4 Registration Form Page 4 of 6 CANADIAN TAX INFORMATION In accordance with Canadian tax law, taxes are collected on member dues and application fees relating to PMI and the following Canadian chapters: Canada's Technology Triangle (CTT), Durham Highlands, Lakeshore, Levis, Manitoba, Montreal, New Brunswick, Northern Alberta, Nova Scotia, Ottawa Valley, Regina/S. Saskatchewan, Southern Alberta, South Western Ontario, and Southern Ontario. Canadian residents should include applicable taxes in the space provided. The rate of tax and the total amount that you owe will vary depending on the province where you have permanent residency. Tax calculations by province are 15% for Nova Scotia; 14% Prince Edward Island; 13% for New Brunswick, Newfoundland/Labrador and Ontario, % for Quebec; 12% for British Columbia, 5% for all other provinces. If your employer is paying for your membership and has been granted tax-exempt status by the appropriate Canadian authorities, please attach a declaration or other evidence to avoid being billed for unpaid taxes. 6 Registration Form Cancellation/Refund Policy Global Congress and SeminarsWorld Full refunds, less processing fee per registrant, will be granted for cancellations received in writing six (6) weeks prior to the event (25 March 2016). Refunds less per registrant will be granted to written requests received between 25 March and 22 April Registration substitutions may be made in writing without penalty prior to 22 April If a nonmember substitutes for a member, nonmember rates are applied. Refunds will not be granted after 22 April For SeminarsWorld, if PMI cancels a seminar for any reason, they assume no responsibility for nonrefundable airline tickets or other travel costs. PMI will make every effort to immediately notify registrants of a cancellation of a seminar. PMI reserves the right to cancel any seminar that has insufficient enrollment.
