2 A Client s Need for Medical Care Client and support system Need for theirhome management Knowledge of local resources Minor symptoms - may affect ADLS, abilities, etc. Variety of Care Providers Appropriate services provided Critical issue or increasing symptoms may warrant outpatient medical care Need for increased care May require hospitalization or placement Never-ending changes (which may jump around) 2

3 Parkinson s Disease Progressive neurological condition that currently affects over onemillion people in the US with out 60,000 diagnosed annually About 90% of diagnoses happen after age 60. Characterized by motor problems including slowness of movement, rigidity, tremor, balance/gait problems, flattened affect, difficulty speaking or low voice volume, difficulty with fine motor movements, increased risk of depression (and other emotional problems), difficulty swallowing/chewing, urinary problems, constipation, skin problems, and sleep disruptions. No known cause but most likely related with the loss of neuron functioning particularly in the areas of one s brain related to dopamine production (which is linked to purposeful movement.

4 Who are the medical care providers potentially involved with someone diagnosed with Parkinson s Disease? Movement disorder specialists (neurologist specializing in Parkinson s Disease) Nurses and Social Workers (in multiple locations) Physical, Occupational, or Speech Therapists (may be in multiple settings such as hospital, outpatient, skilled nursing, or home health settings or privately hired) Transportation providers Pharmacists Neuropsychologists, psychiatrists, and counselors Neurosurgeons Home Health, Palliative Care, or Hospice Teams Homecare Family (immediate and extended or family of choice) or also other families that are affected Elder Law attorneys, financial planners, and Medicare Brokers Durable Medical Equipment providers Aging Life Care Managers Nutritionists/Dieticians Sleep disorder specialists Urologists and Gastroenterologists Residential Facility Staff Dentists Home Organizers, Move Managers, and Placement assistance Primary Care MD and non-related specialists Emergency Room and hospital providers and discharge planners Home Modification Specialists Veterans Based services Research or education based organizations such as the American Parkinson Disease Association

7 What other mechanisms fund medical needs outside of Medicare and health insurance? What else might be needed that is not Medicare-reimbursed? Medicare is the primary health insurance for older adults and adults with disabilities. What does Medicare cover?

8 For Financial Planners your code of ethics and rules of conduct clearly state that there is a principal of Competence as well as very specific obligations to clients regarding one s professional scope of services. Under competence, it is the attaining, maintaining, and providing of an adequate level of knowledge and skill specific to financial planning. However, it also states that financial planners also have the wisdom to recognize limitations of knowledge and to offer consultation with or referrals to other professionals are necessary outside of their own scope of services.

9 What is an Aging Life Care Manager? Typically, social workers, nurses, gerontologists or other health /mental health professionals Knowledgeable: specialized training, experience in the field, familiar with community resources Follows a strict Code of Ethics & Standards of Practice Hourly fee: $90-150/hour with assessment and family meeting traditionally taking approximately 10 hours. After assessment, the family directs care used with varying minimums.
