1 Appendix 1 Salford CCG Priorities and Objectives Our Priorities 1.) Contribute in a leading way to the development and deployment of GM devolution, ensuring all key stakeholders are involved and consulted 2.) Drive the development of a NW sector approach to commissioning delivery where appropriate and particularly in the context of planning for Healthier Together implementation 3.) Deliver on the in year objectives of the Salford CCG strategy, including driving the delivery of our strategic objectives and associated outcomes (as detailed within the attached) in particular, ensuring strong primary care voice and participation in further development of integrated care 4.) Ensure strong alignment and engagement with all our stakeholders and in particular our members, local partners and population

2 Quality: Engage with our members, population and providers to minimise variations in quality and secure continuous improvement Overall Clinical Lead(s): Dr Jeremy Tankel and Dr Annette Johnson Specialist Area Clinical Leads: Dr Girish Patel (Safeguarding Children), Dr Jeremy Tankel (Adult Safeguarding), Dr Peter Budden (Medicines Management) Executive Lead(s): Francine Thorpe Achieve and sustain zero instances of MRSA (NHS OA7 S) Reduce the number of antibiotics prescribed in primary care by 1% (or greater) and reduce the number of co-amoxiclav, cephalosporins and quinolones prescriptions as a percentage of the total number of selected antibiotics prescriptions as a percentage of the total number of selected antibiotics prescribed in primary care by 10% from 2013/14 values Prevent and reduce total CDIFF infection numbers to 60 or better over (NHS OA7 SM) Support 30 GP practices in implementing the Productive Practice toolkit to improve efficiency and productivity Support a cohort of 9 GP practices in improving medication safety as defined by the GM medicines optimisation standards using a Breakthrough Collaborative Series (BTS) Ensure that the CCG meets the statutory requirements and recommendations as outlined in the new Safeguarding Assurance and Accountability framework supporting GP practices in embedding safeguarding processes and supporting victims of domestic abuse by implementing the IRIS project Develop and implement an organisational patient experience strategy linked to our overall strategy Establish process to share outcome of reported incidents and their analysis by practices to ensure that lessons learned are implemented and disseminated Implement the Association of Medical Research Charter in relation to improving the evidence base and quality of services we commission, developing a Salford CCG research and development strategy aligned to the objectives of Health Innovation Manchester

3 Community Based Care: Support and invest in primary and community based care services to increase integration and the provision outside hospitals Overall Clinical Lead(s): Dr Jenny Walton, Dr Steven Elliot Executive Lead(s): Karen Proctor Increase the proportion of older people at home 91 days after discharge from hospital into reablement / rehabilitation services from 73.5% in 2013/14 to 80.5% by March 2016 (NHS OA4 SM BCF) Reduce the number of people reporting a negative experience of GP and out of hours services by 7.8% (NHS OA6) Undertake a series of community service reviews and implement recommendations Plan for the end of national contract for local Clinical Assessment and Treatment Services (CATS) Develop a Primary Care Strategy Agree the Salford Standards for the General Practice Develop and implement a plan to extend access to general practice outside core opening times Develop an operating model(s) for general practice working within Salford s Integrated Health and Social Care system Facilitate and support the federation of general practice Implement and undertake new processes in relation to general practice contracting duties in collaboration with NHSE (co-commissioning) Develop and manage processes to assist in the monitoring and improvement of quality within general practice Review Local Commissioned Services for Opticians and Pharmacies

4 Integrated Care: Enable people to retain their independence and quality of life through integrated health and social care services with partners. Clinical Lead(s): Dr Paul Bishop Specialist Area Clinical Leads: Dr Babar Farooq (Children and Young People), Dr Paul Bishop (Integrated Adults and Older People) Executive Lead(s): Jennifer McGovern (Older people and Learning Disabilities), Karen Proctor (Continuing Health Care, Community and Hospital Integration) Reduce the amount of time people spend avoidably in hospital through better and more integrated care in the community and outside of hospital by 2.8% (NHS OA3 QP BCF) Reduce the rate of permanent admissions to residential and nursing care homes by 3.9% by the end of March 2016 (BCF) Reduce delays to transfer for care out of hospital by 2.7% by March 2016 (BCF) Roll out of Integrated Care Programme for Older People Model throughout the year Production and approval of all business cases for Integrated Care Programme commissioning intentions Achievement of annual financial balance for the Integrated Care Programme pooled health and social care budget Working with partners, develop the business case for an Integrated Care Organisation for adult health and social care services Engage in a leading way in the Salford New Models of Care Vanguard Programme Develop the Salford Integrated Record by incorporating social care, mental health and wellbeing data Review support for carers (children and adults) Scope opportunities for integrated care for children s services Refresh the Salford Strategy for Learning Disabilities Ensure robust processes are maintained in relation to Continuing Healthcare and that the CCG delivers its associated statutory duties Manage relationships and specific commissions with schools and third sector providers

5 In Hospital Care: Support secondary care reconfiguration through Healthier Together and maintain a focus on the delivery of NHS constitutional standards Clinical Lead(s): Dr Paul Bishop (Scheduled Care) and Dr Elaine Tamkin (Unscheduled Care) Executive Lead(s): Steve Dixon Reduce the number of people having a negative experience of hospital care by 0.2% (NHS OA5 QP NHSE CCG assurance of good or outstanding for performance domain (overall measure of constitution standards mainly hospital care) Commence planning for the implementation of the Greater Manchester Healthier Together Programme recommendations for Salford / Northwest sector Working with health and social care partners, develop and manage system resilience plans Ensure delivery of NHS Constitution standards across operational performance and patient access Undertake specific specialty reviews i.e. Dermatology and Neurology Undertake a series of hospital service reviews and implement recommendations

6 Long Term Conditions and Mental Health: Prevent people developing long term conditions and ensure those that do have access to high quality, patient centred services Clinical Lead(s): Dr Girish Patel (LTC), and Dr Tom Tasker (Mental Health) Specialist Area Clinical Leads: Dr Steven Elliot (LTC Cancer), Dr Sheila McCorkindale (LTC Diabetes and Kidney Disease) Dr Tin Aye (EoLC), Dr Tom Regan (LTC CVD), Dr Girish Patel (LTC Liver and Respiratory) Executive Lead(s): Karen Proctor (Long Term Conditions and CAMHS), Jennifer McGovern (Mental Health) Improve the health related quality of life score for people with one or more long term condition including mental health from an average of 64.5 to 67 (NHS OA 2 BCF) Increase access to psychological therapies achieving 15% or better (NHS OA2 QP) Achieve dementia diagnosis rate (people 65 and over) of 75.2% or better by March 2016 (NHS OA2 QP SM) Achieve and maintain recovery rates for psychological therapy treatment of 50% or better (NHS OA2 SM) Reduce potential years of life lost through conditions considered amenable to healthcare by 4.7% (NHS OA1 QP) Review the first year of the Local Commissioned Service for Long Term Conditions Develop disease-specific standards for incorporation into the Salford Standards for General Practice Undertake a series of reviews and projects related to services for Liver Disease, Cancer, Respiratory Disease, Renal Disease, Diabetes and CVD Commission an early intervention in psychosis service to meet NICE compliance and 2016 waiting time targets Ensure Salford remains compliant with the Mental Health Crisis Concordat Procure a locally based service for people with Aspergers and ADHD Develop a Commissioning Strategy for Autism Spectrum Conditions Produce a CAMHS Transformation Plan Secure Dementia Friendly Community Status Review the first year of MATs (Memory Assessment and Treatment service)

7 Effective Organisation: Develop our systems, processes and people to deliver high quality, value for money services Clinical Lead(s): Dr Hamish Steadman (Chair) Executive Lead(s): Steve Dixon (Finance and IM and T), Hannah Dobrowolska (Governance, Performance, Planning, OD, Communications and Engagement) Overall CCG assurance rating from NHS England of Good or Outstanding for all 5 assurance components Staff survey participation of 90% (noting 2015 is the baseline year) Stakeholder Survey participation of 70% for CCG members and results in the top third of all CCGs for 50% of topic areas Produce a comprehensive Governance Statement and gain a Head of Internal Audit opinion of Significant Assurance for the CCG s systems of internal control Implement integrated performance, planning and risk reporting system for CCG business planning Develop (with SCC and key providers) Salford s Sustainability Plan to articulate shared priorities for the next 5 years and deliver NHS planning requirements and financial sustainability in line with GM Devolution plans Customer service team to deliver 90% of responses associated with general PALS, complaints, concerns, claims, MP enquiries and Freedom of Information requests within required timescales Deliver 75% or more of actions for EDHR, OD, Comms and Engagement, Sustainable Development and Risk Management strategies Update the CCG s 5 year financial plan to identify the level of resources available for the CCG s commissioning strategy Achieve all of the statutory finance targets for 2015/16 Ensure robust contracts are negotiated, agreed and signed with all providers before 31 st March 2016 Ensure the CCG delivers the 2015/16 objectives outlined in the CCG s strategic estates plan Deliver all areas within the Primary Care IM and T work plan
