There may be further testing required that is not listed below. You may or may not have to do all the testing listed. SLEEP STUDY EGD UGI STRESS ECHOCARDIOGRAM NUTRITION CONSULTATION WEIGHT LOSS VISITS PSYCHOLOGICAL EVALUATION When criteria is completed and signed off, then your request for surgery will be submitted to your insurance company for approval. Once we receive your approval for surgery Liza Claire will call to inform you. If you receive an approval letter in the mail from your insurance company and have not been contacted please call Liza to inform her. Liza - will prepare your chart for the surgery scheduler. Tina- our surgery scheduler will call you to discuss surgery dates and any pre operative testing required. She will also inform you of when to start your pre operative diet and how long you should be on it. Tiffany - will contact you regarding any Surgical Fees for surgeon. 7 PRE OPERATIVE DIET Weeks Before Surgery 1. Sugar free clear liquids unlimited. Example: Crystal light, unsweetened tea, water, broth, decaf coffee (no sugar). 2. Sugar free popsicles. 3. Protein Shakes: Recommended Bariatric Advantage Brand 27 grams protein / serving grams (women) 3 shakes / day ** grams (men) 4 shakes / day Multivitamin daily. Why do I need to do this? The pre op liquid diet has two purposes: 1. To shrink your liver and make surgery easier for you and your surgeon. 2. To get you used to the liquid diet and protein shakes PRIOR to your surgery so you can pick what you like and what tastes good to you. SLEEVE AND REVISION PATIENTS YOU WILL RETURN TO THIS DIET IMMEDIATELY POST OP UNTIL YOUR DRAIN IS REMOVED.

8 STAGE 1 DIET FOOD YES NO MEAT, PROTEIN AND PROTEIN SUBSTITUTES: Baby-food meats (all types), Protein shakes-low carb, Cream soups-low fat. CRUNCHY PEANUT BUTTER, ALL OTHERS MILK AND MILK PRODUCTS: Fat-free (skim) or 1% milk, sugar free or low fat yogurt, (approximately) 80 calories per serving. CHOCHOLATE MILK, SWEETENED CONDENSED MILK, 2% OR WHOLE MILK, ICE CREAM FRUITS AND VEGETABLES: Applesauce, pureed banana, Other fruit/vegetables without seeds or hulls that have been pureed and thinned until liquid consistency. ALL OTHERS BREADS AND CEREALS: Hot cereals cooked in milk (grits, cream of wheat) thinned until liquid consistency. BREAD AND CRUNCHY CEREALS OTHERS: Sugar free gelatin or popsicles, sugar free fat free pudding / custard. CHEWING GUM, SWEETS PROTEIN SHAKES: Recommended Bariatric Advantage Brand 27 grams protein / serving. TOTAL PROTEIN NEEDED A DAY: grams (women) 3 shakes / day grams (men) 4 shakes / day. When do I use this diet? GASTRIC BYPASS: Home from hospital to two weeks post surgery. GASTRIC SLEEVE / REVISIONAL SURGERIES: After drain removed, when approved by surgeon. Lap Band: 1) Home from hospital to two weeks post surgery. 2) 24 hours after every band fill.

9 STAGE 2 FOOD YES NO MEAT, PROTEIN AND PROTEIN SUBSTITUTES: Lean soft and moist meats including fish, poultry, or pork, low fat cottage cheese, low fat or nonfat cheese, soft tofu, eggs or egg substitutes, soft low fat casseroles with soft vegetables. CRUNCHY PEANUT BUTTER, FRIED EGGS, ANY MEAT THAT IS DRY, TOUGH OR CHEWY. RED MEAT MAY NOT BE TOLERATED IN THIS PHASE. MILK AND MILK PRODUCTS Fat-free (skim) or 1% milk, sugar free or low fat yogurt, (approximately) 80 calories per serving. CHOCHOLATE MILK, SWEETENED CONDENSED MILK, 2% OR WHOLE MILK, ICE CREAM. FRUITS AND VEGETABLES Any fruit/vegetables without seeds or hulls that have been cooked soft. Soft unsweetened canned fruits or vegetables. Fresh soft fruit. ALL OTHERS-INCLUDING RAW VEGETABLES (such as salads) AND RAW CRUNCHY FRUITS (especially those with tough skin / seedy. BREADS AND CEREALS OTHERS Hot cereals cooked in milk (oatmeal, grits, cream of wheat), cold cereal (high protein) soaked in milk, toasted bread or plain soda crackers. Sugar free gelatin or popsicles, sugar free fat free pudding / custard. SOFT GUMMY BREAD AND CRUNCHY CEREALS OR SWEET CEREALS. CHEWING GUM, SWEETS When do I use this diet? GASTRIC BYPASS / SLEEVE/ REVISONS: 2 6 WEEKS POST OP. LAP BAND: 2-3 WEEKS POST OP.

10 STAGE 3 Food Yes Better Not No MEAT, PROTEIN AND PROTEIN SUBSTITUTES: Lean meats, egg or egg substitutes, tofu, regular low fat casseroles or crock pot meals Tough meat or meat with gristle. Some Patients do not tolerate red meat. FRIED OR BREADED MEAT, FRIED EGGS. MILK AND MILK PRODUCTS Fat-free (skim) or 1% milk, sugar free or low fat yogurt, (approximately) 80 calories per serving. CHOCHOLATE MILK, SWEETENED CONDENSED MILK, 2% OR WHOLE MILK, ICE CREAM. Fruits Fresh, canned, frozen or cooked fruit. Be cautious with apples, grapes or other fruits with tough peels or skins. Dried fruits. FRUITS THAT HAVE ADDED SUGARS OR SAUCES. Vegetables Fresh, canned frozen or cooked vegetables. Vegetables with very tough skins or a lot of seeds. FRIED OR BREADED VEGETABLE PRODUCTS. Bread Dry, coarse, or welltoasted bread, crispy low fat crackers or baked tortillas. Soft breads that become gummy, breads with a lot of nuts and seeds or dried fruit. SWEET BREADS, DANISHES, DOUGHNUTS OR PASTRIES. Cereals All cooked and dried cereals without added sugar. Cereals containing fruits or nuts. CEREAL WITH ADDED SUGAR OVER GRAMS PER SERVING. POTATOES, RICE, PASTA OTHERS Boiled, mashed or baked potatoes, well cooked whole wheat pasta or rice. Sugar free fat free pudding / custard. Potato skins, rice and noodles. Popcorn, nuts, chili or extremely spicy foods INSTANT NOODLE DISHES.FRIED, SALTY SNACK FOODS CREAMED SOUPS UNLESS LOW FAT VERSIONS, CHEWING GUM, SWEETS. When do I do this diet? GASTRIC BYPASS/SLEEVE/REVISIONS: After 6 weeks post op. LAP BAND: After 3 weeks post op.
