USF FCU has put together this Dividend Checking Account Switch Kit, making it easy for you to transfer your checking account to us. Just follow these step- by-step instructions. If you have any questions or need assistance, simply call one of our friendly and knowledgeable member service representatives at (813) option 7. Here s How to Make the Switch: Not a USF FCU Member? Follow these steps: STEP 1 - Open a Savings and Dividend Checking Account with USF FCU. Just stop by one of our convenient locations and open a Savings and a Dividend Checking Account. You will need to bring with you: Proof of eligibility such as a USF ID Card, USF acceptance letter, current class schedule, USF pay stub etc. Proof of permanent address (i.e. driver s license with current physical address) and if applicable, proof of local current address (i.e. residence hall assignment, lease agreement, electric bill). A minimum deposit of $30 ($5 for the required basic savings account and $25 to serve as your initial checking deposit). 2 Already a USF FCU Member? Follow these steps: STEP 1 - Open a Dividend Checking Account with USF FCU. Just stop by one of our convenient locations and open a Dividend Checking Account. You will need to bring with you: A minimum deposit of $25 to serve as your initial checking deposit. Proceed to step 2 For instructions on how to open your dividend checking account by mail click here. STEP 2 - Close Your Old Checking Account. Make sure you leave sufficient funds in your old account to cover any outstanding checks and automatic withdrawals. This process may take several weeks. Once your transactions have posted, close your old account completely and ask your previous financial institution to send you the balance of your account. Also destroy your old ATM and/or debit cards, as well as your unused checks and deposit slips. STEP 3 Make the Switch. We ve provided the forms you ll need to close your accounts and change any direct deposits or automatic withdrawals you may have. If you need help completing any of these forms, visit any of our convenient locations and one of our representatives will be happy to assist you. Switch Kit Forms Provided: Close Account Form Send this form to your previous financial institution. If you had more than one, you will need to send separate forms to each one. Change Payroll Direct Deposit Send this form to those sources that make direct deposits into your account: Your employer The company handling your retirement, pension or settlement payments Social Security Administration Please allow at least 30 days for change to take effect. Automatic Withdrawals Send this form to anyone who makes automatic withdrawals from your account: Mortgage company Homeowner s insurance Auto insurance Life insurance Gym Membership Other Please allow at least 30 days for change to take effect.

3 USF FCU : This is the number which appears on the lower left of your USF FCU checks, and identifies us as your financial institution. This number is listed on the Change Payroll Direct Deposit and Change Automatic Payment/Withdrawals forms, included with this Switch Kit. You will need to provide this number to companies and institutions which you ve authorized to automatically deposit or withdraw funds from your USF accounts.
