1 FORT SETTLEMENT MIDDLE SCHOOL SCIENCE SAFETY CONTRACT FBISD SCIENCE SAFETY CONTRACT FORT SETTLEMENT GRAPHING CALCULATOR CONTRACT Fort Settlement Middle School requires that all students participating in science in 6th, 7th, and 8th grade complete the following items: 1. Lab safety training video and in class safety training. 2. Lab safety test in which student must score 100% 3. Lab safety contracts for FSMS and FBISD signed by student and guardian 4. Graphing calculator agreement signed by student and guardian Fort Settlement Science Safety Contract The Fort Settlement / FBISD Science Lab Safety Regulations and Consequences are in place to minimize risk and prevent hazards in science classes and labs. Fort Settlement Middle School has established the following regulations and consequences for laboratory participation in compliance with the FBISD student safety contract rules and regulations. 1. Perform activities specifically authorized by your teacher. 2. Listen to all instruction and read all procedures before handling any lab equipment. 3. Know the location and appropriate use of all safety equipment in the lab. These include but are not limited to: fire extinguisher, fire alarm, water sources (eye wash, chemical shower, sinks), telephone, fire escape route nurse, first aid kit, and fire blanket. 4. Report all accidents to teacher immediately (injury or equipment damage) 5. Chemicals should only be handled under direct teacher supervision. 6. Use all protective equipment (goggles, aprons, gloves) when instructed to do so by your teacher. 7. Never eat, taste, drink, or inhale substances during a lab investigation. 8. Place all broken glass and chemicals in designated disposal containers. 9. Jokes, pranks (horseplay) are not tolerated in lab at any time. 10. Wash hands with soap and water after handling chemicals, plants, cultures, and animals. Avoid facial contact with hands until cleaned.

2 11. Keep combustible materials away from flames. 12. Clean your lab work area and equipment. All electrical equipment should be unplugged and stored safely before you leave the lab. 13. Never remove lab equipment / lab materials from the classroom. 14. Read and understand all instructions before proceeding with a lab. CONSEQUENCES: Students in violation of any Fort Settlement / FBISD Science Safety regulation may incur the following consequences: Immediate removal from the lab activity Loss of procedure points for the lab activity Disciplinary documentation. Parent/guardian notification. Referral to grade level Principal. Payment for any damaged equipment. I have read and understand the Fort Settlement Science Department Regulations and Consequences. I agree to follow these regulations and any additional printed or verbal instructions provided by my teacher or the Fort Bend Independent School District during the entire time I am a student at Fort Settlement Middle School. 3 FBISD STUDENT SAFETY CONTRACT I WILL: Follow all instructions given by the teacher. Protect eyes, face, hands, and body when involved in science experiments. Clean and dry my lab work area at the end of the lab. Wash my hands after each lab experiment. Know where to get help fast. Know the location of first aid, eye wash, fire blanket, and fire extinguisher. Conduct myself in a responsible manner at all times. I have been instructed in the lab safety and emergency techniques needed for my science class. I have made 100% on the lab safety quiz or test, which allows me to participate in lab activities. I understand and agree to follow the lab safety regulations set forth above and in the Lab Safety Guide I received from my teacher. I am aware that my safety and the safety of my classmates depends on my behavior in the laboratory. 4 Eye and Face Protection Guidelines 1. Eye and face protection shall be required where there is a reasonable probability of injury that can be prevented by such protection. 2. Safety goggles shall be worn by every teacher and student in labs when grinding or cutting solids heating materials working with chemicals 3. Prescription lenses and contact lenses are not eye protection devices, therefore, if eye hazards are present, appropriate eye protection must be worn in conjunction with the prescription lenses and or contact lenses. Contact Lenses in the Laboratory The use of contact lenses in some laboratory environments can pose a danger to the eyes and/or the lens. Listed below are some facts to be considered concerning contact lens use in the laboratory environment: Should an accident occur which involves splashing chemicals into the eye, the lens may hold the chemical on the eye. In such an accident as described above, the time it takes to remove the lens is added to the time before washing and/or medication can be administered. Soft contact lenses may increase the risk of eye injury because they may pick up chemicals which enter the air as fumes. In such cases damage may occur to the lens, if not to the eye. In spite of the above facts, there are teachers and professors who have continued to wear contact lenses in the laboratory and have not experienced any difficulty. Considerations for the wearing of contact lenses in the laboratory are: Routine wearers of contact lenses may suffer "spectacle blur" when they switch to spectacles, and this temporary reduction in visual efficiency could result in misreading of labels. Contact lenses provide better peripheral vision than eyeglasses. The use of unvented goggles will provide the appropriate protection in most cases. The decision to wear or not wear contact lenses in the laboratory should, therefore, be the student's and the parent's. Please check the appropriate choice below, sign and return to your son's/daughter's teacher. Whatever your decision should be, it is up to your son/daughter to follow your choice My son/daughter does not wear contact lenses and will wear goggles during labs when required to do so. My son/daughter, will wear contact lenses under unvented goggles during labs. My son/daughter, will remove contact lenses prior to lab and will wear glasses under the goggles.

5 Calculator Contract Rules for Acceptable Use of School-Owned Graphing Calculators The Science and Math Departments are pleased to offer our students access to schoolowned graphing calculators. The Science and Math departments have established rules for acceptable use of the graphing calculators. We ask that students and their parents/guardians read and discuss these rules together. Access to school calculators is a privilege not a right. Students/parents can chose not to accept the offer to use a school calculator. Student and parent/guardian is expected to sign and return the Student contract for Use of School-Owned Graphing Calculators acknowledging the student's understanding of the rules and responsibilities of using school owned graphing calculators and the consequences of violating that contract. Upon election to make use of a school-owned graphing calculator, a student is expected to follow the rules of acceptable use. Rules for acceptable use of school-owned graphing calculators: 1. Only students who have returned a parent signed Student Contract for Use of School-Owned Graphing Calculators are permitted to use a school-owned graphing calculator. 2. Each student who has parental permission to use a school-owned graphing calculator will be assigned a specific, numbered calculator for each specific activity (middle school), for entire year (high school) for his/her use in class. (Note that a student's assigned calculator will also be assigned to other students in others class periods.) 3. School-owned graphing calculators may only be used under direct teacher supervision. 4. Students may only use the school-owned calculator assigned to them. 5. Students may not remove school-owned calculators from the classroom. 6. Students notify teacher immediately if school-owned calculator is faulty due to physical damage, data/program corruption, or low batteries. 7. Students notify teacher immediately if any inappropriate data is found on school-owned graphing calculators. 8. Students are prohibited from using school-owned calculators in a way that would bring harm to others or disrupt use by others, such as file intrusion, or committing any electronic theft or vandalism on another users work. 9. Students understand that data stored on school-owned calculators is not private.

6 10. Students are expected to use proper etiquette and language when e entering data or other information onto school-owned graphing calculators. Guidelines for Calculator Memory: 1. Students are prohibited from changing any school-owned graphing calculator settings, configurations, or programming except when instructed to do so by their instructor. 2. Students may not install any software, programs and/or other utilities onto school-owned graphing calculators unless instructed to do so by instructor 3. Students may not remove any installed software, programs and/or other utilities from school-owned graphing calculators unless instructed to do so by instructor. 4. Students are not allowed to open school-owned graphing calculator cases or make modifications to calculators unless instructed to do so by instructor. 5. Instructors or other authorized school personel may periodically review school-owned graphing calculator memory contents to verify appropriate use. 6. Students may not access or download any programs, files, or information via computer graph-link without permission from instructor. III. Consequences for violation of the Rules of Acceptable Use: Written essay describing how the student violated the Calculator Contract-essay must be signed by parent. Financial responsibility for all costs associated with the restoration of a damaged or misused school-owned graphing calculator including equipment repair/replacement and labor. I will closely follow oral and written instructions provided by my teacher during the use of my assigned school-owned graphing calculator, and I understand the consequences for violating the rules of acceptable use. Check one: I give my permission for my child to use an assigned school-owned graphing calculator for classroom instructional purposes. I certify that the information on this form is correct. I do not give my permission for my child to use an assigned school-owned graphing calculator. However, my child may still use his/her personal graphing calculator for instructional purposes.
