Oklahoma State University Cooperative Extension Service shall provide all bin liners, restroom soap, hand towels and/or paper products unless otherwise stated and approved by both parties in this agreement. 2. The terms of this agreement shall be a one year period beginning July 1, 2015 at /month unless notice to cancel this agreement has been given, in writing, by either party by mail or in person thirty (30) days prior to the end of term. a. Pricing for cleaning not included in monthly charge is as follows: b. Carpet Cleaning.18 cents a square foot Strip and wax.28 cents a square foot 3. If a party in this agreement fails to perform according to its obligation hereunder ( the nonperforming party ), the party claiming non performance ( the claiming party ) will send the non-performing party written notice by USPS mail, specifying the manner of non performance. This notice will provide that the non performing party will have fifteen (15) days from the receipt of the notice to correct the items of non performance. If these items have not been corrected within the fifteen (15) day period, the claiming party may then terminate this agreement by sending the non-performing party a thirty (30) day notice of termination by USPS mail.

Report any damage, breakage, and/or visible plumbing problems and/or security breach that are noticed during cleaning operations to OSU Extension Center immediately in writing or to an emergency contact if necessary. 5. Secure all access points to the building upon completion of cleaning and exiting the building as instructed by OSU Extension Center. 6. Apply Yourself Janitorial shall invoice for monthly services of which payment is due thirty days from invoice date. Insurance and indemnification: Apply Yourself Janitorial shall furnish certificates of insurance as follows: Worker s Compensation insurance as required by law. Comprehensive General (Public) Liability to include (but not be limited to), the following: coverage for work at the OSU Extension Center with limits of not less than 100,000.00/300,00.00/100,000.0o with excess umbrella of liability 1,000, Apply Yourself agrees to defend, indemnify and hold harmless the OSU Extension, its officers, employees, agents and servants for claims for death, bodily injury and damage to tangible property caused by the sole negligence or a wrongful act of Apply Yourself Janitorial in the performance of this Agreement, and to pay all claims, damages, judgments, legal costs, adjuster fees and attorney fees related thereto.
