Ryan, on the other hand, had been insured by VIPA Loss of Licence Insurance. His family will receive the maximum life insurance available of $250,000. Jack s MBF cover and Ryan s VIPA insurance costs were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $2, Pilot Contribution: $0
2 PAY AN IMMEDIATE LUMP SUM? Pilot Details: Permanent Loss of Licence, Illness Age 52 B737 Captain Salary $190,889 MBF Joining Age 31 MBF Benefit VIPA Benefit $740,000 lump sum $13,521.30/month, $486,766 lump sum after two years Paul and Mark are 52 year old B737 Captains at earning $190,889. Both have suffered side effects of cancer treatment, causing permanent loss of licence. Paul, with maximum MBF cover, will receive an immediate $740,000 lump sum. Mark, covered by VIPA Loss of Licence Insurance will receive monthly payments of $13, per month for two years (equal to 2 times 85% of his salary divided by 24 monthly payments) and then receives a lump sum payment of $486,766 at the end of the second year (equal to 3 times 85% of his salary). The total sum Mark is eligible to collect under the VIPA Loss of Licence Insurance is $811,889 however, the lump sum payment is not available immediately. The majority of Paul s MBF cover and Mark s VIPA Insurance were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $3, Pilot Contribution: $507.86
3 MAKE PAYMENTS BASED ON A PERCENTAGE OF SALARY? Pilot Details: Permanent Loss of Licence, Heart Condition B737 F/O Age 41 Salary $124,078 MBF Joining Age 38 MBF Benefit $800,000 VIPA Benefit $527, Lauren and Kate are 41 year old B737 First Officers at with an annual income of $124,078. Recently, the women have been diagnosed with a heart condition that has caused permanent loss of licence. Lauren, with maximum MBF cover, receives an immediate $800,000 lump sum regardless of her salary. Kate, covered by VIPA Loss of Licence Insurance, only receives $527, because her benefits are based on salary (equal to 2 times 85% of her salary for the first two years as monthly payments and 3 times 85% of her salary as a lump sum payment at the end of year two). Lauren s MBF cover and Kate s VIPA insurance costs were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $2, Pilot Contribution: $0

Pilot Details: Permanent Loss of Licence, Mental Illness A330 F/O Age 34 Salary $130,158 MBF Joining Age 32 MBF Benefit $800,000 VIPA Benefit $304, Alex and Dan are 34 year old A330 First Officers at earning $130,158. Recently, both men have been diagnosed with serious mental illnesses, and have permanently lost their licences due to failing the CASA Class One Medical. Alex, with maximum MBF cover, receives an immediate $800,000 lump sum. Dan, covered by VIPA Loss of Licence Insurance will receive up to $304, over two years. The VIPA Loss of Licence Insurance pays monthly benefits over the first two years (a total sum equal to 2 times 85% of salary) and only 25% of the scheduled lump sum benefit the pilot is entitled to at the end of year two. This decrease in benefit payment is due to the special conditions that apply to pilots who lose their licences either directly or indirectly due to any mental or nervous conditions. Alex s MBF benefits are paid in full regardless of mental health conditions. Alex s MBF cover and Dan s VIPA Insurance costs were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $2, Pilot Contribution: $0
5 OFFSET AGAINST OTHER INCOME? Pilot Details: Permanent Loss of Licence, Back Injury Ejet F/O Age 38 Salary $99,262 MBF Joining Age 37 MBF Benefit $216,000 VIPA Benefit $168,464 Andrew and Melissa are 38 year old Ejet First Officers at earning $99,262. While commuting to work together, they were in a car accident. The two pilots suffered back injuries causing temporary loss of licence. Andrew, a member of the MBF, will receive $9,000 monthly benefits for up to two years, amounting to $216,000. Melissa, who is covered by VIPA Loss of Licence Insurance, will receive monthly benefits up to two years amounting to $168,464 (based on 2 times 85% of salary). If Andrew earns other income his MBF benefits will continue to be paid in full, whereas Melissa s payments will be offset against any additional income earned. Andrew s MBF cover and Melissa s VIPA insurance costs were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $2, Pilot Contribution: $0

They have decided to go on RORO to be able to spend equal time caring for their children. During a bicycle ride through the country, they were hit by a 4WD and suffered debilitating spinal cord injuries. These injuries caused permanent loss of licence. Julie, with maximum MBF cover will receive an immediate $800,000 lump sum. Robert, covered by VIPA Loss of Licence Insurance, will only receive $405, because his benefits are based on his RORO salary (not full time income). His VIPA Loss of Licence Insurance is equal to 2 times 85% of his salary for the first two years as monthly payments and 3 times 85% of his salary as a lump sum payment at the end of year two. Julie s MBF cover and Robert s VIPA insurance costs were reimbursed by under the terms of the EBA negotiated by AFAP. MBF Cover: $2, Pilot Contribution: $0
