2 Page 2 of 5 Information This leaflet tells you about the procedure known as an MR Arthrogram; it explains what is involved and the possible risks. If you think you may be pregnant please contact the Radiology Department before your appointment. If you take any medication to thin your blood (aspirin, warfarin, clopidogrel) please ring the Radiology Nurses on as these may need to be stopped prior to your appointment. Please note- do not drive following your appointment as you may not be covered by your insurance company. We recommend that you do not wear jewellery as we may have to ask you to remove this during your examination. If you need an ambulance please ask your GP surgery to arrange this for you, and tell them it is a 'special appointment. If you travel by car please allow plenty of time to find a parking space and have change available for the meter (someone must be able to drive you home-see above). What is an MR Arthrogram? A Magnetic Resonance Arthrogram, is an investigation of a joint, usually the shoulder, elbow, wrist, hip, knee, or ankle. It is a two part procedure: The first part involves placing a needle into the joint to add a contrast agent, sort of dye (this takes place in X-Ray B); The second part of the procedure is the Magnetic Resonance Scan when the pictures will be taken immediately after your injection (this takes place in X-Ray A). Why am I having an MR Arthrogram? This test has usually been arranged following discussion with your Orthopaedic Consultant. It is more accurate than a standard MR scan, in assessing internal structures of your joint, and can help plan when surgery is required. Common reasons for an MR Arthrogram are to help find any damage to cartilage, ligaments, labrum, or laxity/instability of the joint. What are the Advantages? The advantages over a plain MR scan without injection are increased accuracy in finding injuries within the joint. This information will help plan whether surgery is necessary, and which type of surgery is best.

3 Page 3 of 5 What will happen during the Arthrogram? The Doctor performing the injection (usually the Consultant Radiologist) will discuss any questions you may have about the test. You will be asked to lie down on the x-ray table by the Radiology Nurse. The area of skin over your joint will be cleaned with an antiseptic solution. The x-ray camera will be moved into position over your joint by the Radiographer. You will be warned that you are getting a local anaesthetic injection. You will feel a slight scratch, and then stinging whilst the anaesthetic takes effect. After this it is usual to experience slight pushing, but it should not be painful. The needle is placed into the joint using x-ray guidance. To check that the needle is in the right place, some x-ray dye is injected first, and once we are sure the needle is in the right place, a different MR dye is then injected, all through the same needle. The whole procedure usually takes only a few minutes. You will then walk straight to the MR scanner, or if the injection is in your hip, knee, or ankle, you will be taken by wheelchair to MR, to have your pictures taken. This is to avoid too much movement in your joint. Are there any risks when having an MR Arthrogram? Thankfully the risks of the procedure are very small. You may get bruising where you had your injection. The skin which has been numbed by anaesthetic will return to normal feeling after 1-2 hours. Discomfort following injection can last for a few hours, but should then improve. It is possible to be allergic to the dyes used, but this is rare. If you are known to have an allergy, please let us know on the day. Most people will feel completely back to normal after 24 hours, and are usually back to work the following morning. In the days following the test, if the area around your joint becomes more swollen, more painful, difficult to move, or the skin becomes reddened, or if you feel unwell, feverish or develop a temperature, you will need to return to the hospital emergency department to make sure there are no signs of joint infection. The risk of developing an infection after the procedure is very low as a fully sterile technique is used. Are there any alternatives to having an MR Arthrogram? You can have an MR scan without joint injection, but this will not always show internal damage in the joint. If you are unable to undergo an MR scan, for example because you have a pacemaker, or are very claustrophobic (fear of small spaces), the procedure can be done as a CT Arthrogram instead. How do I get my results? The Consultant Radiologist will need to look at your MR pictures and produce a report. Your results will then go directly to your Orthopaedic Consultant Surgeon, and you will get the results at your next outpatient clinic appointment.
