13 QUALIFIERS FS Licensed Locations Qualifier Designated qualifier must be a full time employee Must function in a supervisory capacity of employees operating under scope of license Cannot be assigned to more than one licensed location One for every 10 employees Exception Category II LP Gas Dispenser Vacancies in Qualifier reported immediately By the qualifier By the licensed company 20 working days to replace the vacancy suspension 60 working days revocation Operating without a qualifier may result in administrative penalty QUALIFIER CARD NOT A LICENSE 13

14 MASTER QUALIFIERS FS designated at each licensed location LP Gas Dealer Type 0601 LP Gas Installer Type 0803 Documentation that individual is Manager Owner Primarily responsible for overseeing the operations of the licensed location Vacancy reported immediately By the Master Qualifier By the licensed company 20 working days to replace 60 working days to replace the vacancy if a 601 qualifier is on staff 90 working days revocation Must have a Master Qualifier employed at the license location Operating without a Master Qualifier may result in administrative penalty MASTER QUALIFIERS FS To become a Master Qualifier Must be a current Qualifier in the same license type Provide Documentation of minimum 1 year work experience in the gas industry Pass a master qualifier competency exam Must Provide Documentation that he/she is Manager Owner or Primarily responsible for overseeing the operations of the licensed location Must renew at 3 year intervals Renewal application Submit documentation of 16 hrs of continuing education within the 3 years Proof of employment with a licensed category I LP Gas Dealer/Installer or applicant $30.00 renewal fee 14

15 MASTER QUALIFIER CERTIFICATE QUALIFIERS & MASTER QUALIFIERS IMPORTANT FS (7) Qualifiers and Master Qualifiers are held legally responsible for compliance with LPG laws and regulations and can have their qualification suspended, revoked or not-renewed for noncompliance. 15

18 TRUCK REGISTRATIONS FS Each LP Gas bulk delivery truck, leased or owned, must be registered At the time of application Annually thereafter Truck Inspections by the Bureau Annually Fee of $50.00 billed at the time of inspection FINAL INSPECTION FS (4) Before the License is Issued License application file fee paid Site plan approved fee paid Certificate of Insurance Qualifier Master Qualifier Truck(s) Registration Final Inspection Conducted by LP Gas Inspector Completely to Code Includes Training Documentation Fire Safety Analysis where applicable Operation and Maintenance Procedures where applicable 18

19 LP GAS LICENSE RESPONSIBILITY TO INFORM FS Persons engaging in the business of servicing, testing, repairing, maintaining or installing LP gas equipment and systems Present proof of licensure prior to working on equipment or system to consumers, owners, or end users Present proof of licensure upon request of consumers, owners, end users, or persons who have authorized such work. Any alterations/modifications to the system must be reported in a timely manner to the servicing gas supplier Includes work performed by consumers 19

20 TRAINING AND DOCUMENTATION Training Requirements NFPA 58 Chapter 4.4 FAC 5F ALL employees must be trained To cover the scope of their job function Refresher Training Required All employees INCLUDING Qualifiers Minimum 3 year intervals Training must be documented Sources of Training Outside Training Internal Training Training Records kept at the licensed location 20

21 CEU TRAINING FAC 5F Outside training must be approved by the Bureau of LP Gas Inspection Internal Training No approval required Must cover one or more of the following topics Inspection and maintenance of LP Gas facility and equipment State and Federal LP gas laws, rules and regulations, codes and standards Gas emergency procedures, fire protection, and risk management planning Gas storage and distribution facility operations and safety Gas transportation and delivery Gas liquid transfer Gas tanks, cylinders and equipment Gas liquid and vapor distribution systems and equipment Gas equipment and appliance service, installation and repair TRAINING DOCUMENTATION FAC 5F Documentation must include Employee Name Primary Job Responsibilities/Duties Date of Employee Training Description of Training/Materials Name, address and phone number of trainer Verification by supervisor signature Must be maintained at the employees place of business Available for inspection upon bureau request Instructor receives credit for training Includes CEUs where applicable 21

22 AUTHORITY TO INSPECT FS The department may inspect at any reasonable hour to determine if any violation of this chapter or rules of the department Any facility, building, system, equipment, vehicle or premises Where LP gas or equipment is offered for sale, stored, transported, or being repaired or installed INVESTIGATION &SAFETY FS Authority to investigate the cause and circumstances of any accident involving loss of life, personal injury, or property damage where there is reasonable cause to believe LP Gas was involved Take samples or evidence Make a written report of the investigation Becomes public record upon completion Authority to assist state, county, municipal and local governments in promoting safety 22

23 NOTIFICATION OF ACCIDENTS ACCIDENT REPORTING FS IMMEDIATELY report any gas-related accidents which result in: Death Personal injury requiring professional medical treatment Estimated damage to property exceeding $1,000 Uncontrolled ignition of LP Gas resulting in death, personal injury, or property damage exceeding $1,000 Failure to report may result in administrative penalty 23

24 ACCIDENT REPORTING HOTLINE USE OF LP GAS LIMITED FAC 5F LP Gas may NOT be used As a source of pressure in lieu of compressed air to operate ESVs, spray guns, or other similar equipment As a refrigerant in mobile air conditioning systems 24

25 INTRODUCING GAS INTO CONTAINERS FOR TRANSPORTATION FAC 5F No dealer in LP Gas shall introduce LP Gas into any container to be transported in any vehicle unless the provision of NFPA 58 Chapter 9 is met The Dealer is responsible to insure that transportation of container brought to his premises and while upon or about his premises complies with the provision CONTAINERS Florida Laws, Rules and Regulations 25

26 UNSAFE CONTAINERS OR SYSTEMS FS FAC 5F Restrictions NO LP Gas shall be introduced into or removed from a container or system that has been identified by the department as not meeting code requirements. 28 UNDERGROUND TANKS; REGULATORS 5F Two-stage regulator system required First Stage in the container dome above potential water level or regulator vented with outlet above potential water level Second stage at the nearest practical location where LP gas line enters the building Inside allowed with proper venting of the regulator Exception requires Bureau of LP Gas Inspection approval PRIOR to installation + LEAK TESTING UG SYSTEMS FAC 5F All UG containers must be visually Inspected at each fill Bar Probe Leak Survey required at intervals not to exceed 60 months where installed at Places of Public Assembly Group Living Facilities Health Care Facilities Detention and Correction Facilities Industrial or Commercial Facilities Educational Facilities Survey upon change of contract or at intervals not to exceed 60 months, whichever comes first Bar Probe Leak Survey Record Must be maintained by the supplier at the local office of distribution Must be date-tagged after testing Systems not requiring Bar Probe Leak Survey must Have visual Inspection at the time of each fuel delivery Be documented by a notation on the fuel delivery ticket or other form designated for this purpose by the supplier Have documents that are maintained by the gas supplier for a period of 12 months This rule does NOT apply to underground installations already subject to the provision of Title 49 of the CFR regulation Parts 191 and

32 DEFINITIONS Out of Gas Fuel remaining in the container(s) will NOT support combustion The gauge reading is NOT an acceptable means of determining if the container is out of gas Interrupted Service Gas flow to appliances or equipment stops regardless of the cause LEAK CALLS FS Consumer leak call Verifiable leak call Must physically respond within 24 hours Emergency Responder Call Must physically respond within 2 hours 32

33 OUT OF GAS 5F (1) Out of Gas call Prior to delivery Close all container valves Physically check all outlets and look for evidence of changes Put equipment back into operation if accessible NOT accessible to put back in service leave container valve closed leave written notification WRITTEN NOTICE FAC 5F (1) (c) Written notification MUST include Name & phone number of servicing company Description of the work performed Description of any unsafe conditions found A telephone contact for restoration of service A statement warning against reactivation of service by unauthorized persons 33

34 SUSPECTED LEAK FAC 5F (2) The LP Gas license holder or trained employee Check for leakage by performing an appropriate leak test in accordance with Annex D of NFPA edition Make necessary repairs or leave system in a safe condition Where repairs are made, the system is to be placed back in service in accordance with NFPA 54 Chapter 7 INTERRUPTED SERVICE FAC 5F (3) Where Interruption of service has occurred The LP Gas license holder or employee Must check for leaks in the affected areas of the system Refer to NFPA 54 Annex D Perform a leak test of the system prior to placing the system back into service Refer to NFPA 54 Annex D The Leak Test is not required where The LP Gas license holder or employee has caused the interruption of gas service and The license holder or employee has remained on the system site and Monitored the system during the service HOWEVER The repairs shall be leak tested by means of an approved combustible gas detector or leak detector solution 34

35 CONNECTING OR DISCONNECTING CONTAINERS OR SYSTEMS NOTIFICATION REQUIRED FAC 5F CONNECTING OR DISCONNECTING FAC 5F (1) Connecting or disconnecting a container without notifying the owner is prohibited. Where the cylinder, tank or system contains LP gas Notice shall be received by the owner at least 2 working days prior to the installation Evidenced by a signed receipt Signed certified mail receipt Signed receipt of hand delivery Facsimile transmission receipt 35

36 CONNECTING OR DISCONNECTING FAC 5F (2) Where the cylinder, tank or system is out of gas and authorization by the end user has been received The cylinder, tank or system may be disconnected downstream of the system regulator or meter The Owner of the disconnected cylinder, tank or system must be notified within 24 hours Written notification must be provided within 5 working days Evidenced by a signed receipt Signed certified mail receipt Signed receipt of hand delivery Facsimile transmission receipt CONNECTING OR DISCONNECTING FAC 5F (1) The disconnection of any cylinder or tank or system must be done in compliance with NFPA 58 Cylinder and Tank valves must be turned off and the service valve plugged with a brass or steel fitting All other cylinder, tank or system openings shall be properly plugged 36

37 CONNECTING OR DISCONNECTING FAC 5F (1) The owner of the disconnected cylinder, tank or system must remove the cylinder or tank from the premises within 30 working days of the notification Removal by any party other than the owner or party authorized by the owner is prohibited OUT OF SERVICE ACCOUNTS FAC 5F Containers >100 GWC and Out-of-Service for 12 months or more: Must be identified Must initiate appropriate container abandonment procedures within 60 days Must be abandoned according to NFPA 58 procedures; or Remove; or Have a mechanical device installed to prohibit activation; or Be pressure tested every 12 months Records must be maintained of all inactive accounts Available for inspection by the Bureau upon request To Reactivate, the supplier must perform an appropriate pressure leak safety check 37

38 OUT OF SERVICE DISPENSERS 5F (3 &4) Vehicle fuel dispensers or dispensing stations out of service 6 Months shall be made safe by Evacuation of any remaining LP Gas from the tank, reducing tank pressure to vapor pressure All container openings must be plugged 12 months Removed from the premises PLANT SECURITY NFPA 58 Chapter FS

39 PLANT SECURITY NFPA58 Chapter To minimize entry by unauthorized persons, the Facility operator shall provide Security awareness training for employees Limit unauthorized access to the plant area including Container appurtenances Pumping equipment Loading and unloading facilities Container filling facilities Enclose with a minimum 6 foot high industrial type fence, chain link or equivalent 2 means of egress Gate locked when unattended PHONES & EMERGENCY CONTACTS FS (3) Name/Emergency Contact for After-Hours Emergencies Must be Prominently posted at each bulk storage site Given to local fire department personnel Messages must be relayed to the appropriate person within 1 hour of receipt 39

43 REQUIRED PLANS NFPA 58 Chapter 6.25 Fire Safety Analysis Required on all installations consisting of an aggregate water capacity of more than 4000 gallons and shall Include the modes of fire protection for the facility Be prepared and submitted by the owner, operator or their designee Be presented to the authority having jurisdiction and Local emergency responders Be updated when the storage capacity or transfer system is modified KEEP THE LINES OPEN SET UP WALK THROUGHS COMMUNICATE! 44 REQUIRED PLANS NFPA 58 Chapter 14 Written Operating Procedures are The basis for conducting activities associated with a bulk plant, an industrial plant, refrigerated, marine, and pipeline LP-Gas system Address all aspects of LP Gas transfer appropriate for the facility including Inspection of hoses and fittings Connections and disconnections procedures Updated when a change occurs that affects the operation of a system and prior to its startup Kept in a common location REQUIRED PLANS NFPA 58 Chapter 14 Written Maintenance Procedures The basis for maintaining the mechanical integrity of the LP-Gas system Updated when a change occurs that affects the maintenance of a system Maintenance Manuals for all equipment at the facility must Be kept at the facility Available to maintenance personnel For unattended facilities they may be kept at a location where accessible for maintenance personnel servicing the facility Include routine inspections and preventative maintenance procedures and schedules Be made available during normal office hours to the authority having jurisdiction Be retained for the life of the equipment 44
