3 Department of Health (DH) pathfinder project was agreed between the DH and NHS Leeds South and East CCG, with the objective: To deliver a feasibility study and model for potential roll-out Work collaboratively with an independent sector partner that enables consortia to more effectively and efficiently segment its population and target its engagement Also exploiting technology and private sector expertise

4 Desired outcomes and objectives from stakeholders NHS Leeds South & East CCG/DH Objectives Creation of a solution to enable effective patient, carer and public involvement Recruit 5000 (2.5%) local people to the involvement network Share the learning with other NHS organisations MSD Objectives Improve understanding of diverse populations and how to engage with them Assess transferability of engagement method To pilot the development of a commercial software tool to support PPE Desired outcomes for Public & Patients Awareness of opportunity to get involved in shaping services The choice to engage on health issues in which they have a personal interest in a way which works for them Patients will influence pathway redesign and commissioning to meet local need

5 Project Outcomes Outcomes for NHS Leeds South & East CCG/DH An engagement solution capable of segmenting the population according to health interest and choice on engagement method The engagement solution to demonstrate evidence of increased involvement of local people in decision making and service design by the CCG Outcomes for MSD Increased understanding of the broader development of CCGs as part of the Health and Social Care Act 2012 A commercial solution created through Joint Working which supports patient engagement and our customers in a time of change Outcomes for Public & Patients 3791 people aged 19 years plus (2.4% of the south and east Leeds area) who have the opportunity to help shape the services the CCG commissions. Giving people the choice of the involvement method that fits their lifestyle.

9 How was it developed Database development began with a joint meeting of NHS Leeds South & East and MSDi to discuss the aims and objectives for the database within the Project and to establish a list of basic requirements. Following the meeting, the NHS Leeds South & East Project Manager was able to sketch out the initial requirements and expectations and submit these to MSDi. Using these, a preliminary version of the database was developed and shared with NHS Leeds South & East for comment and next stage development. This process continued as the database system developed with regular teleconferences taking place between both teams with amendments, additional requirements and a number of iterations of the database being implemented as the development continued.

13 Key Features The software is capable of effectively segmenting its membership data by demographic information and interests It creates an involvement history for each member. It also offers the ability to capture you said, we did outcomes from engagement activity.

14 Who can use it The database can be used across the whole locality and/or indivdual practices When a patient is enrolled there is an option to restrict their data to just the practice or shared with the CCG Practices can create their own projects and invite patients or CCGs can create cluster wide projects and events
