1 REQUEST FOR PROPOSAL Janitorial Service Provider Yakima Valley Farm Workers Clinic EHIBIT A SERVICES 1. Terms: 1.1 Contractor Requirements Contractor s Employees. Contractor agrees to maintain a staff of properly trained and experienced personnel who are direct vendor company employees to ensure satisfactory performance under this Agreement. No sub-contractors or franchisees will be allowed to provide Services. Contractor will provide YVFWC a list of all employees providing Services or supporting the provision of Services at the Locations and keep the list current Quality of Services. Contractor warrants and agrees that the Services will adhere to professional standards, be performed in a timely manner, and be accurate and free from any material defects. Contractor agrees that Contractor will, at Contractor s own cost, correct all material defects in the Services as soon as practical after Contractor becomes aware of the defects. If Contractor fails to correct material defects in the Services within a reasonable time, then YVFWC may correct the defective Services at Contractor s expense. This remedy is in addition to, and not in substitution for, any other remedy for defective Services that YVFWC may have at law or in equity Trash Bag Disposal. Contractor will dispose of all bags of trash (black bags) in appropriate trash bins or dumpsters and will not drag trash bags across building floors or carry leaky trash bags across building floors. Contractor will dispose of hazardous waste bags (red bags) in the appropriate collection site. The actual collection site will be determined at each Location on initial site walk-through Mop Water. Contractor will replace water in mop buckets every following each third exam room and will leave each mop bucket clean at the end of each work shift Furniture Movement and Cleaning. Contractor will move exam tables, phlebotomy chairs, and operatory chairs, to the extent they can be moved, during strip and wax procedures. Contractor will be responsible for cleaning the bases of the exam tables, assistants' chairs, phlebotomy chairs, and operatory chairs as outlined in the matrix below Flooring. Contractor shall schedule carpet cleaning services to allow 24 hours drying time prior to the site opening for business. Contractor will utilize commercial-grade carpet cleaning system at all Locations. Contractor shall provide YVFWC a monthly floor care calendar by the 1st of each month, detailing the specific date(s) floor care will be performed at each Location. 1

2 1.1.7 Cleaning Product and Method Suitability. Contractor will always use cleaning products and methods which are manufacturer approved for the cleaning/care of all flooring, furniture, and surfaces in each Location Supply Room. Contractor will maintain an orderly supply room Security. Contractor will ensure their employees properly arm/disarm the building security system and lock the exterior doors of the building(s) as required. Contractor will ensure their employees are trained to call the security monitoring company phone number (located on each YVFWC security keypad), and wait onsite for security personnel to arrive when accidentally setting off the security system. Contractor will not permit anyone, except their employees, inside any Location at any time. Contractor will ensure that all of their employees wear a shirt or jacket bearing the name of the company they work for and a name badge while at any Location Location Requirements and Needs. Contractor will work with YVFWC corporate maintenance and each Location maintenance supervisor to determine specific Location requirements and needs. These items should include, but not be limited to, specific times certain areas must be cleaned, which lights should be left on overnight, which doors should be locked or left unlocked, and where the communication log will be kept Contractor will provide YVFWC a copy of Contractor s blood born pathogen clean up training documents within thirty days of the Effective Date YVFWC Responsibilities Trash Bags. YVFWC will provide black trash bags, red infectious waste bags, all paper products, hand soap, and hand sanitizer as needed Cleaning Responsibilities. YVFWC staff will clean the following patient contact areas in medical exam rooms, phlebotomy labs (blood draw), and dental operatories; exam tables, assistants' chairs, phlebotomy chairs, and operatory chairs. Contractor will not be responsible for and will not clean the patient contact areas on exam tables, assistants' chairs, operatory chairs, or phlebotomy chairs in the medical exam rooms, phlebotomy labs, and dental operatories Alternatives. Proposals for Services which do not meet all the requirements or constraints may be considered by YVFWC if sufficient information is provided by Contractor on an alternate method of achieving the same outcome which YVFWC considers of equal value. 2
