In addition, the program includes an Eligibility Specialist position to assist and monitor entitlements, disability applications, and other benefit programs. The program also includes two Clinician III positions one of which serves as the Eastern State Hospital Liaison and manages the Discharge Assistance Program which provides financial supports for individuals being PAGE 1 OF 9

2 discharged from state hospitals. The second Clinician III provides oversight of the Peer Support Services which includes one part-time employee. For the month of January the Mental Health Case Management program had a total caseload of 283 individuals actively receiving services with 3 admissions and 6 discharges or transfers. The average caseload for a case manager remains around 32 individuals. MH case managers screened as many as 33 current case management service recipients for GAP and at least 10 were approved and enrolled as a result. The unit received 115 referrals for assessment. Number of active outpatient counseling cases managed 132 Number of children and families receiving therapy 82 Total number of new outpatient referrals for January 28 PAGE 3 OF 9

4 Group Therapy Psycho-educational groups currently being offered weekly include the Adolescent Anger Management Group and the Dialectical Behavior Therapy Group for Girls. The DBT Group for Girls did not meet in January. A new group will be started in February Child and Adolescent Case Management The Child and Adolescent Case Management program provides targeted case management and intensive care coordination services to children with mental health needs. Number of children receiving active case management services in January 28 Intensive Care Coordination Intensive Care Coordination (ICC) services are provided to prevent children from entering a residential placement and to return them to a homelike setting as quickly as possible when they are placed in a facility. 1 client received ICC services in January. Please note: The ICC program has been modified which has reduced the number of ICC referrals and created a need for new services. In January CSA approved these new services and corresponding fee rates for future referrals. Therapeutic Day Treatment There are currently 40 students in four Chesapeake schools receiving TDT services and 11 students who are on a referral list. G.A. 5 PACT (PROGRAM FOR ASSERTIVE COMMUNITY TREATMENT): The PACT census for January was 73 individuals with one admission and one discharge during the month. One individual was hospitalized at a local psychiatric hospital and three individuals were hospitalized at a State psychiatric facility. One individual remained in the Chesapeake City Jail. In the month of January, PACT had one individual attending Tidewater Community College and one individual taking classes at Old Dominion University. Five individuals were employed during the entire month. PACT had four individuals who resided in Shelter Plus Care housing in Portsmouth; six individuals with Housing Choice (Section 8) vouchers; six individuals resided in assisted living facilities; three individuals resided in a boarding home and three individuals resided in Virginia Supportive housing programs for the homeless. PSYCHOSOCIAL REHABILITATION SERVICES (COASTAL CLUBHOUSE): Staff continued to facilitate Leisure, Medication Management and various Educational groups. The Men s and Women s groups met weekly. The Clubhouse continued to promote community integration, selfadvocacy, resource awareness, and Peer development. Our interns continued to research and present groups to members. Members participated in a bi-weekly Nutrition Seminar. The volunteer group continued to make tray favors for the hospital and for Allzwell assisted living facility. The weekly trip to the recreation center also continued. At the end of January, seven clients had lifetime memberships to the recreation center. Staff accompanied members to the Rivercrest Community Center weekly for them to participate in the various activities that are offered. Members attended a Valentine Party at Opportunity House in Portsmouth, Virginia. Members also facilitated the leisure activity group that meets weekly. Eight members actively participated as cashiers for the Snack Bar and Snack Cart. More members actively participated in the preparation of food items for the Snack Cart and Snack Bar. Members who helped with the shopping have increased. In January the I Saw That! award was presented to four members for outstanding team players for their contributions. Staff trained over five members in the food preparation process and health department regulations for cleanliness. Two members were trained in lunch collection procedures and process. The Health Department inspection was conducted in January and the license were renewed. SKILL BUILDING: The Mental Health Support Services program has a program supervisor and six full-time clinicians providing services to a population of 47 seriously mentally ill adults. There was one admission and one discharge in the month of January. The Medication Management case coordinator had a caseload of 148 individuals with no admissions or discharges in January. 6 The Telepsychiatry clinic was again run entirely with the assistance of two volunteers and one intern. Telepsychiatry clinics are held Monday through Wednesday from 8:30 a.m. to 4:30 p.m. each day. Capacity evaluations, which are required for individuals seeking authorized representatives and possibly taking steps to obtain a guardian, are coordinated through the senior licensed clinician assigned to this program. There were two referrals and but no capacity evaluation completed in January. PAGE 6 OF 9

7 CASE MANAGEMENT: January 2014 INTELLECTUAL DISABILITY SERVICES Total Referrals: 9 New Referrals: 9 New School Referrals: 0 Pending Assignment: (entered in Credible but not assigned to CM) 0 Pending Intake: (assigned to CM but not registered) 8 Requested Addition to the Statewide Waiting List: 0 Active Case Management (CM) 327 Facility Monitoring 8 Reopened Cases 1 Admitted: 6 Discharged: 9 One (1) individual no longer has case management needs. 1 Two (2) individuals several attempts have been made by the case manager to contact with no response. 2 Three (3) individuals placed on waiting list. No other services requested at this time. 3 One (1) individual has been open for more then 90-days and a psychological evaluation was not obtained. 1 One (1) individual did not complete the intake process. 1 One (1) individual moved to another state. 1 Number of individuals transitioned from residential facilities to the community 0 Note: Report covers period from January 01, 2015 January 31, The following is an update on Individual Service Authorization Requests (ISARs). This is the procedure utilized to access ID Waiver services. 8 INFANT SERVICES UNIT: New referrals in January: 21 Currently serving: 222 Admitted: 21 Discharged: 15 Being processed for admission: 36 Pending screening/intake: 0 During this month, we had the Chesapeake Public School s Part B transition planning team attend our staff meeting to strengthen our partnership as we enter the height of referral time to prepare our toddlers and families for starting school in September We invited our contract agencies to the meeting as well so that our system can better serve our families. We also invited Head Start personnel from the Office of Human Affairs to streamline our referral process and strengthen our partnership. We are working on developing a letter and some resources to share with local elementary, primary, intermediate and middle schools about our services that they can share with families as needed. We continue to work on acquiring all party signatures for the Chesapeake Interagency Memorandum of Agreement Among the Agencies Involved in the Implementation of Part C of the Individuals with Disabilities Education Act (IDEA). PREVENTION UNIT: For the month of January, staff provided 30 hours of direct service time to 211 program participants. Staff attended meetings on Children s Ministry, Family Engagement, Commission on Health and Well Being, Out-of-School-Time, Region V Prevention Council, Chesapeake RU Ready coalition, Youth Partnership, Community and Schools Together, Hampton Roads Parenting Education Network, Suicide Prevention/Awareness and Youth Use of Alcohol, Tobacco and Other Drugs. The Out of School Time Committee is preparing for its first Out-of-School-Time Conference in May, partnering with several other city departments. Staff met with a representative who works with homeland security and the Chesapeake Youth Committee and discussed possible future partnership in spreading internet safety awareness. Staff presented Teen Dating Violence workshop to the Youth Advisory Board and the Chesapeake Public School Guidance Counselors (Middle and High School). The next STEP (Systematic Training for Effective Parenting) training began January 26, 2015 with 6 participants signed up. PAGE 8 OF 9

9 Staff is providing a Life Skills Group at Rivercrest Community Center, working with 4 th grade students. Staff continues to offer Girl Power at Oscar Smith Middle School, Deep Creek Middle School, Crestwood Middle School, Indian River Middle School and Joliff Middle School. Power UP is offered at Crestwood Middle School. Staff received positive feedback from Michell Sorey, Chief of Probation and Parole for Chesapeake for Mental Health First Aid Training provided to probation officers. Staff are partnering with CHKD, Chesapeake Public Schools, and other City departments to plan and coordinate a Chesapeake R U Ready? Family Night for citizens in March This FREE family night will focus on families with children 3-10 years old and provide workshops, resources and giveaways. Participants will receive important information on preparing for kindergarten, and making elementary and middle school transitions. Staff will partner with the Division of Community Programs to offer Internet Safety classes as part of their Safe Healthy Ethical Lifelong Lessons (SHELL) curriculum.
