2 2014 COMMUNITY REPORT A Message from Our Chief Executive Officers As part of Hospital Corporation of America s North Florida Division, Fort Walton Beach Medical Center and Twin Cities Hospital are pleased to share our 2014 Community Report with you. We are committed to providing compassionate, high quality healthcare to the communities in which we work, live and play. Our first commitment is quality care for our patients a commitment that earned both our hospitals special recognition from The Joint Commission in Along with our commitment to quality is our commitment to compassion not only for our patients and staff, but also for our communities. As one of the area s largest private employers, with over 1,300 employees, we are proud of our employees investment of their time, talents and finances to non-profit agencies, including United Way and its various agencies, American Cancer Society, American Heart Association, Okaloosa County Schools, Chambers of Commerce, and more. Although reimbursement for services rendered is critical to the operations and stability of our hospitals, we recognize not all individuals have the ability to pay for medical services. In keeping with our commitment to serve the community and in respect to healthcare, we continue to provide free and reduced care to the indigent and to persons covered by governmental programs at below-cost, such as Medicaid and subsidized health services. Last year this amounted to over $22,749,597 in charity and uncompensated care. We thank our Boards of Trustees, Medical Staffs, Employees, and Volunteers for their dedication in providing the best care, service and support to our patients, visitors and their families. We were enjoying a normal day at home October 25, 2013, when our 4-year-old daughter Beth swallowed a marble. I successfully performed the Heimlich maneuver, but when I did a finger sweep to clear the marble, I discovered it had not come out. I called 911 and the ambulance took us to Twin Cities Hospital. Beth was breathing independently, but with some difficulty. Dr. Hughes made her comfortable while x-rays were taken. The marble had traveled to the cavity connecting the nose and trachea and still threatened her airway. Dr. Hughes consulted with an ENT Physician and together they developed a plan. Dr. Hughes and the medical professionals at Twin Cities Hospital were able to safeguard Beth s airway, and the marble was removed with no negative or lasting effects to Beth. The staff at Twin Cities Hospital kept Beth calm while reassuring my husband and I that everything was going to be okay. We were allowed us to stay in the room with Beth throughout the entire visit. Today, Beth is back to her lively, vivacious self and we can t thank Twin Cities Hospital enough for the wonderful care our daughter received that day.

6 4 Giving Back to Our Community Community Support Our facilities and staff members provided both financial and volunteer support to multiple organizations including colleges, schools, and non-profit and charitable groups along the Emerald Coast. Our partnership with area nursing programs continued to play a role in the education and development of the next generation of healthcare professionals. In 2013, hundreds of nursing, pharmacy, radiology, dietary and paramedic students trained at Fort Walton Beach Medical Center and Twin Cities Hospital under the guidance of our nurse and staff preceptors. We also provide meeting space to many area support groups. In 2013, individuals throughout the area improved the quality of their lives through the following support groups: Bosom Buddies, Emergency Management Services, Folks with Strokes, Look Good Feel Better, Prostate Awareness, Ostomy, Smoking Cessation, Trauma Intervention Program, Twin Cities Ministerial Association, and more. Volunteering Our staff and physicians believe in going beyond one s personal duties to help others in need. From providing free medical care within our communities and in areas outside our country where medical needs are non-existent or underserved, to serving on local United Way Agencies or other non-profit organizations that benefit humanity and our communities, we are committed to making a difference in the lives of others, both inside and outside our HCA facilities COMMUNITY REPORT A Healthier Community Improving the healthcare of our community is part of our hospital mission. Our hospitals actively participated in area health fairs, often providing blood pressure checks and glucose checks free of charge to thousands of individuals. We also partnered with the area Sickle Cell Foundation to assist in screening individuals who are genetically predisposed to carry the disease, as well as speaking at their seminars and classes to educate students and patients. Our speakers bureaus educated community groups on a variety of health topics, including heart disease, cancer, stroke, diabetes and drug safety to name a few. In addition, Fort Walton Beach Medical Center hosted its annual Cancer Survivor Day. Fort Walton Beach Medical Center and Twin Cities Hospital partnered with Northwest Florida State College to host the 2 nd Annual Women on Wellness Symposium a one day event dedicated to enriching the health and wellness of women in our communities. Our hospitals also played active roles in the United Way Days of Caring, American Heart Association Heart Walk, March of Dimes March for Babies, and American Cancer Society s Relay For Life. 7 OUR COMMITMENT TO OUR COMMUNITY 5 Awards and Recognition Fort Walton Beach Medical Center and Twin Cities Hospital are accredited by The Joint Commission, a national surveyor of healthcare organizations. The Joint Commission accredited hospitals have demonstrated they meet the highest standards in their industry by passing an in-depth, on-site facility survey. Fort Walton Beach Medical Center and Twin Cities Hospital have earned The Joint Commission s Gold Seal of Approval, a visible symbol of our commitment to providing the highest level of care. The Joint Commission named Fort Walton Beach Medical Center and Twin Cities Hospital as Top Performers on Key Quality Measures for the third consecutive year. Fort Walton Beach Medical Center received recognition for heart attack, heart failure, pneumonia and surgical care, and Twin Cities Hospital received recognition for pneumonia and surgical care. Teamwork between all disciplines involved is demonstrated in this recognition as a Top Performer in Core Measures to ensure our patients received the highest quality of care, treatment and services they deserve and expect. The Okaloosa County School District s partnership with Fort Walton Beach Medical Center and Twin Cities Hospital provides more wellness opportunities for our teachers and staff. 8 6 Quality and Patient Safety Despite our achievements, Fort Walton Beach Medical Center, Twin Cities Hospital and the HCA North Florida Division recognize the importance of patient safety and continue to make it paramount.our initiatives ensure the safety and quality of care given to our patients each day. Pediatric Early Warning Scoring System (PEWS) In 2012, HCA s Clinical Services Group introduced an early warning system addressing adult patients (MEWS). A corresponding Pediatric Early Warning Scoring System (PEWS) was implemented in Hospitals have realized that many codes in general care areas are preventable. Warning signs associated with patient deterioration may occur up to 8 hours prior to a code event. Use of rapid response teams has decreased the risk of respiratory and cardiopulmonary arrest outside of the (P)ICU. The challenge lies in identifying early signs of deterioration and intervening. PEWS provides assessment, early intervention and stabilization for pediatric patients, prevents clinical deterioration or arrest outside of the PICU, and triggers a Rapid Response by a team of critical care experts. The system also provides a definitive way to get the necessary attention at the time needed by presenting quantifiable evidence of deterioration and a standardized means of communication that can be used to prioritize workload. PEWS has been validated for use with hospitalized pediatric patients, newborn to 17 years, on medical/surgical units COMMUNITY REPORT TheraDoc: Infection Prevention Surveillance System TheraDoc was piloted in 2012 at six locations and deployed more broadly this year. It is designed to shift Infection Prevention s time from clerical to clinical tasks by educating clinical staff on infection prevention interventions, accurately submit data to the Centers for Disease Control and Prevention s National Healthcare Safety Network database for purposes of the Centers for Medicare and Medicaid Services and state public reporting requirements, and reduce the occurrenceof hospital acquired infections and hospital outbreaks by providing system-wide analysis of facility, market, division, and provider level data. TheraDoc supports reducing hospital acquired infections by the following measures: Real-time alerting: Automated alerts ( /pager) notifying of potential infections and communicable diseases (Influenza, TB). Pre-built reports: TheraDoc identifies infection related trends: common hospital locations, common physician, common procedures, common nursing staff, etc., to allow for near real-time drill down and follow-up. Reporting: Transparency of infection related events at the local facility, market, division, and corporate via access to reports. Surveillance: Facilities currently monitor ICU patients only. TheraDoc allows for hospital-wide surveillance. Results from the initial pilot included a 30% reduction in the rate of central line associated bloodstream infections (CLABSI), along with a 50% reduction in the number of CLABSI infections year over year. Catheter associated urinary tract infection (CAUTI) rates experienced a 20% reduction and the number of CAUTI infections were reduced by 36%. Through adoption and implementation of PEWS, our goal is to demonstrate the effectiveness of timely assessment and appropriate intervention during events in which changes in the pediatric patient s condition warrants critical response and evaluation.

9 OUR COMMITMENT TO OUR PATIENTS 7 Telemedicine Initiative As part of the North Florida Division s Outreach Initiative, telemedicine services for stroke patients were implemented at Gulf Coast Regional Medical Center (GCRMC). The division partnered with InTouch Health to provide 24/7 stroke care to patients. The program allows physicians to remotely consult with patients, family members, and other healthcare providers located in rural hospital ERs at a moment s notice from anywhere in the world. Through an Internet-based connection to the RP-Lite robot, physicians are able to provide real-time audio/video consultations at the patient s bedside. By getting immediate access to a GCRMC neurologist, patients may be eligible to receive time-critical medications such as tissue plasminogen activator (tpa), which can save brain function and reduce disability. As a result, emergency physicians can more quickly treat stroke patients in the rural ER setting without requiring transfers to another facility, and reduce or eliminate long-term stroke impacts on patients. Affiliation agreements with several hospitals in the outlying area surrounding GCRMC were established, increasing access to these vital services. In the coming year, this program will be implemented at several additional North Florida Division facilities, greatly enhancing the region s ability to care for stroke patients. Meaningful Use Patient Portal During 2013, a great deal of preparation was done in order to launch the Meaningful Use (MU) Patient Portal in early 2014 across the organization. The goals for the portal are to improve the patient experience, encourage patient engagement, and create the technology foundation for meeting MU Stage Two requirements. The MU Patient Portal will be available online, on-demand and will reflect select portions of the patient s medical record at the hospital. 10 8 Specialized Care Emergency Excellence Highly skilled emergency physicians and nurses, combined with best practices and the latest technology, benefitted nearly 156,000 residents and visitors last year. Our protocol-driven care that begins in the field, through direct communications with local EMS teams, meets stringent standards ensuring emergent medical needs are provided timely. Our strong focus on enhancing care and improving ER processes has also dramatically reduced average wait times to below the national average. In fact, we take so much pride in our reduced average ER wait times, our current Average ER Wait Times are posted on electronic billboards throughout our communities, available on our websites at FWBMC.com and TCHospital.com, by texting ER to 23000, and through itriage smartphone technology for iphone and Android. Both Fort Walton Beach Medical Center and Twin Cities Hospital are leading the way in making this important time decision criteria available to residents and visitors of our communities through these innovative technologies COMMUNITY REPORT Destin Emergency Care Center Destin Emergency Care Center (DECC) is located at 996 Airport Road, less than one mile north of Highway 98, across from the Destin Airport. The DECC assesses and treats all ages, from neonatal to geriatric patients. With an emphasis on timely assessment and treatment, the DECC staff are skilled in the specific age-related needs of these diverse populations in this clinical setting. The DECC is equipped to handle medical, pediatric and/or psychiatric emergent health care needs. Patients requiring an advanced level of care will be stabilized and transported to the facility of their choice which has the required services available. Pediatric Emergency Room Fort Walton Beach Medical Center opened its new Pediatric Emergency Room in April It is the only one of its kind in the tri-county area including Okaloosa, Walton and Santa Rosa Counties. The Pediatric ER is a separate emergency room open 24/7 that is dedicated solely to treating the emergency healthcare needs of infants, children and adolescents from birth to 15 years of age. The Pediatric ER recognizes that children s medical emergencies require a special level of care. Our child-friendly clinicians are trained to help children cope with medical emergencies.

11 OUR COMMITMENT: SPECIALIZED CARE 9 Heart Care The Heart Center at Fort Walton Beach Medical Center is the Tri-County s only comprehensive cardiovascular program. The Heart Center at Fort Walton Beach Medical Center provides the latest and most advanced treatments in cardiac care, including cardiac catheterization with coronary angioplasty and stents. These advanced cardiac treatments ensure that life-saving technology and treatments are available right here in our community. The American College of Cardiology and the Centers for Medicare and Medicaid Services have established guidelines for how soon a hospital must treat a patient having a heart attack once they arrive at the hospital. This is often referred to as the door to balloon time. Fort Walton Beach Medical Center s Emergency Department and Catheterization Lab Team work closely together in meeting this standard of care. During 2013, the Medical Center s heart attack response team s average door to balloon time was well below the national recommended standard of 90 minutes. Average times at Fort Walton Beach Medical Center ranged from 46 minutes to 54 minutes. The team s motto is our hustle saves your heart muscle. For patients recovering from cardiovascular surgery and interventional cardiology procedures, Fort Walton Beach Medical Center offers a cardiac rehabilitation program which assists the cardiac patient in achieving and maintaining a state of optimal health and daily living activities through education, regular exercise, dietary counseling and lifestyle changes. 12 COMMUNITY REPORT Comprehensive Oncology Program Fort Walton Beach Medical Center is an associate of the UAB Health System Cancer Community Network. We re leading the local fight against cancer and are able to provide enhanced access to care prevention, early detection, education and screenings. The UAB Health System Cancer ASSOCIATE OF HEALTH SYSTEM CANCER COMMUNITY NETWORK Community Network, developed by UAB Medicine and the UAB Comprehensive Cancer Center, is a network of hospitals across Alabama, Florida and Georgia that provides access to the latest discoveries in cancer research and delivers an unmatched level of expert cancer treatment. Oncology Patient Navigators The Oncology Patient Navigators at Fort Walton Beach Medical Center are with patients through every step of their diagnosis and treatment. Sarah Lindsay, RN, OCN and Certified Oncology Patient Navigator, and Buddy Duncan, a former hospital chaplain who now serves as our Community Navigator, have teamed up to help patients and their families navigate through the medical environment, knocking down some of the barriers caused by medical jargon and the complex nature of health care delivery. Our Patient Navigators will meet with you and your caregivers to offer education, referrals and support services. Their outreach extends beyond the hospital and is a resource for people who may have received a diagnosis, but are not yet at a point where hospital services are required. Our Oncology Patient Navigators also help confirm doctor s appointments and even go with them to appointments as a co-pilot. They serve as personal guides and facilitators in dealing with the emotional and financial challenges associated with cancer. Diagnostic Imaging The Diagnostic Imaging Service Departments at both Fort Walton Beach Medical Center and Twin Cities Hospital are honored to be leaders in non-invasive medical imaging. Our teams of Radiologists and medical professionals utilize the latest tools in identifying various medical conditions through the use of the latest in radiologic technology. FWBMC installed the new MRI Suite, GE Optima MR450w 1.5T Magnet, while Twin Cities Hospital installed the GE BRIVO 355MR. Both machines provide advanced imaging quality, as well as quiet technology and increased patient comfort. 13 OUR COMMITMENT: SPECIALIZED CARE 11 Orthopedic & Spine Surgery Programs Fort Walton Beach Medical Center and Twin Cities Hospital s Certified Orthopedic Joint Replacement Programs have earned The Joint Commission s Gold Seal of Approval, with Twin Cities Hospital being the first facility in the area to receive this designation. The disease-specific accredited program is designed with the patient in mind and consists of a preoperative educational class. The class allows the patient to prepare for and be knowledgeable of the care they will receive during their hospital stay. A multidisciplinary team of physicians, nurses, physical therapists, and case managers work together to ensure quality care throughout the patient s stay. In addition to general orthopedic surgery, our orthopedic surgeons offer advanced procedures including minimally invasive knee and hip resurfacing to relieve pain and help patients return to normal activities. Additionally, we offer comprehensive inpatient and outpatient rehabilitation services for recovering orthopedic surgical patients. Robotic Surgery Fort Walton Beach Medical Center provides the community with a $2.2 million da Vinci Si robotic surgical system. The da Vinci Surgical System provides surgeons with an alternative to both traditional open surgery and conventional laparoscopy, putting a surgeon s hands at the controls of a state-ofthe-art robotic platform. The da Vinci System features a magnified 3D high-definition vision system and special wristed instruments that bend and rotate far greater than the human wrist. Together, the da Vinci System and Single-Site instruments allow physicians to perform gallbladder surgery through a single incision in the naval. The da Vinci lobectomy surgery is also a minimally invasive procedure, with only a few small incisions between the patient s ribs, which offers precise removal of potentially cancerous tissue. For the patient, benefits may include significantly less pain, less blood loss, less scarring, shorter recovery time, a faster return to normal daily activities, as well as better clinical outcomes. Fort Walton Beach Medical Center s Spine Surgery Program is recognized by Becker s Review as a Top 101 Spine Program in the nation. Our team of highly trained Neurosurgeons, operating room staff, nurses and rehabilitation specialists work collaboratively to ensure spine surgery patients receive the high caliber of care and outcomes.

14 COMMUNITY REPORT Women s Services Fort Walton Beach Medical Center and Twin Cities Hospital are committed to helping women with their healthcare needs through every phase of life.
