1 FREQUENTLY ASKED QUESTIONS What Is a Legal Plan? A group legal plan is a low-cost and convenient employee benefit designed to meet the personal legal needs of today s workforce. By choosing this benefit, plan members receive a full range of personal legal services paid for through payroll deductions. Why Should I enroll in The Legal Plan? - Each year, more than 50% of moderate-income households have at least one new or ongoing legal problem. Yet most of these families don t seek legal help. Instead, they attempt to deal with the problems themselves, turn to non-legal third parties for help, or take no action. - Most people don t have an attorney, don t know how to select an attorney, or believe they can t afford representation. Easy, affordable access to professional legal counsel is the most fundamental benefit the legal plan provides. - Group purchasing power provides tremendous value. Participating in a legal plan is like having at attorney on retainer at a discount. The more often you use it, the more value it delivers. In many cases, the plan pays for itself with just one use. What are Hyatt Legal Plans Credentials? - Hyatt Legal Plans, a MetLife company, is the largest provider of group legal plans in the United States and has been in the business since Hyatt services over five million people at more than 1,200 leading organizations. What are the Covered Services? MetLaw offers telephone advice and office consultations for an unlimited number of matters and full representation for the following: Preparation of Wills (simple or complex), Living Wills, and Trusts (revocable and irrevocable) Purchase, Sale and Refinancing of Primary Residence Purchase, Sale and Refinancing of Second or Vacation Home Debt Collection Defense, Identity Theft Defense Personal Bankruptcy Civil Litigation Defense, including Administrative Hearings and Incompetency Defense School Hearings Pet Liabilities Tenant Negotiations and Eviction Defense (tenant only)

2 Name Change Prenuptial Agreement Adoption, Guardianship or Conservatorship Preparation of Powers of Attorney, Affidavits, Deeds, Demand Letters, Promissory Notes, Mortgages Review of Personal Legal Documents Juvenile Court Defense Elder Law Matters Consumer Protections and Small Claims Assistance Personal Property Protection Restoration of Driving Privilege and Traffic Ticket Defense (No DUI) Protection from Domestic Violence Boundary-Title Disputes Zoning Applications Tax Audits What Services are Excluded/Not Covered? Excluded Services are those legal services that are not provided under the plan. No services, not even a consultation, can be provided for the following matters: Employment-related matters, including company or statutory benefits Matters involving the employer, MetLife and affiliates, and plan attorneys Matters in which there is a conflict of interest between the employee and spouse or dependents in which case services are excluded for the spouse and dependents Appeals and class actions Farm and business matters, including rental issues when the Participant is the landlord Patent, trademark and copyright matters Costs or fines Frivolous or unethical matters Matters for which an attorney-client relationship exists prior to the Participant becoming eligible for plan benefits. Are my Dependents Covered? - Your monthly payroll deduction of $16.50 covers services for the employee s spouse/domestic partner and dependents. - Hyatt will use the company s dependent definition. What are Hyatt Legal Plans Standards for Network Attorneys? - Hyatt s plan attorneys must meet stringent criteria before joining Hyatt s network. - Plan attorneys are regularly reviewed to ensure they maintain Hyatt s high standards. - Plan attorneys have an average of 23 years of experience.

3 Can I Select an Out-of-Network Attorney? - You always have the option to select an out-of-network attorney. A fee reimbursement schedule that shows the maximum amount payable for specific services under the plan is available in BenefitSource. - To receive reimbursement, you simply call Hyatt s Client Service Center at (800) and request a Fee Reimbursement form. After you complete the form, attach the attorney s bill and submit it to Hyatt. - If the out-of-network attorney s charges are in excess of the maximum amount payable, the excess is the responsibility of the employee/plan member. How long am I required to stay in the plan? - You must remain in the plan for one benefit year. - New employees may enroll when they join the company and are required to remain in the plan for the remainder of the benefit year. What Happens If I am on an Unpaid Leave of Absence? Group Legal Plan benefits are suspended during an unpaid leave of absence. If enrolled, you will not be eligible for Group Legal Plan benefits during an unpaid leave of absence. Group Legal Plan benefits resume upon return to work. What Happens If I Leave Visa? If your employment with Visa ends, the Plan will cover the legal fees for those covered services that were opened and pending during the period you were enrolled in the plan. Of course, no new matters may be started after you become ineligible. PORTABILITY Hyatt Legal Plans If you are enrolled in the legal plan and your employment with Visa Inc. terminates, you can continue coverage by porting the plan. To do so, you must contact Hyatt Legal Plans' Client Service Center, at , within 31 days of his/her termination date and indicate that you want to port the plan. You will be required to pay a fee upfront directly to Hyatt Legal Plans, equaling the monthly group legal plan rate $17.50 multiplied by the number of months in the set portability period (currently 30 months). Coverage during the portability period is the same as group coverage: the plan design and dependent coverage remain the same. Please note that portability is currently a set period of 30 calendar months, but that period is subject to change by Hyatt Legal Plans.

4 How Do I Access and Utilize Plan Services? As a member, you are able to access plan services online or via telephone. Online Access Once you enroll for this benefit you will receive a welcome letter with your Membership Number. This Membership Number verifies your eligibility. To access the plan services, go online to Click on Members Log In at the top of the page. Enter your Membership Number and home zip code to access the plan. Your Membership Number can also be used by your spouse/domestic partner and/or eligible dependent child(ren) Click on "Covered Services" to review the services available and determine whether your legal matter is covered (your Network Attorney will make the final determination); Click on "Attorney Locator" to find the most convenient Network Attorney Office for you Click on "Obtain Case Number" to make note of this case number as you will need to provide this to your Network Attorney. Call the Network Attorney you have selected to schedule an appointment at a time convenient to you. Advise the attorney's office that you are a new client referred by Hyatt Legal Plans and would like to schedule an appointment.. Evening and Saturday appointments are also available. Telephone Access To use the plan, you can also talk to a Customer Service Representative by calling Hyatt s Client Service Center at Monday through Friday 8am -7pm Eastern time. Please be sure to have your Membership Number available when you call. The Client Service Representative will: Verify the caller s enrollment for services Make an initial determination of whether and to what extent the employee s case will be covered (the plan attorney will make the final determination of coverage) Give the employee a Case Number Provide the employee with the name and telephone number of the Plan Attorney most convenient to the employee Answer any questions the employee might have about the plan After receiving a Case Number, you may call the Plan Attorney for an appointment. Appointments are available evenings and Saturdays. Who Do I Call For Questions?
