1 At Acting for Kids and Teen we teach acting for TV/Film specifically. Our coaches are all working actors, have studied acting and love working with kids. Our classes focus on learning acting skills for TV/FIlm and give kids a chance to be around other kids who are interested in the arts. Registration for our weekly acting classes is open and on-going. We also offer workshops throughout the year that focus on specific aspects of acting and other industry topics. Upon registration you will be asked to acknowledge and accept the following policy and waivers. If you have any questions please feel free to reach out. or Ongoing Class Tuition: Tuition is $195 per month, charged to your credit card on the first of each month, with a "rolling" enrollment. Meaning once you register we will continue to charge your card and keep your student on the roster unless you provide written notice by the 20th of the month for the next month's classes. Cancellation notices received after the 20th of the month will be processed for the following month, ie, cancellation notice received October 27, cancellation will be extended to December 1, with tuition for November charged on November 1. Mid-month registration will be pro-rated. We accept Visa, MasterCard, Discover and AMEX. Covid 19 Waiver: I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW) has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW) can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio staff, and other studio clients and their families. I voluntarily seek services provided by Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW) and acknowledge that I am increasing my risk to exposure to the pg 1

2 Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my class or workshop. I attest that my child and/or myself: Are not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. Have not traveled internationally within the last 14 days. Have not traveled to a highly impacted area within the United States of America in the last 14 days. I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. Have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities. We are following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. I hereby release and agree to hold Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW) harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW). I understand that this release discharges Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW) from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Acting for Kids & Teens, LLC (AFKT) and The Studio NW (TSNW). This liability waiver and release extends to the studio together with all owners, partners, guest teachers and employees. pg 2

3 Enrollment Make-ups: On Going Classes- If a student misses a class we encourage them to take advantage of make-up classes. They need to take a makeup class within three months of the missed class. Make-ups are forfeited if the student is no longer enrolled in on-going classes. No more than 2 make-ups can be taken in a month unless prior arrangements have been made. Habitually missed classes will result in a discussion to determine the student s commitment to Ongoing Classes. Tuition is not prorated due to missed classes. Workshops: If a student misses a workshop they are allowed to use it as a credit toward another workshop within that same calendar year. We offer only account credits for missed or cancelled workshops. If make-up workshops are habitually missed more than three times, the fee is forfeited. Photo/Video Release: By signing this release form, THE STUDIO NW/ACTING FOR KIDS AND TEENS, LLC, to use the following personal information: (1) My picture - including photographic, motion picture, and electronic (video) images. (2) My Voice - including sound and video recordings I hereby grant to THE STUDIO NW/ACTING FOR KIDS AND TEENS, LLC, it's subsidiaries, licensees, successors and assigns, the right to use publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (Video) form, sound and video recordings of my voice, and printed and electronic copy of the information described in sections (1) and (2) above in any and all media, including without limitation, cable and broadcast television and the internet, and for exhibition, distribution, promotion, advertising, sale, press conferences, meetings, hearings, educational conferences and in brochures and other print media. The permission extends to all languages, media, formats and markets now known or hereafter devised. This permission shall continue forever unless I revoke the permission in writing. I further grant THE STUDIO NW/ACTING FOR KIDS AND TEENS, LLC all right, title, and interest that I may have in all finished pictures, negatives, reproductions, and copies of the original print, and further grant THE STUDIO NW/ACTING FOR KIDS AND TEENS, LLC the right to give sell, transfer, and exhibit the print in copies or facsimiles thereof, for marketing communications, or advertising purposes, as it deems fit. pg 3

4 I hereby waive the right to receive any payment for signing this release and waive the right to receive any payment for THE STUDIO NW/ACTING FOR KIDS AND TEENS, LLC use of any of the material described above for any of the purposes authorized by this release. I also waive any right to inspect or approve finished photographs, audio video, multimedia, or advertising recordings and copy or printed matter or computer generated scanned image and other electronic media that may be used in conjunction therewith or to approve the eventual use that it might be applied. I acknowledge that I have read the foregoing and I fully understand the contents. Waiver: Assumption of Risk RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AND PARENTAL CONSENT AGREEMENT ("AGREEMENT") IN CONSIDERATION of being permitted to participate in any way in the The Studio NW/Acting for Kids & Teens LLC classes and/or workshops, and any activities associated with The Studio NW/Acting for Kids & Teens LLC and Katie O'Grady I, for myself for personal representative, assigns, heirs, and next of kin: 1. ACKNOWLEDGE, agree, and represent and I understand the nature of classes and that I am qualified, in good health, and in proper physical condition to participate in such activities. I further agree and warrant that if at any time I believe conditions to be unsafe I will immediately discontinue further participation in any classes, workshops, or activities associated with The Studio NW/Acting for Kids & Teens LLC and or Katie O'Grady. 2. I FULLY ACCEPT AND ASSUME ALL RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation in Acting for Kids & Teens LLC/The Studio NW classes, workshops, or activities associated with Acting for Kids & Teens LLC/The Studio NW and Katie O'Grady. 3. I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Acting for Kids & Teens LLC/The Studio NW, Katie O'Grady, her respective administrators, directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers, and if pg 4

5 applicable, owner and leaser of premises on which the activity takes place, (each considered one of the "RELEASES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASE", AND I FURTHER AGREE that if, despite the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the "RELEASES", I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE "RELEASES" from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. I further agree to arbitration of such claims pursuant to the rules of the American Arbitration Association. 4. Should any portion of the agreement be held uncomfortable, it shall be severed from the agreement leaving the remaining portions enforceable. I HAVE READ THE AGREEMENT, FULLY UNDERSTAND IT'S TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY THE LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
