1 POSITION DESCRIPTION / PERFORMANCE EVALUATION Name: Job Title: Maintenance Supervisor Prepared by: Date: Supervised by: CEO/Administrator Approved by: Date: Job Summary: The maintenance supervisor is responsible for the entire maintenance of the buildings, grounds and equipment. The supervisor is expected to perform routine maintenance, conduct necessary inspections and organize all repair work. DUTIES AND RESPONSIBILITIES: Performance is clearly outstanding;performance is superior it far exceeds standards E = Excellent or expectations;performance is exceptional on a continuous basis. G = Good S = Satisfactory NI = Needs Improvement Performance generally meets or exceeds standards or expectations;attains all or nearly all of position objectives. Performance is adequate it meets standards or expectations, and is developing within the position. Fails to meet one or more job expectations. U = Unacceptable Performance is below accepted levels;fails to meet most job expectations. Demonstrates Competency in the Following Areas: E G S NI U Knowledge and familiarity with all equipment and tools. Performs routine maintenance according to daily, weekly, monthly, quarterly and yearly schedules. Makes arrangements with outside contractors to perform maintenance that not qualified to do. Maintains all department daily and periodic logs and records. Care for grounds, including yard work, trash pick up and seasonal maintenance. Organizes work in order of priority and adjust personnel accordingly. Remains aware of all current problems within the facility s operation and continuously review the status. Consults reading material and other information regarding new requirements and procedures. Performs facility maintenance plumbing, electrical, etc. which is qualified to perform. Cooperatively works with all departments, assisting when needed. Coordinates the orientation of all new maintenance personnel and maintains records. Trains and supervises the maintenance assistants effectively, evaluating their performance. Maintains an inventory of supplies adequate to keep all operations running efficiently by monitoring and replacing stock levels as needed. 1

Sets up trapieze equipment as needed. Remain aware of safety and infection control and be sure that procedures are conducted accordingly. Participates actively in the quality assurance program. Reviews and revises policies and procedures as needed, or at least annually. Professional Requirements: E G S NI U Adheres to dress code, appearance is neat and clean. Completes annual education requirements. Complies with requirements, including all state and federal regulations. Adheres to the Group IV level of HIPAA Minimum Necessary Standard when using, disclosing or requesting Protected Health Information (PHI). Reports to work on time and as scheduled. Wears identification while on duty. Attends annual review. Works at maintaining a good rapport and a cooperative working relationship with physicians, departments and staff. Acts proactively in managing time, workload and other departmental duties. Represents the organization in a positive and professional manner. Resolves personnel concerns at the departmental level, utilizing the grievance process as required. Compliant with policies and procedures regarding department operations, fire, safety and infection control. Complies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department. Total Points 2

3 Education/Experience Requirements: High school diploma or GED. Experience working as a handy man with knowledge of electrical, plumbing and other mechanical related duties. Preference will be given to those individuals who have experience working in a health care setting or within a maintenance department. Skills: Able to communicate effectively in English, both verbally and in writing. Able to meet deadlines and prioritize work. Able to work cooperatively with other departments and staff. Able to manage, train and motivate staff. Physical Demands: Able to stand, bend over and climb when necessary. Able to move and lift equipment up to 50 lbs.
