2 PURPOSE OF THE POSITION The Clinical Nurse Specialist Gastroenterology and Hepatology position is an advanced nursing practice role that is responsible for providing specialist nursing expertise in the gastroenterology service and nursing leadership across services to support the coordination and care of patients with Irritable Bowel Disease and Hepatitis. This role will: provide nursing leadership within the service in order to ensure patient treatment and care planning is comprehensively co-ordinated and informed by specialist knowledge utilising care models that best meets the specialty whilst in partnership with the patient and family/whaanau. contribute clinical expertise for the development, maintenance and review of quality activities which include policies, procedures and wider initiatives in Gastroenterology and across services. as a senior nurse this role has the responsibility for leading and improving the quality of professional nursing practice and promoting high standards of care by supporting an environment where excellence can flourish. work closely with all members of the multidisciplinary team to ensure that Gastroenterology and Hepatology patients receive seamless, patient/family whaanau centred care across hospital and ambulatory services whilst strengthening integration initiatives already underway regionally and in localities of Counties Manukau. 4 NATURE AND SCOPE OF RESPONSIBILITIES Key Accountability Clinical Practice Standards / Achievements Provide clinical expertise in the planning and delivery of patient care in both the hospital (including Out Patient Department) and Primary Health Care settings. Undertake Nurse led clinics under the direction of the service consultants Using a proforma and patient pathway provide assessment, treatment and ongoing care to patients via a nurse led clinic. Shared care of patients with Gastroenterology & Hepatology Consultants. Using a proforma and patient pathway organise appropriate tests and procedures to assist in patient diagnosis Deliver patient care and co-ordination of services within an integrated care model. Promote cost effective multidisciplinary health care practice. Promote innovative and creative clinical nursing practice and ensure it is validated with best practice and research activities. Working with key stakeholders in the regional service, hospital and community, in providing input into related standards of practice, protocols, policies, referrals, treatment and guidelines. Work collaboratively with all members of the health care team to ensure staff receives high quality educational input and support. Initiate and /or contribute to forums that critique current practice and research activities. Leadership Act as an effective and positive role model by demonstrating high levels of clinical competence within the multidisciplinary team and primary health care. Advocate on behalf of patient/family/colleagues as appropriate and in a culturally safe manner Work proactively and collaboratively with Medical staff and Primary Care to effectively develop a Gastroenterology & Hepatology service, maximising resources whilst maintaining a high standard of care. Act as an educational resource for clinical staff and provide coaching and leadership as required.

Community Services Situations may often call for tact, diplomacy and will require information to be handled in a discreet and sensitive manner. In conflict situations will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome. PROBLEM COMPLEXITY The Clinical Nurse Specialist is expected to manage a variety of complex and unpredictable problems that are associated with the assessment, treatment and management of Gastroenterology and Hepatology patients. Coordination of care and appropriate services need to be patient /family / whaanau focused. There will be a requirement to be able to prioritise issues and negotiate timeframes while providing a quality customer service. The Clinical Nurse Specialist will provide clinical expertise for patients seen in a variety of inpatient, outpatient settings and associated phone consultations.
8 The range of problems will be diverse and require solutions customised to meet the circumstances of the client. Therefore opportunities will arise to provide innovative options to the client based on proven outcomes. There will be demands to meet deadlines, maintain accuracy and quality of information. Failure to do so could impact on the organisation in terms of patient care and treatment and increased waitlist times. Will refer to a Gastroenterology or Hepatology Consultant for advice or second opinion. SCOPE FOR ACTION The Clinical Nurse Specialist is empowered to make decisions or recommendations relating to patient care, tests/procedures required and treatment. The role requires the ability to use initiative and problem solving skills to develop innovative approaches to issues. Discretion is required to be exercised in releasing confidential information to the appropriate parties. DIMENSION OF THE POSITION Will be responsible to the Charge Nurse Manager and Head of Department for Gastroenterology for the achievement of agreed objectives and operates within the confines of Counties Manukau District Health Board. 9 PERSON SPECIFICATIONS Qualification Minimum Registered Nurse with current NZ Annual practicing certificate Post graduate diploma (or in progress) Preferred Relevant Post Graduate qualification (completed or in progress) Experience Skills/Knowledge/ Behaviour Personal Qualities A minimum of five years recent clinical experience in Medicine/Gastroenterology/Hepat ology. Proficient level PDRP Experience in problem solving, priority setting, and planning. Ability to self evaluate and reflect on practice. Demonstrated ability to use evidence based practice Ability to critique research and use it as the basis of practice. The ability to work independently and be a member of a team Knowledge of current issues in Gastroenterology. Basic Knowledge and understanding of medico/legal and ethical responsibilities A strong patient focus. A strong commitment and genuine interest in quality and service. A capacity to demonstrate strong clinical leadership. A commitment to the development of the nursing profession within CM Health The ability to communicate effectively with all levels of staff and develop relevant networks.
