The clinic times are used for clients with no insurance who have a medical problem. 5 Adler Geriatric Assessment Center Yale New Haven Hospital 20 York Street New Haven, CT Telephone: (203) Fax: (203) Web Site: Medical Resource Directory for CMHC Geriatric Services The Dorothy Adler Geriatric Assessment Center is an outpatient consultative service that provides comprehensive assessment of older persons. The center works with persons who have medical, psychological, cognitive or social problems impeding function or threatening independent living. The goal of the assessment is to identify and treat corrective problems and to locate and coordinate community resources to optimize the patient s and family s living situation. The Centers staff consists of Geriatric physicians and Psychologists, Case Managers, a Geriatric nurse practitioner, and a Patient Care Associate. Medical Director: Sidney T. Bogardus, M.D. Liaison: Kathy Maturo Monday, Wednesday and Thursday 8:30AM to 5:00 PM Tuesday and Friday 8:00AM to 4:30 PM Medicare covers approximately 80% of the charges for the visit. 13 Medical Resource Directory for CMHC Pain Services Advanced Diagnostic Pain Treatment Center 150 Sargent Drive New Haven, CT Telephone (203) Fax (203) The Advanced Diagnostic Pain Treatment Center runs presently as a private practice model focusing on the management of chronic pain. All the physicians are board certified and fellowship trained in pain management. Pain is managed in a variety of ways using the most conservative approaches first. Treatment may include medications, trigger point injections, epideroscopy, etc. There is an affiliated research laboratory in Albany and Providence. The providers also consult at Hospital of Saint Raphael and Hospice.
