4 Exercise Referral Scheme WHAT IS THE EXERCISE REFERRAL SCHEME? Exercise Referral is a 20 session physical activity programme for patients with medical conditions, whose health could benefit from leading a more active lifestyle. The programme provides an individualised physical activity programme, to enable patients to increase sustainable physical activity levels and reduce sedentary behaviour. East Riding of Yorkshire Council s Exercise Referral scheme is ran in accordance with the National Quality Assurance Framework for exercise referral systems (2001) 2. AIM OF THE EXERCISE REFERRAL SCHEME Exercise Referral Scheme uses a customer centric approach to ensure that all participants are helped and encouraged to make behaviour changes in order to incorporate physical activity into their daily lives. It enables participants to maintain a healthy lifestyle which can contribute to a reduction in the incidence of coronary heart disease and other medical conditions. 3. WHAT WILL THE EXERCISE REFERRAL SCHEME PROVIDE FOR PARTICIPANTS? A tailored programme of exercise for 20 sessions to support lifestyle change which can include gym, chair based and exercise classes An analysis of participants needs and expectations relating to their own health and well being Visual and verbal assessment by fitness professionals at every Exercise Referral session throughout the 20 sessions, to ensure high levels of success and to ensure safety of all involved Support to exercise at any East Riding of Yorkshire Council s Leisure Centre s A choice for all patients to chose when they wish to exercise and how often they attend per week in order to complete their 20 sessions. (ERYC recommend all patients to come twice a week to receive the full benefit) 4. ACCESSING THE EXERCISE REFERRAL SCHEME Patients who are inactive and would benefit from a course of physical activity can be referred onto the scheme by their GP. (Please see the contra-indications section 10) Individuals must be willing to make changes to their lifestyle in order to access the service.
5 04 Exercise Referral Scheme 5. REFERRAL PROCESS GP completes referral form, patient signs to say they are committed to scheme. GP sends referral form to patients chosen Leisure centre. OR GP completes electronic referral form and books in an appointment then and there with the patient. Participant contacted by there assigned personal instructor at their chosen leisure site. At the first session the personal instructor completes a health check. Then the personal instructor will design a suitable exercise programme. Participant completes the 20 session exercise programme. A final assessment is carried out by the instructor. After the 20 sessions fitness professionals signpost participants into the most suitable exercise and fitness activities to allow them to continue with their health regime.
6 Exercise Referral Scheme QUALIFICATIONS OF THE EXERCISE REFERRAL FITNESS INSTRUCTOR All Exercise Referral fitness instructors at East Riding of Yorkshire Council s Leisure Centre s hold a Level 3 qualification in Exercise Referral from the following trainer providers: Wright Foundation GP Exercise Referral Course YMCA GP Exercise Referral Course (formerly Gayton Group) Other recognised Level 3 Exercise Referral qualification providers In addition to this, Exercise Referral instructors may also hold or be working towards other relevant qualifications, including: British Association of Cardiac Rehabilitation teaching certificate (BACR) Motivational Interviewing Level 4 (currently being accredited by Prevention and Lifestyle Behaviour Change Framework) Nutrition and Weight Management Open College Network Level 3 Mental Health First Aid Smoking Cessation Training Level 1 and 2 NVQ 3 Personal Trainer NVQ 3 Advanced Instructor First Aid/CPR Certificate 7. ROLES AND RESPONSIBILITIES OF THE PERSONAL TRAINER The personal trainer will: Ensure all participants registering on the programme have a signed referral form, and have been referred by an appropriate GP, nurse or health professional Contact the referred participant to arrange a suitable start date for their programme Complete a pre and post questionnaire and participant record form with the participant at the start and end of the programme. This records participants personal details, current lifestyle and readiness to change Produce an appropriate exercise programme based on the participant s health status, stated medical conditions and information provided by the GP, nurse or health professional Make every effort to contact the participant if they do not attend the programme; after a first missed session they will attempt to contact the participant by telephone to arrange a new appointment. If telephone contact proves difficult for five days a letter will be sent by post. If the participant continues to not respond they will be referred back to the G.P with the date the participant stopped attending and the reason, if known. This will be the same process if participants cancel up to three consecutive sessions. Store all participant records in a locked place ensuring that access to them is restricted to personnel involved in the running of the scheme Direct participants into other healthy lifestyles programmes within the leisure centres on completion of the Exercise Referral scheme.
7 06 Exercise Referral Scheme 8. ROLES AND RESPONSIBILITIES OF THE GP The GP/Practice will: Identify any suitable participant for referral onto the scheme and discusses the scheme with the individual, so that they are aware of what it entails. Obtain the participants agreement to be referred. If the patient is willing to be referred immediately the GP completes the referral form which they and the participant sign (providing information on the participants medical history, current health status, medication etc.). Send the completed copy of the participant referral form to the fitness co-ordinator at the patients chosen ERYC leisure centre. Respond to any queries from the personal trainer, Healthy Lifestyles Development Officer e.g. regarding the participant s medical condition. Contact the Healthy Lifestyles Development Officer ( ) if any issues / problems arise. 9. CLINICAL AND LEGAL RESPONSIBILITIES The Exercise Referral scheme adheres to the National Quality Assurance Framework for the Exercise Referral Systems (Department of Health, 2001) The framework was written following specific consultation with medical practitioners and with reference to and consultation with the Department of Health. For more information please refer to the full document, which can be found at: When the individual is specifically referred for exercise by the health practitioner, responsibility for safe and effective management, design and delivery of the exercise programme passes to the exercise and leisure professionals. 10. ABSOLUTE CONTRA-INDICATIONS FOR REFERRALS Participants should NOT be referred onto the Exercise Referral Scheme if they have any of the following contra-indications: Ventricular or aortic aneurysm Unstable angina (diagnose less than a month a ago, without known trigger factors, at rest or minimal exertion, changes in frequency and severity) Resting systolic blood pressure > 180mmHg or resting diastolic blood pressure >100mmHg Uncontrolled tachycardia >100bpm at rest Unstable or acute heart failure. Uncontrolled arrhythmia Recent Embolism (patients can be accepted post 6 months) Awaiting Investigation for any cardiac complaints Uncontrolled Asthma Unstable/severe mental health issues Unstable valvular heart disease Recent cardiac event (patients can be accepted post 6 months after cardiac event)
