1 Investigation of Accidents and Serious Railway Incidents at Level Crossings Global Level Crossing Safety & Trespass Prevention Symposium 2016 Helsinki
2 Presentation The presentation will focus on: Traffic situation at the level crossings. Design. Changes over time. New safety challenges. Safety recommendations. Some conclusions. Investigation procedures single accidents and thematic (class investigation).
3 Thematic (class investigation) The thematic investigation is on reviews on serious incidents over a period of two years, approximately 640 incidents and near-misses. Discusses problems related to design, safety and maintenance. Inspections of relevant level crossings to gain better understanding of the problems.

7 Thematic (class investigation) The barriers stay down for too long time. Level crossing near stations with a complex traffic pattern. Level crossing at station with crossing trains. Level crossings on open lines with an uncomplicated traffic pattern.
8 Single accident (1) The car driver suffered light injuries. Line speed 130 km/h. 14 incidents have been recorded on this level crossing prior to the accident. Line of sight according to regulations. No signalling problems.

10 Single accident (1) The barriers are equipped with a sign at the inside, instructing the driver to drive through the barriers in case of emergency. The driver failed to follow the instruction. AIB Norway question the efficiency of these kinds of signs.
11 Single accidents (1) The AIB Norway proposed one safety recommendation to the IM to evaluate object solutions for level crossing with a high risk of vehicles being trapped between the barriers. The IM response to the safety recommendation is that introduction of object solutions is not in their plan for the near future.
12 Single accident (2) Level crossing used by pedestrians and cyclists. Fatal accident. Line speed 90 km/h. IM had received many reports. Line of sight, in both directions, was according to regulations. Possible use of ear-phones.
13 Single accident (2) The increased number of users had not been taken into account when the municipality made its risk analysis. The IM had not communicated the high number of incident reports to the municipality. No measures were taken when the number of users reached the maximum acceptable level.
14 Single accident (2) A safety recommendation was addressed to the Ministry of Transport and Communication. To revise the method of analysing used by the municipality Every potential hazard in case of changes in the local environment of the railway have to be addressed.
15 Fatal accident in station area Person hit by crossing train and died immediately. Used head phones.
16 Single accident (3) Level crossing used by pedestrians and cyclist. Limited car traffic. Fatal accident (pedestrian). Line speed 50 km/h.
17 Single accident (3) The investigation found that there had been a series of illegal crossings. Over a period of about one week in six level crossings we found the following distribution:
18 Single accident (3) Full barriers with flexible ends are not described in any analysis or technical regulations. Several analysis were carried out when the line and level crossings were upgraded. Five measures were identified, but they were not implemented at the time of the accident. The safety recommendation addressed to the Metro company points at risk assessment carried out of level crossing protection system. The Metro company adopted the safety recommendations and necessary measures have been taken and accepted by the Safety Authority.
19 Single accident (4) Level crossing for pedestrians. Maximum speed 70 km/h. Fatal accident. Front camera available.
20 Single accident (4) It is assumed that the deceased was focusing on the vehicle at the opposite platform. A light rail system usually emphasizes openness and accessibility at its stops. Physical barriers are underused.
21 Single accident (4) One safety recommendation was proposed: The Light Rail stops should be designed in a manner that facilitates safe crossing by all users. Measures have been taken by the Light Rail company.
22 Some conclusions (1) Much of the infrastructure today seems to be based on a few basic assumptions about pedestrians and drivers: They have their attention, especially vision and hearing, directed towards the infrastructure and the traffic situation (mobile phone). They respect the guidance of lights, signs and physical barriers (conscious wrongdoing). They act in a rational and safe manner (second train coming). The single accidents, and the class investigation, both suggest that these assumptions are not valid on a fairly regular basis in level crossing and trespass situations. Accidents and near accidents are then sometimes the result. How do we respond to this challenge?
23 Some conclusions (1) Design of the level crossing. Traffic patterns, barriers, barriers stay down, information on the barriers etc. Pedestrians use mobile phones (sound and picture) Has to be taken into account when designing or renewing a level crossing. Changes over time. Risk analysis. Other measures?
