1 CHARTER TOWNSHIP OF SHELBY REQUEST FOR PROPOSAL The Charter Township of Shelby is soliciting proposals to install fiber optic and data cabling to serve a new Police Building that is under construction as outlined below: PROJECT: PROJECT ADDRESS: and Van Dyke, Shelby Township, MI OWNER: Charter Township of Shelby, Van Dyke, Shelby Township, MI OWNER CONTACT: SCOPE OF WORK: REFERENCE PLANS: PROJECT TIMING: DUE DATE: QUESTIONS: Timothy Wood, Building Department Director Phone: (586) twood@shelbytwp.org The scope of work is defined in Attachment A - Scope of Work Reference plans are attached as follows: Attachment B Site Plan; Attachment C- New Police Building Floor Plan; and Attachment D Municipal Building Floor Plan The subject building is under construction and will likely be ready for this project to be undertaken in November, The ability of respondents to complete the work in a timely manner will be a consideration. Sealed proposals are due on or before Thursday, October 27, 2011, 3 p.m. local time to the owner, at the Charter Township of Shelby Municipal Offices Clerks Department, Van Dyke, Shelby Township, Michigan, Proposals received after this date and time will not be accepted. No oral, facsimile, or electronic proposals will be considered. Proposals shall be submitted along with Attachment E, a township supplied cost proposal form, in a sealed envelope labeled Fiber Optic and Data Cabling Project Proposal Shelby Township Police Building attention Terri Kowal, MMC Township Clerk. Submit three (3) complete copies of the proposal and the cost proposal form. Proposals received will be publicly opened and read aloud starting at 3:05 p.m. on Thursday, October 27, Any questions regarding this project shall be directed to the Owner Contact, Timothy Wood at (586) or via to twood@shelbytwp.org. Any requests for information or clarifications shall be made in writing via so that all prospective respondents may be notified. MANDATORY MEETING: A mandatory meeting to review the proposed work areas will be held on Thursday, October 20, 2011 at 3 p.m. local time at the offices of the Charter Township of Shelby Van Dyke, Shelby Township, Michigan, Page 1 of 10
2 Inasmuch as the work areas are under construction, attendees shall bring their own hard hat for safety purposes. Respondents are required to register for the meeting by providing an notification to the Owner Contact, Timothy Wood, at no later than Wednesday, October 19, 2011 at 3 p.m. local time. PROPOSAL: At a minimum, proposals shall include the following: Qualifications: A brief description of your firm, qualifications and direct experience with relevant projects. Technical: Describe in detail the specific services, materials and equipment your firm proposes that demonstrates your firm understands the scope of work. Describe in detail your firm s ability to complete the project. Provide a timeline for completion. Warranty: Provide detailed information on warranties offered for services, materials and equipment. Project References: Give name, address, phone number and address of client representatives for at-least three (3) projects you have completed which are similar to this project. Briefly describe the scope and budget of each referenced project. Sub-consultants or sub-contractors: Identify proposed sub-consultants and/or subcontractors who will be involved in the project, if any. Include the information described above for each sub-consultant and sub-contractor. Cost Proposal: Complete Attachment E, a township provided cost proposal form. Miscellaneous: Provide evidence of insurance coverage, statement of current claims/judgments and disclosure of conflicts of interest as outlined elsewhere in this request for proposal. RIGHTS RESERVED BY THE OWNER: The Owner reserves the right to waive any informalities in bidding, reject any or all bids, accept any bid when, in the opinion of the Owner such action will serve the best interests of the Charter Township of Shelby, consistent with competitive bidding requirements. No bid may be withdrawn after the above date and time for receiving bids for a period of ninety (90) days. The Charter Township of Shelby prefers that interested contractors have a business located in the State of Michigan, in Macomb, Oakland or Wayne Counties. CONFLICTS OF INTERESTS: All respondents agree to disclose any, direct or indirect, current or future, conflict of interest between themselves and the Charter Township of Shelby, and the employees of said entity. 3 ERRORS AND OMISSIONS IN PROPOSAL: CURRENT CLAIMS/ JUDGMENTS: CONTRACTURAL REQUIREMENTS: INSURANCE REQUIREMENT: The Charter Township of Shelby shall not be liable for any errors or omissions in proposals. The Charter Township of Shelby has the right to request clarification or additional information from the respondents. A statement shall be provided in the proposal regarding any claims, judgments, arbitration proceedings, or suits pending or outstanding against your firm or its officers, principals, employees, and/or sub-contractors under your firm s supervision. The selected firm shall be required to enter into a formal contract with the Charter Township of Shelby. The Charter Township of Shelby reserves the right to terminate the contract should the contractor fail to perform to the satisfaction of the Township. The selected firm shall provide professional liability insurance coverage prior to the beginning of any work, with the Charter Township of Shelby named as additional insured. Such insurance shall be maintained throughout the life of the contract. The selected firm shall carry no less than $1,000,000 general liability; $1,000,000 professional liability; and statutory workers compensation insurance. Page 3 of 10
4 Attachment A Scope of Work General Description: The project includes the installation of a new fiber optic cable from the IT room of the Township municipal office building to the IT room of a new police building under construction on the Township campus. The project also includes the installation of a data cable from a storage room within the township municipal office building to the IT room of the new police building. Further, the project includes the installation of data cabling, a relay rack, wall/surface mounted jacks and other related materials to serve the new police building. Details: Fiber optic cabling from the Township municipal building to the new police building 1. Furnish and install 12 strands of 62.5 micron multimode OSP rated fiber optic cable and 6 strands of single mode OSP rated fiber optic cable from the IT room of the Township municipal office building to the IT room (Room 152) of the new police building under construction on the Township campus. See the reference site and floor plans for the approximate location of the cables. 2. As an alternate to item #1 above, furnish and install 24 strands of 62.5 micron multimode OSP rated fiber optic cable and 12 strands of single mode OSP rated fiber optic cable from the IT room of the Township municipal office building to the IT room (room 152) of the new police building under construction on the Township campus. 3. The contractor shall provide interior/exterior conduits and mounting materials, as necessary, to facilitate routing the fiber optic cable from the Township municipal office building IT room server rack to connect into the owner supplied exterior underground conduits adjoining the municipal building mechanical room. The fiber optic cable shall then be routed in the township supplied underground conduits to the IT room (room 152) of the new police building. From there, the fiber optic cable shall be routed within the IT room to an owner provided ceiling ladder rack and down to a four post adjustable relay rack that shall be furnished and installed by the contractor (this same rack will be used for data cabling in the new police building see more information on the rack in the data cabling details for the new police building below). The rack shall be grounded and bonded. 4. Furnish and install fiber enclosures at each end of the fiber optic cable. The enclosures shall be rack mounted in the existing rack within the Township municipal building IT room and the new relay rack, furnished and installed by the contractor, within the IT room (room 152) of the new police building. 5. Furnish and install fiber adaptor panels within the fiber enclosure at each end of the fiber optic cable. Project related strands are to be terminated with SC style connectors into the adaptor panels. The optical attenuation for each mated field-installed pair shall not exceed 0.75dB per ANSI/TIA/EIA 568B standards. 6. The fiber optic cable ends shall be labeled. The labeling scheme shall be coordinated with the Shelby Township IT Department. 7. At the conclusion of the work, the fiber optic cable shall be tested to industry standards and the owner shall be provided with digital test results. Page 4 of 10
5 Attachment A (continued) Scope of Work Data cable from the Township municipal building to the new police building 1. Furnish and install an 18AWG-6 conductor-shielded wire, plenum rated, from the storage room (room 123) near the Township Clerk s offices, second level of the Township municipal building, to the IT room (room 152) within the new police building. 2. The contractor shall furnish and install interior/exterior conduits and mounting materials, as necessary, to facilitate routing the wire from the storage room (room 123) near the Township Clerk s offices, second level of the Township municipal building room, to connect into the owner supplied exterior underground conduits adjoining the municipal building mechanical room. The wire shall be routed in concealed ceiling spaces where accessible. From there, the wire shall be routed in the owner supplied underground conduits to the IT room (room 152) of the new police building. A six foot coil of wire shall be left at each end to enable connections by others. 3. At the conclusion of the work, the cabling shall be tested to industry standards and the owner shall be provided with digital test results. Installation of data cabling, relay rack, wall/ceiling jacks and other related materials in new police building 1. Furnish and install 100 Category 5e data cables from a relay rack within room 152 to work area locations identified on the new police building floor plan. 2. Furnish and install 12 analog Category 5e cables from a 110 block within room 152 to work area locations identified on the new police building floor plan. 3. Furnish and install a Category 5e 25 pair cable and #2 ground wire from room 152 to room 167 in a Township supplied conduit. Provide a 10 foot coil of cable and wire at each end to be terminated by others. 4. Cables shall be plenum rated and be installed splice free. Provide all necessary materials for mounting the cables above ceilings and routing them to Township supplied conduits at the work area outlets. As an option, the Township may consider using above ceiling ladder type cable trays in certain locations. Therefore, the Township requests a unit price (per foot) proposal to install ladder type cable tray, of aluminum construction, 6 spacing, 18 width, Monosytems, Blind, or approved equal. 5. Furnish and install Panduit executive style, or equal, faceplates for wall outlets and Panduit, or equal, surface mount boxes in modular furniture and in plant locations. 6. Data lines are to be terminated at the work area locations with Panduit, or equal, Category 5e rated jacks. 7. Each jack location shall be labeled to match a corresponding position in the IT room 152. The labeling scheme is to be coordinated with the Shelby Township IT Department. 8. The Category 5e cables are to be terminated onto Panduit, or equal, 24 port patch panels. Page 5 of 10

Provide front and back wire managers between patch panels for the routing of patch cords, as well as the incoming cabling The contractor shall furnish and install a 4-post adjustable relay rack (same one used for the fiber optic cable) that is 84 high, 19 wide and deep. The rack is to be fastened to the concrete floor using anchors at all four corners. The rack shall be UL listed as a communications accessory and be powder coated black. 11. Furnish and install an APC 9-outlet power strip on the rack to provide a location to plug in hubs, routers and other electronic equipment. 12. The rack shall be bonded by a 6 AWG bonding cable to the owner supplied grounding lug within the IT room At the conclusion of the work, the cabling shall be tested to industry standards and the owner shall be provided with digital test results.
