1 APPLICANT GUIDELINE All Scholarship Applicants must: 1) BE a graduating high school student enrolled in a Mecklenburg County school (public or private) Note: Delta Sigma Theta, Charlotte Alumnae Chapter Debutantes are ineligible for chapter sponsored scholarships. 2) SUBMIT a completed application packet with the following information: o A completed and signed Application Form o Two current signed letters of recommendation: One letter must be from a teacher or school official One letter must be from an adult (e.g. employer, minister, community leader). Letters from relatives are not eligible. Note: If you are applying for the Centennial Scholarship, one of your letters must be from a Public Service organization. Applicant must include contact information (phone number and/or ) for the person representing the organization. o Recommendation letters should include the length of time the reference has known the applicant and in what capacity, applicant s strengths and weaknesses, and why the reference believes the applicant deserves the scholarship. o The letters of recommendation must be original versions, signed by the writer and placed in a sealed envelope (photo copies, faxes and electronic submissions are not acceptable). o An official transcript: Official high school transcript must be in the original envelope from the school and should be sealed and stamped by the school. Opened transcripts will not be accepted. Must include the applicant s first semester grades for his/her senior year. o A typewritten Essay: (See Part IV of the application for details and essay topic) 3) MAIL a completed, typewritten application along with attachments, official transcript, and recommendations to: CAC Scholarship Committee c/o Pamela M. 2 Applicant Name: Applicant High School: SCHOLARSHIP SELECTION AND CRITERIA (Check all that apply for consideration. You may apply for more than one scholarship. Checking a specific box indicates your intent to apply for the specific scholarship listed in bold letters above the box.) Scoggins/Shadd Breaking Barriers Scholarship (non renewable) Must have a cumulative GPA of 2.75 or higher Must demonstrate financial need (please refer to the Federal Government s Poverty Guidelines) by completing Part V. Note: Failure to complete Part V will disqualify the application for this scholarship. Improved Performance Scholarship (non renewable) Must be a graduating male or female senior Must have a cumulative GPA of 2.5 or higher and have shown significant improvement in his/her academics over a two (2) year period. Note: Transcript must show two years of high school grades Must plan to attend a two (2) or four (4) year institution of higher education in the U.S. during the upcoming academic year (Award disbursement based on confirmed enrollment for Fall 2013) Merit Scholarship (non renewable) Must have a cumulative GPA of 3.5 or higher Centennial Scholarship (Four Year Supported Scholarship) Must have a cumulative GPA of 3.0 or higher for each consecutive year Must demonstrate financial need (please refer to the Federal Government s Poverty Guidelines) by completing Part V Note: Failure to complete Part V will disqualify the application for this scholarship. Significant leadership and involvement in extracurricular activities as demonstrated in application and reference letters Demonstrated Public Service above and beyond high school requirement. One of the letters of recommendation must be from a verifiable public service organization, and it must be submitted on the organization s letterhead. Note: The amount of the annual scholarship may vary
