About this role The Hospital Afterhours program at Ipswich Hospital was introduced to ensure all patients that are either on shift during day, afternoon or night are cared for by the right person with the right skills at the right time, optimising patient safety and ensuring that this service delivers patient centred care aligned to the values within the health service. The Hospital Afterhours program consists of two parts; the Hospital at Night program and the Afterhours Nurse Unit Manager (NUM). The Hospital at Night program was introduced in May 2014 to ensure patients who are admitted after hours were in a safe system of care that focuses on reducing variation, standardising practice and providing multi-disciplinary clinical and operational leadership at night. The Grade 7 Clinical Care Coordinator provides nursing leadership, clinical governance, case management and complex clinical care and support for patients within a multi-disciplinary Hospital at Night Team at Ipswich Hospital. The role includes the key coordination functions of complex evidence based care delivery across a continuum, business and materials management, and education provision for patients and their family / carers and staff from 20:00Hrs to 06:30Hrs seven days a week. The Grade 7 Afterhours NUM is part of the Hospital Afterhours program at Ipswich Hospital who provides nursing leadership, clinical care coordination, case management and review of complex clinical care and support for patients and staff rostered afterhours. The role involves the Afterhours NUM being a clinical resource person, helping with problem solving to resolve complex patient situations and provide clinical leadership and mentorship from 13:00-21:30hrs, Monday to Friday throughout Integrated Care Stream and Saturday and Sunday throughout all of Ipswich Hospital. The key accountabilities of the role are: Provide an expert clinical opinion, consultancy, advice, and direct care for patients and their family/carer afterhours at Ipswich Hospital. Lead the Hospital at Night Program and the After-hours NUM role including joint medical, nursing and operational staff clinical handover, safety briefs and routine scheduled rounds of Ipswich Hospital in partnership with the Central Patient Flow Unit s Nurse Manager. Provide direction and prioritisation of clinical care to hospital at night response team consisting on nursing and medical staff in partnership with the identified Medical Clinical Care Coordinator. Provide senior support and guidance to the nursing staff, increasing and maintaining staff s knowledge and skills relating to the care of designated client group through the provision of just-in-time and scheduled education and access to appropriate and contemporary learning resources. Model nursing leadership within nursing and inter-professional teams by aligning decisions and resource utilisation with vision, goals and evidence which supports effective patient-centred care, service planning and development, and fiscal integrity afterhours. Work within the Central Patient Flow Unit Team in a collaborative partnership model with the Nurse Manager for Patient Flow. Ability to respond and manage emergent environment, patient and staff safety risk after hours. Provide clinical governance, safety, quality and value to consumers and the organisation through compliance with relevant standards and frameworks and continuous improvement initiatives. Maintain the patient of concern list and provide responsive care requirements to support the inpatient teams to respond to activation of Ryan s Rule by patients family/carers. Actively contribute to maintaining a safe workplace that values the health and safety of co-workers, clients or visitors. Actively contribute to developing and maintaining a culture where staff are vigilant to risks to themselves, their co-workers, clients or visitors (including health and safety, business and operational risks).

2 Actively participate in the Health Service Performance Planning and Appraisal and Individual Development Planning processes. Staffing and budget responsibilities The position is accountable for the operational nursing workforce and resource utilisation within the Hospital Night Team and the Afterhours NUM role, including the Grade 6 Hospital at Night Response Nurses. In conjunction with the Patient Flow Nurse Manager, participate in the performance appraisal and individual development processes, or performance improvement processes, for staff within the designated clinical / portfolio area. Within operational role functions, the role is accountable for identifying the quantum of resources required for service delivery and the effective and efficient utilisation of resources to support key performance indicators and fiscal imperatives. There will be a requirement to rotate between the Hospital at Night Clinical Care Coordinator position which is a night duty position to lead the Hospital at Night Team and to provide expert senior nursing skills to support staff and provide direct clinical care after hours from 20:00hrs seven days a week and the Afterhours NUM role from 13:00-21:30hrs Monday Sunday. There may be a requirement to rotate into the Nurse Manager of the Nursing & Midwifery Resource Team that operates Monday to Friday 07:30-16:00Hrs. There may be a requirement to rotate into the Patient Flow Nurse Manager role which operates on a 24 hour clock with 12 hour shifts. Attributes required for effective performance in this role The information in this section is used to assess candidate suitability to perform effectively in the role. Mandatory Qualifications / Professional Registration: Appointment to this position requires proof of qualifications and registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of clinical duties. Completion of Advanced Life Support and Paediatric Life Support. Demonstrated competence and clinical experience within the relevant clinical specialty preference to critical care background or at an advanced level for a minimum of five years. While not mandatory, a relevant qualification (e.g. Advanced Clinical Nursing; Health Management / Leadership; Education) will be well regarded. This position is required to support consumers and staff within a designated clinical specialty across the hospital and Health Service. As a result, the possession of a licence to operate a class C motor vehicle is mandatory, as the incumbent is expected to deliver these services at the most appropriate facility. Demonstrated advanced skills to lead a multi-disciplinary team including medical, nursing and operational staff. Demonstrated knowledge and skills relating to human resource management and financial literacy and their application to the healthcare environment Demonstrated legislation / policy knowledge and the ability to develop documents supporting the application of same within healthcare workplaces.
