching, Under LTN Licence agreements you will commit to attending Lambeth Tennis Network Forums and supporting Tennis initiatives in the borough. You will be charged a licensing fee of per annum You will pay a Non Programme commercial court hire rate. Currently per court per hour (subject to review from 1 April 2012). You agree to voluntarily participate in collective LTN showcase events and Lambeth Tennis development initiatives throughout the year. I wish to register as a Lambeth Tennis Network Non Programme Coach If ticked, please go to Part 4 6

7 Part 3 Coaching History and Programme Coaching History Please indicate any prior coaching experiences and professional experience which will support your application: Coaching Experience Example: Coaching assistant Clapham Common Age Groups 7 to 50 years Period and Type of Employment 4 years, Casual Employment Note: Coaches are welcome to add additional attachments of supporting documents (such as affiliations and alliances referees or letters of support from prior experience / current professional relationships) that may demonstrate their eligibility to become an LTN Licenced coach.
