1 SHUFFLE MASTER CLASSIC QUALIFYING PROMOTION - OFFICIAL RULES Beginning on Wednesday, June 1, 2016 through Wednesday, August 31, 2016 a Table Games exclusive promotion will be offered. During the promotion, any Table Games player hitting a qualifying Classic hand will be awarded entry into the $250,000 Shuffle Master Classic Three Card Poker National Championship. Qualifying Classic hands are as follows: Qualifying Game Qualifying Classic Hand Three Card Poker Six Card Straight Flush Four Card Poker Caribbean Stud Poker Let It Ride Mississippi Stud Ultimate Texas Hold Em (First Five Cards Only) Texas Hold Em Bonus (First Five Cards Only) Crazy 4 Poker Flushes Gone Wild Natural (First Five Cards Only) DJ Wild Natural In the event a player hits a Classic hand while playing the appropriate Qualifying Games, the dealer will immediately contact the floor supervisor, shift manager or pit boss. Upon reporting a Classic hand, the table affiliated with the Classic hand will be temporarily placed out of play while the winner verification process is completed. The floor supervisor or dealer will obtain a photo copy of the player s identification. A Thunder Valley Team Member will complete the attached Shuffle Master Classic Winner Verification Form in conjunction with obtaining copies of the player s identification and obtaining any additional information for winner verification. A Thunder Valley Team Member will have the patron complete a W2G form for tax purposes, assuming the Classic hand triggers a large enough payout. A Thunder Valley Team Member will give the qualifying customer a designated Shuffle Master Classic qualification packet that contains a serial number to register and will keep a record of the qualification packet receipt form signed by the winning player. General Rules Participants must be 21 years of age or older and are requested to have a Thunder Rewards card to be eligible. All applicable taxes and fees are the responsibility of the winner. All unclaimed prizes are considered forfeited. Participants in this promotion agree to abide by the rules of this promotion and to be photographed for advertising and publicity purposes without compensation. In accordance with Internal Revenue Service regulations, winners of prizes valued at $600 or above must provide their name, address and taxpayer identification number (SSN) prior to receiving their prize. Management is not responsible for errors or omissions. Any dispute or situation not covered by the above rules will be resolved by Thunder Valley Casino Resort management in a manner deemed by them to be the fairest to all concerned, and that decision shall be final and/or binding on all contestants. In addition, Thunder Valley Casino Resort reserves the right to void any ticket, and/or to change, cancel, or modify this promotion at any time.

2 QUALIFICATION RULES 1. Void where prohibited or restricted by law. 2. All player participants must be at least 21 years of age ( Participant(s) ) to participate and Participants must provide valid proof of age acceptable to the participating casino facility ( Participating Casino ) and/or Bally Gaming, Inc., also referred to throughout as Bally or Shuffle Master. 3. This Promotion is subject to all applicable federal, state, and local laws and regulations, including gaming laws and regulations, and all aspects of the Promotion are subject to the approval of the appropriate regulatory authorities. 4. All Participants who qualify for the Shuffle Master Classic in Las Vegas ( Qualifiers ) are subject to review and verification. 5. Qualifiers are responsible for any and all taxes, licenses, registrations and other fees, as applicable. 6. Qualifiers may be subject to reporting and withholding requirements pursuant to federal IRS regulations. Qualifiers will be responsible for all federal, state, and/or local taxes. Participants and Qualifiers are required to cooperate with the Participating Casino and/or Shuffle Master in completing necessary forms. 7. All decisions regarding the interpretation of rules, eligibility, etc. for this Promotion will be made by the Participating Casino and/or Shuffle Master whose decisions are final. 8. Malfunctions in game play, patron cheating or advantage play (if advantage play is illegal where the Participating Casino is located) associated with any participation in the Promotion or in obtaining an entry to the Shuffle Master Classic in Las Vegas will void all game play and pay results, including any potential Promotional award offered. The Participating Casino and Shuffle Master reserve the right in their sole discretion to disqualify any individual who is found to be tampering with the entry process or the operation of the Promotion, to be acting in violation of the Promotional rules, or to be acting in an unsportsmanlike, dishonest or disruptive manner, or with the intent to disrupt or undermine the legitimate operation of the Promotion, or to annoy, abuse, threaten or harass any person involved in the Promotion. Additionally, the Participating Casino and/or Shuffle Master may impose penalties, including but not limited to disqualification from the Promotion or loss of the right to participate in any other Promotion conducted by Shuffle Master or the Participating Casino, upon any person who gives, makes, issues, authorizes or endorses any statement or action having, or designed to have, an effect prejudicial or detrimental to the best interest of the Promotion as determined by the Participating Casino and/or Shuffle Master, acting in its sole and absolute discretion.

By entering the Promotion, Participants release Shuffle Master and the Participating Casino from any liability whatsoever, and waive any and all causes of action related to any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the Promotion, whether under a theory of contract, tort, warranty, or other theory. Shuffle Master assumes no responsibility for any damages sustained by Participants. Without limiting the generality of the foregoing, Shuffle Master is not responsible for: any acts or omissions by employees in the conduct of the Promotion; for incomplete, misdirected, late, lost, stolen, destroyed or damaged Promotion entry forms; for any technical, hardware, software, or telephone failures or malfunctions of any kind; for lost or unavailable connections, fraud, incomplete, garbled, or delayed computer transmissions, whether caused by Shuffle Master, users, or by any of the equipment or programming associated with or utilized in the Promotion or by any technical or human error which may occur in the processing of entries which may limit, restrict, or prevent a Participant s ability to participate in the national championship event; or for any equipment or dealing malfunction associated with the Promotion. 10. Shuffle Master reserves the right to modify, extend, suspend or cancel this Promotion, in whole or in part, if it determines that the Promotion is technically impaired or corrupted or that fraud or technical problems, failures, malfunctions or other unintended circumstances have impaired the integrity, administration, security, proper play, and/or feasibility of the Promotion as contemplated herein. Such modification, extension, suspension or cancellation of the Promotion will be subject to regulatory approval, if applicable. 11. Any attempt by any person to deliberately damage this Promotion may be a violation of criminal and civil laws and should such an attempt be made, the Participating Casino and/or Shuffle Master reserve the right to seek damages from any such person to the fullest extent of the law. 12. Employees and immediate family members of the Participating Casino, Shuffle Master, and any of their subsidiaries, affiliates or parent companies and immediate family members of employees and the employees of the advertising agency affiliated with this Promotion are not eligible to participate in the Promotion. Immediate family members are defined as spouse, child or anyone sharing the same residence as the applicable employee. 13. Individuals who are excluded from casino facilities, including through a government program, State Gaming Commission and/or at Participating Casino discretion, or by their own request, are not eligible, which also encompasses individuals excluded from gaming facilities in Nevada. 14. By participating in this Promotion, Participants agree to the rules contained herein. 15. A copy of these rules and any changes will be available at shufflemasterclassic.com throughout the duration of this Promotion. 16. Qualifiers CANNOT transfer their entry to the Shuffle Master Classic in Las Vegas to another person. The Participating Casino and/or Shuffle Master reserves the right to substitute prizes that have or have not been awarded as long as the value is substantially the same as the original prize offered in the event that the original prize becomes unavailable. In no event will the Participating Casino and/or Shuffle Master be obligated to award more prizes than the number of prizes stated in these Official Rules.

4 17. Prizes unclaimed by Qualifiers shall not be awarded to other Participants. 18. Where a situation arises that is not covered by these Official Promotion Rules, the Participating Casino, Shuffle Master and/or any regulatory authority having jurisdiction over the Participating Casino or Shuffle Master shall have the authority to render a judgment, including the imposition of a penalty, in accordance with the best interests of the Promotion and the maintenance of its integrity and public confidence. SPECIFIC GAME RULES: 19. To qualify for a seat in the National Shuffle Master Classic tournament, players must hit a Classic hand during regular game play on a qualifying game during the term Promotion, scheduled from June 1, 2016 through August 31, 2016 (as subject to adjustment by Shuffle Master: Qualifying Game Three Card Poker Four Card Poker Caribbean Stud Let it Ride Mississippi Stud Crazy 4 Poker Texas Hold Em Bonus Ultimate Texas Hold Em Flushes Gone Wild DJ Wild Qualifying Classic Hand Six Card Straight Flush (First five cards only) (First five cards only) NATURAL (First five cards only) - NATURAL 20. Qualifying hand must be hit at a Participating Casino, on a participating Shuffle Master game. 21. Any qualifier for the Shuffle Master Classic in Las Vegas must submit the required application materials on or before the deadline for entry into the Shuffle Master Classic. 22. Winning hand and player must be verified at the Participating Casino, following the proper winner verification procedures. 23. Qualifiers will receive a two night stay, and round trip airfare for one person, up to a value of $1,250. Any airfare and hotel expenditures above $1,250 will be the responsibility of the qualifier. If a Qualifier spends less than $1,250 on airfare and hotel accommodations, any remaining balance of the $1,250 balance will not be given to Qualifiers. Qualifiers are responsible for payment of all food, beverages, taxes, and gratuities in connection with travel and stay in Las Vegas. Qualifiers are responsible for transportation expenses during their stay in Las Vegas and are responsible for any accommodations expenses at the Hotel in excess of two nights. In the event the Shuffle Master Classic is cancelled for any reason, the Qualifier will receive the booked travel and no compensation will be paid in lieu of the Shuffle Master Classic cancellation.

5 24. Crazy 4 Poker, Four Card Poker, Mississippi Stud, Let It Ride, Caribbean Stud, Texas Hold Em Bonus, and Ultimate Texas Hold Em require a five card royal flush for qualification. Texas Hold em Bonus ( THB ) and Ultimate Texas Hold em ( UTH ) require the royal flush to be on the first five cards played; THB and UTH players cannot use all seven cards for their royal flush. DJ Wild requires the royal flush to be natural (no joker or deuce allowed). Flushes Gone Wild requires the royal flush to be natural (no deuce allowed) and on the first five cards. 25. Three Card Poker ( TCP ) requires a six card straight flush for qualification. The six card straight flush will use the players three cards combined with the dealers three cards. 26. Odds of hitting the five card royal flush are 1 in 650,000. In DJ Wild, the odds of hitting the natural royal flush are 1 in 717,000. Odds of hitting the six card straight flush on TCP are 1 in 565, Folded hands DO NOT qualify. 28. The National Shuffle Master Classic event is tentatively scheduled for Monday, September 26 th, 2016 at The Venetian Las Vegas, with such location in Las Vegas subject to change at the discretion of Shuffle Master. 6 PROMOTION USAGE GUIDELINES AND RESPONSIBILITIES 1. The gaming location(s) scheduled by the Customer in the Contact Form ( Location(s) ) will be the Location(s) where the Shuffle Master Classic qualifying promotional event (the Promotion ) will be held. In the event the Promotion will be held at Locations in addition to those listed in the Contact Form, Customer and Bally will agree in writing to the additional Locations. 2. Customer will be responsible for obtaining all required regulatory and other approvals for offering the Promotion at the Locations prior to the start of the Promotion ( Location Approvals ). Bally will reasonably assist Customer with obtaining any such approvals, as requested by Customer. Location Approvals will include, but are not limited to, Customer s internal approvals and authorizations, Customerrequired submissions to regulatory or governmental entities having jurisdiction over Customer or the Promotion activities as offered, and any other requirements within Customer s applicable internal control standards. 3. Bally may supplement or amend the rules and usage guidelines of the Promotion and will advise Customer promptly in the event any such changes are required. In addition, the Promotion will be conducted in accordance with any applicable laws, rules and regulations to which Bally is subject and Bally will comply with such requirements, in addition to complying with the provisions of any required Location Approvals. 4. Customer will be responsible at all times for the conduct and services provided by Customer s on-site floor staff at the Locations providing the Promotion to Customer s patrons, and Bally shall not be liable for any claims arising from Customer s staff related to providing the Promotion. 5. Customer will be responsible for all surveillance, camera coverage and other security measures associated with the games utilizing the Promotion. 6. Customer will be responsible for resolving any disputes with patrons arising from the Promotion in accordance with Customer s existing patron dispute resolution processes. 7. Each party acknowledges that the other party and/or its affiliates are subject to gaming, lottery or similar licensing requirements in various jurisdictions. Each party shall cooperate fully with the other party (or any of its affiliates) in providing to the other party any information of whatever nature that the other party deems necessary or appropriate in assuring itself that it possesses the good character, honesty, integrity, and reputation applicable to those engaged in the gaming and lottery industries. Each party represents and warrants that there is nothing in that party s background, history or reputation that would be deemed unsuitable under any standard applicable to such industries. If, during the term of the Promotion, either party (or any of its affiliates) is notified (formally or informally) by any governmental authority that the engagement of, or conducting business with, the other party may or will jeopardize any license or ability to be licensed of that party (or any of its affiliates) or if a party (or any of its affiliates) concludes that the other party may fail to meet the above criteria or the compliance committee of that party or any of its affiliates otherwise raises an objection with respect to the other party, that party may immediately terminate this agreement upon written notice to the other party. 8. Customer represents, warrants and covenants to Bally as follows: (1) all information furnished by Customer to Bally in connection with Bally s due diligence and compliance review process is complete and accurate; (2) Customer shall, in connection with the Promotion, (a) maintain complete and accurate books and records and (b) comply with all applicable laws, rules and regulations, including, but not limited to, those relating to anti-corruption, anti-money laundering, competition, licensing and registration; and (3) Customer has not offered or paid, and will not offer or pay, directly or indirectly, (a) anything of value to any public official or candidate for political office, or any relative or agent thereof, for purposes of obtaining any official action or benefit relating in any way to this Promotion or agreements related to it or (b) any commission or finder s or referral fee to any person or entity in connection with this Promotion or any activities on behalf of Bally. 9. Bally s branding and style guidelines are provided at and Customer and Location(s) shall review and comply with all such Bally marketing requirements.

7 WINNER VERIFICATION PROCESS 1. Prior to the start of each shift, please verify active table positions and qualifying games that will be engaged with the Shuffle Master Classic promotional event. 2. As applicable, prior to the start of each shift, please advise dealer staff that the Shuffle Master Classic promotional event is ongoing and tables offering the promotional event will be subject to this winner verification process. 3. Prior to the start of each shift, verify that adequate copies of the Winner Verification Form and Shuffle Master Classic qualification packages are readily available for use by staff and for distribution to a Qualifier if a Classic Hand is hit during that particular shift. 4. In the event a patron hits a Classic Hand while playing the appropriate Qualifying Games, as listed below, the dealer will immediately contact the floor supervisor, shift manager or pit boss. 8 5. The applicable floor supervisor will attend to the table reporting the Classic Hand and assist the dealer with the verification process. 6. Since a large pay will already be required on attaining a Classic Hand, the floor supervisor or other appropriate security personnel will secure all surveillance recording of table activity, whether by dedicated camera, partial floor coverage cameras or other applicable surveillance. 7. The floor supervisor or dealer will retrieve government-issued identification from the player who hit the Classic Hand and will verify hand on shuffle (if applicable). The identification will be copied and provided to Shuffle Master as soon as possible following the Classic Hand being hit. 8. The floor supervisor or other appropriate facility staff member will complete the attached Shuffle Master Classic Winner Verification Form in conjunction with obtaining copies of the patron s identification and obtaining any additional information for winner verification. 9. The floor supervisor or other appropriate facility staff will have patron complete W2G form for tax purposes, assuming the Classic Hand triggers a large enough payout. 10. The floor supervisor will give the qualifying customer a designated Shuffle Master Classic qualification packet that contains a serial number to register and will keep a record of the qualification packet receipt form signed by the winning patron. 11. The floor supervisor or other appropriate staff member will then ask the player his or her permission to be photographed for publicity. If the winner agrees, the floor supervisor or appropriate staff member will take a photo of the customer with the winning hand and/or qualification packet and complete the publicity release form. 12. The floor supervisor or other appropriate facility staff will send an within 24 hours of the win to Louis.D Andrilli@scientificgames.com with the following information for the patron who qualified: a. Winner Verification Form b. Publicity Release Form and Picture c. Qualification Packet Receipt Form d. Copies of Patron Identification e. Copies of any W2G forms issued
