2 Athletic Plicies and Prcedures Perspectives Charter Schls Athletic Plicies and Prcedures have been prepared t make infrmatin readily available t caches, student-athletes, and parents, as well as make athletics at Perspectives Charter Schls successful. Perspectives has pledged its supprt t the Athletic Department by prviding resurces t maintain the facilities and equipment at each campus. It is the respnsibility f the student-athlete t preserve and use these items in gd judgment. After reading these plicies and prcedures, the student-athlete and parent/guardian shuld sign the handbk acknwledgment frm at the back f the student handbk. These signatures indicate that the student-athlete and parent/guardian agree and will cmply with the plicies and prcedures described within. The parent/guardian signature als serves as permissin fr their sn r daughter t participate n an athletic team. Philsphy The verall philsphy f Perspectives Charter Schls Athletic Department is cnsistent with its unique character-building curriculum A Disciplined Life. This curriculum guides the schl's effrts t address every student-athlete as a whle persn, and prvide him r her with skills that will supprt the transitin t higher educatin and beynd. Perspectives teaches and reinfrces the principles f A Disciplined Life n and ff the playing field. Student-Athlete Academic Eligibility Plicy High Schl Student-Athlete Eligibility Unless they are entering high schl fr the first time, high schl student-athletes are eligible fr athletic participatin cnsideratin if they have received credit fr cmpleting at least five (5) classes f high schl wrk, as apprved by Perspectives Charter Schls in the previus semester. Additinally, high schl student-athletes must achieve a 2.0 GPA r better, with n mre than ne failing grade n their end-f- quarter reprt cards, during their sprts seasn. Student-athletes must als have achieved a C r better in A Disciplined Life class. These reprts are received every ten (10) weeks. The prir quarter s grades will be used t determine athletic eligibility fr sprts which start in the subsequent quarter. Fr sprts starting in Quarter 1, Quarter 4 grades frm the previus year will be used t determine academic eligibility. Fr Quarter 2 and Quarter 4 grades, current semester ending grades can als be used t determine eligibility. If a high schl student-athlete des nt meet the abve requirements, he r she will be placed n athletic-academic prbatin, and will be allwed t practice, but must sit ut any cntests and nt receive athletics-related early dismissals until he r she is meeting the abve-mentined eligibility. Student-athletes n athletic-academic prbatin, with a GPA between 1.50 and 1.99, will be eligible fr reinstatement after three (3) weeks. Student- athletes n academic-prbatin, with a GPA between 0.00 and 1.49, will have their athletic eligibility reviewed after five (5) weeks, nce prgress reprts r quarter grades are released. 2

3 Student-athletes with a 2.0 GPA r better, with n mre than ne failing grade and a grade lwer than a C in A Disciplined Life class, will be eligible fr reinstatement after three (3) weeks. Student-athletes with a 2.0 GPA r better, with mre than ne failing grade, will be eligible fr reinstatement after three (3) weeks. T gain reinstatement, ineligible student-athletes must attain the academic standards listed in the current plicy. 9 th grade high schl student-athletes wh are cmpeting in athletics during Quarter 1 will have their academic eligibility determined after Quarter 1 grades have been released. Until that pint, all 9 th grade high schl student-athletes are eligible fr athletic participatin. Middle Schl Student-Athlete Eligibility Middle schl student-athletes must be passing all classes with n mre than ne grade lwer than a C n their end-f-quarter reprt cards. Student-athletes must als have achieved a C r better in A Disciplined Life class. These reprts are received every ten (10) weeks. Student-Athlete Expectatins Cnsistent with the principles f A Disciplined Life, all student-athletes must: Demnstrate hnesty, integrity, and decency by adherence t all rules set frth by the cach, Student Handbk, and the Illinis High Schl Assciatin (IHSA) By-Laws. Take respnsibility fr yur actins by understanding that athletic participatin is a privilege, and as a student-athlete, yu becme a representative f Perspectives Charter Schls. As such, yu are expected t behave in an apprpriate manner, bth n and ff the field. Accept nly quality wrk frm yurself by realizing the imprtance f academic excellence and prper behavir. This includes: Making up any missed wrk Attending study tables r tutring Serving any detentins r ther schl discipline Being a psitive rle mdel t all students in the schl Live a healthy lifestyle by nt using r pssessing tbacc, alchl, sterids, r ther illegal drugs. 3

4 Be psitive and supprtive f each ther by encuraging yur teammates and respecting all ther participants including ppnents, fans, caches, and referees. Respect each ther s differences by refraining frm hazing, defined as any act f humiliatin, r a rite f passage which causes r is likely t cause physical r mental harm r persnal degradatin f anther student. Any breach f the plicies listed in the Student-Athlete Expectatins sectin will result in a review f the student-athlete s athletic eligibility. This review will be cmpleted by schl administratin and the athletic directr. Expectatins f Caches Caches are respnsible fr: Maintaining, enfrcing, and mdeling the principles f A Disciplined Life Infrming parents/guardians and students in writing f the lcatin and time f all practices and cntests, team requirements, team fees, special equipment needs, and any ff-seasn cnditining plans Playing nly eligible student-athletes Distributing, catalging, and the cllectin f unifrms Being respectful t parents/guardians, student-athletes, and the student-athletes and caches frm ther teams, and referees Their team being respectful t parents/guardians, student-athletes, and the studentathletes and caches frm ther teams, and referees Other duties as determined by the athletic directr r principal Expectatins f Parents/Guardians Parents/Guardians and caches wrk tgether t help develp yung men and wmen t becme respnsible, hnrable adults. Winning and lsing is secndary t student grwth and develpment. Parents/Guardians are invited t help build ur teams by: Bringing enthusiastic and respectful fans t events Fundraising fr team needs Cunseling their child thrugh the ups and dwns f cmpetitin Parents/Guardians are welcme t make an appintment with a cach t address questins r cncerns. If a parent/guardian has truble getting an appintment with a cach, r needs further clarificatin, they shuld cntact the athletic directr r principal. Cmmunicatin t expect frm yur cach: Perspectives Charter Schls athletic philsphy Expectatins fr yur child and fellw student-athletes Dates, lcatins, and times f all cntests/practices Sprt-specific requirements (e.g., specific fees and/r special equipment, ff-seasn cnditining) Prcedures t be fllwed, shuld yur child be injured during a cntest/practice Discipline resulting in the reductin and/r eliminatin f yur child s participatin 4

5 Cmmunicatin expected frm a parent/guardian: Cncerns abut yur child s behavir, treatment/mistreatment, expectatins r the athletic philsphy (fllwing prcedures listed belw) Ways t help yur child imprve Advanced ntificatin f any schedule cnflicts interfering with yur child s ability t participate in a cntest/practice Perspectives believes in partnering with ur parents, yet there are sme tpics that are inapprpriate t discuss with yur child s cach. Tpics left t the discretin f the cach: Yur child s playing time Strategies fr the team Plays and referee calls Persnal infrmatin abut ther student-athletes Reprting a Cncern Please d nt cnfrnt a cach during a cntest/practice Call the Athletic Department t set up an appintment with yur child s cach Cme in during yur scheduled appintment t discuss yur cncerns with the cach If the cach des nt prvide satisfactry reslutin, cntact the athletic directr t discuss the situatin further and t set up a meeting. At this meeting, the apprpriate next steps can be determined. Physical Exam and Emergency Cards T participate in athletics, a valid physical examinatin, a red emergency red card, and a blue parental cnsent card must be n file with the athletic department n r befre the first day f tryuts/practice. Student-athletes will nt be allwed t practice r try ut until a valid physical is n file in the athletic department. Per Illinis High Schl Assciatin (IHSA) rules, the student-athlete's physical examinatin must be perfrmed by a licensed physician, physician s assistant, r nurse practitiner as set frth in the Illinis State Statutes n mre than 395 days prir t participatin in any such practice, cntest, r activity. Perspectives strngly encurages all individuals wh plan t participate in interschlastic athletics t cmplete their physical exam between late June and early August. Cmpleting the exam during this time will avid ineligibility issues. Athletic Injuries & Health Insurance Health insurance cverage fr Perspectives Charter Schls student-athletes is the respnsibility f the student-athlete s parent/guardian. Perspectives Charter Schls realizes there are certain risks f physical injury thrugh the participatin in all athletic activities, and the parent/guardian must agree t assume the full risk f any and all injures r damages, regardless f severity that the participant may sustain as a result f athletic participatin. This understanding is detailed in the Athletic Department Parent/Guardian Waiver. The permissin frm must be cmpleted and signed by the parent/guardian t allw their student-athlete t participate in athletics at Perspectives Charter Schls. 5

6 If the parent/guardian des nt carry health insurance fr their student-athlete, they must cntact the athletic department t enrll in a student activity insurance prgram at their wn expense. Cut Plicy The number f student-athletes t be kept n an individual team will be determined by the cach and the athletic directr prir t any cuts being made. Individual caches shall have the sle respnsibility f selecting the members f a team. If the decisin is made t cut students frm a team, the fllwing prcedures will take place: Students will be infrmed that cuts will be made (e.g., annuncements, psting n bulletin bards, etc.) At least tw (2) practices will be required befre cuts are made Caches will determine what factrs will be used t cut students (e.g., ability, attitude, wrk ethic, academic perfrmance, etc.) Student-athletes will be infrmed f being cut in a way that will avid persnal embarrassment t the player (e.g., psted annymusly by number r letter cde) Equipment Student-athletes are respnsible fr each item f equipment that is issued t him r her. Lst r stlen equipment must be paid fr at the replacement cst. A student-athlete will nt be allwed t receive awards, participate in a subsequent sprt, r receive his r her diplma until the equipment recrd has been cleared. Student-athletes are strngly encuraged t nt abuse the equipment. Much mney is spent t supply the best equipment fr their use and safety: please treat all equipment with respect. Transprtatin Please be aware f Perspectives Charter Schls' plicy cncerning the transprtatin f studentathletes t and frm cmpetitins. Perspectives Charter Schls prvides transprtatin t and frm all athletic cntests. All student-athletes are required t use schl transprtatin. Studentathletes may nt participate in an away cntest if they drive themselves t the event r if they use alternate transprtatin withut prir permissin frm the cach. In extenuating circumstances, parents/guardians may need t drive their wn children t r frm a cntest; but caches need written ntificatin and apprval frm the cach, granted in advance. Parents/guardians and/r relatives f student-athletes are encuraged t attend all hme and away cntests in an effrt t shw schl supprt. Transprtatin fr parents/guardians and/r relatives is the respnsibility f the parent/guardian and/r relative and nt that f Perspectives Charter Schls. Perspectives des nt prvide additinal transprtatin, and it is strictly prhibited fr parents/guardians and/r relatives t ride n schl-prvided transprtatin with studentathletes. Students will be chaperned while riding in schl-prvided transprtatin by their caches and caching assistants. Awards Athletic awards are given n the basis f participatin, lyalty, cperatin, and excellence in perfrmance. Each sprt yur child participates in has specific award expectatins. Prir t the start f the seasn, caches will review award expectatins with student-athletes. 6

7 Cncussin Infrmatin A cncussin is a brain injury and all brain injuries are serius. They are caused by a bump, blw, r jlt t the head, r by a blw t anther part f the bdy with the frce transmitted t the head. They can range frm mild t severe and can disrupt the way the brain nrmally wrks. Even thugh mst cncussins are mild, all cncussins are ptentially serius and may result in cmplicatins including prlnged brain damage and death if nt recgnized and managed prperly. In ther wrds, even a ding r a bump n the head can be serius. Yu cannt see a cncussin, and mst sprts cncussins ccur withut lss f cnsciusness. Signs and symptms f cncussin may shw up right after the injury r can take hurs r days t fully appear. If yur child reprts any symptms f a cncussin, r if yu ntice the symptms r signs f a cncussin yurself, seek medical attentin right away. Symptms may include ne r mre f the fllwing: Headaches Pressure in head Nausea r vmiting Neck pain Balance prblems r dizziness Blurred, duble, r fuzzy visin Sensitivity t light r nise Feeling sluggish r slwed dwn Feeling fggy r grggy Drwsiness Change in sleep patterns Amnesia Dn t feel right Fatigue r lw energy Sadness Nervusness r anxiety Irritability Mre emtinal Cnfusin Cncentratin r memry prblems (frgetting game plays) Repeating the same questin/cmment Symptms may include ne r mre f the fllwing: Appears dazed Vacant facial expressin Cnfused abut assignment Frgets plays Is unsure f game, scre, r ppnent Mves clumsily r displays in crdinatin Answers questins slwly Slurred speech Shws behavir r persnality changes Cannt recall events prir t hit Cannt recall events after hit Seizures r cnvulsins Any change in typical behavir r persnality Lses cnsciusness What can happen if my child keeps n playing with a cncussin r returns t sn? Athletes with the signs and symptms f cncussin shuld be remved frm play immediately. Cntinuing t play with the signs and symptms f a cncussin leaves the student-athlete especially vulnerable t greater injury. There is an increased risk f significant damage frm a cncussin fr a perid f time after that cncussin ccurs, particularly if the athlete suffers anther cncussin befre cmpletely recvering frm the first ne. This can lead t prlnged recvery, r even t severe brain swelling (secnd impact syndrme) with devastating and even fatal cnsequences. It is well knwn that adlescent r teenage athletes will ften fail t reprt symptms f injuries. Cncussins are n different. As a result, educatin f administratrs, caches, parents, and students is the key t studentathlete safety. 7

8 If yu think yur child has suffered a cncussin Any athlete even suspected f suffering a cncussin shuld be remved frm the game r practice immediately. N athlete may return t activity after an apparent head injury r cncussin, regardless f hw mild it seems r hw quickly symptms clear, withut medical clearance. Clse bservatin f the athlete shuld cntinue fr several hurs. IHSA plicy requires athletes t prvide their schl with written clearance frm either a physician licensed t practice medicine in all its branches, r a certified athletic trainer wrking in cnjunctin with a physician licensed t practice medicine in all its branches, prir t returning t play r practice fllwing a cncussin, r after being remved frm an interschlastic cntest due t a pssible head injury r cncussin and nt cleared t return t that same cntest. In accrdance with state law, all IHSA member schls are required t fllw this plicy. Parents/guardians shuld als infrm their child s cach if they think their child may have a cncussin. Remember: it is better t miss ne game than miss the whle seasn. When in dubt, the athlete sits ut. Fr current and up-t-date infrmatin n cncussins, visit Waiver and Release f All Claims Athletic Waiver and Release Infrmatin As a Perspectives student-athlete, and/r the parent/guardian f a student-athlete, I recgnize and acknwledge that there are certain risks f physical injury while participating in athletic activities. As the student-athlete, and/r parent/guardian f the student-athlete, I vluntarily agree t assume the full risk f any and all injuries, damages r lss, regardless f severity, that I, as the student-athlete, my minr child/ward, r I, as the parent/guardian, may sustain as a result f said participatin. I further agree t waive and relinquish all claims that I as the student-athlete, my minr child/ward, r I, as the parent/guardian, may have (r accrue t me r my child/ward), as a result f participating in these activities against Perspectives Charter Schls, including its fficials, agents, vlunteers, and emplyees. Bth the student-athlete and the parent/guardian understand that students must cmply with the prvisins f the Student Handbk and ther rules f cnduct established by Perspectives Charter Schls while participating in these activities. By signing the Perspectives Charter Schls Student Handbk Acknwledgment Sheet, I agree t the fllwing: I hereby fully release and frever discharge Perspectives Charter Schls frm any and all claims fr injuries, damages, r lsses that, I as the athlete, my minr child/ward, r I, as the parent/guardian, may have, r which may accrue t me r my minr child/ward arising ut f, cnnected with, r in any way assciated with these activities.
