3 Intended Use RX - ONLY Intended Use Telserve Data Management System Professional Use The Telserve Data Management System Professional Use (Telserve Pro) is an accessory to blood glucose (BG) monitoring systems for the review and evaluation of BG test results to aid in diabetes management and patient care. Telserve collects data from blood glucose meters (BGMs) such as the Telcare BGM. General Precautions Glucose results from alternative testing sites should not be used for insulin dosing. The Telcare BGM and the Telserve system do not provide automated treatment decisions. They are not substitutes for professional clinical judgment. All patient diagnoses and treatment options are to be discussed and performed under the supervision of a Licensed Healthcare Professional (LHP). This system IS NOT intended for emergency calls or for transmitting time-critical data or alarms. Clinical judgment and professional experience are required to check and interpret the information transmitted. BG data may not immediately transfer to Telserve. BG readings on patient BGMs (and not the Telserve website) will determine treatment options. The Telserve system is intended for use by patients with diabetes who use the Telcare BGM. 3

4 Using the Licensed Healthcare The Telserve LHP portal is designed to work with internet browsers that implement the protocol. The tested browsers are: Internet Explorer, Chrome, Firefox, and Safari. Login Instructions: 1 Obtain login credentials by calling Telcare at: (888) Start your internet browser. 3 Go to 4 Log in using the username and password credentials set up with Telcare. 5 The LHP will be directed to the LHP Portal. 4

5 Using the Licensed Healthcare Home Page The toolbar at the top of the home page allows the LHP to navigate anywhere on the site, including Dashboard, Alert List, Refer Patient, Administration, and Log out. Dashboard Graphs To view BG and demographic information graphs, scroll over Dashboard in the toolbar. Dashboard: Allows the LHP filtering options based on their preferred criteria. Criteria include: Age, Diabetes Type and Gender; State, Zip Code and Status; Frequency, Compliance and Prescribed Readings; Extreme Hyperglycemia, Hypoglycemia, Out of Bounds Readings, and Hyperglycemia. 5

6 Dashboard Graphs Pages 6-8 illustrate each type of Dashboard Graph available for the LHP user. Each graph allows the LHP the opportunity to categorize their patients information and use this information to make decisions concerning treatment recommendations. 9 Alert List Alert List allows the LHP to set up alerts to be sent to the LHP based on Last Test Received, Testing Adherence, and Average BG by Zone. Last Test Received 9

12 Messaging Administration Page 1 To view and edit clinical message rules, click Administration from the toolbar. 2 Messaging Options: System Default: Automated s sent to the LHP. 3 Triggered Message Options: s sent only under certain conditions. The LHP defines these conditions. 4 Scheduled Notification Reports: s that can be sent daily, weekly, or bi weekly. 5 Send High Priority Message: s of vital importance can be set up here. LHP can set the criteria for High Priority messages. 6 Click save once the preferred criteria are satisfied. 12

13 Patient List Using the Licensed Healthcare 1 To view textual renditions of data, scroll down to Patient List within the Dashboard. 2 Click a header to sort the data. 3 Use the Search Function to search. 4 Click the patient s name to access their personal portal. (All BG data is located in the patient s personal portal.) 5 Click Export to download all data. 13

14 Using the Licensed Healthcare This is a general overview of the Patient Portal. For more detailed instructions on the Patient Portal, refer to the Patient User Manual located at mytelcare.com/support/new Patient Dashboard 1 The top portion of the Patient Dashboard screen shows BG data and test strip usage data. 2 The far right of the screen shows the Last Glucose Readings (including BGL, date, time, and tag). 3 To view additional readings click Get more 4 To view all of the patient s readings and sort them by Time Tag and Range, click All Readings on the top right of the page. 5 Select different timelines by clicking the buttons below your name. Options include Today, 7 Days, 14 Days, 30 Days, and 90 Days. 6 To download all of the patient s data in.csv format to your computer, click Export Data. 7 To manage the messages for a particular patient, click Message Settings. 8 To set a patient s BG target zones, click Clinical Profile. 15 Using the Licensed Healthcare Patient Graph 1 The middle section of the patient screen presents BG data in graphical format. 2 The graph on the left displays the average BG sorted by Reading Tags. The patient can scroll over Tagged Averages to see other available graph options including: Tagged Averages, Trending, Hourly Averages, Box Plot by Tag, and [DEV] Native Box Plots. 3 The graph on the right presents BG averages by date. Other graph options include: Tagged Averages, Trending, Hourly Averages, Box Plot by Tag, and [DEV] Native Box Plots. Latest Messages At the bottom of the page are the latest messages sent to the patient s BGM. If Telserve receives the reading after the date/time that the reading was taken, no clinical message will be returned to the BGM. 15

16 Using the Licensed Healthcare Messaging (Message Options System Default) 1 Message Options: Allows for basic messages to be sent via to the LHP. 2 Message Libraries: Enables the LHP to choose from a list of generic Default Messages, or messages that the LHP has added to the Message Library. 16

17 Using the Licensed Healthcare Messaging (Message Options Triggered) Triggered message options will automatically send an to the LHP when a patient s BG readings reach specific levels. The LHP can set the criteria for automated messages. Using the Licensed Healthcare 17

18 Using the Licensed Healthcare Messaging (Scheduled Notification Reports) Scheduled LHP Notification Reports. A Scheduled Notification Report will inform HCP s when patient(s) associated with their account have BG readings that meet the definition of a trigger as defined in the table of triggers below. Table 1 - Notification Report Definitions Trigger Definition Extreme Low Any user who has a BG <50. Extreme High Any user who has a BG >350. Sustained Highs Sustained Lows Any user who has had 5 consecutive readings of >250. Any user who has had 3 consecutive readings of <70. Pattern Highs Any user who has 3 readings >250 with the same tag, excluding No Selection, in 7 days. Pattern Lows Any user who has had 2 readings <70 with the same tag, excluding No Selection, in 7 days. Pattern Low Fasting BG Any user who has had 2 readings <70 with the tag Before Breakfast in 7 days. 18

19 Using the Licensed Healthcare Messaging (Scheduled Notification Reports) The LHP can also schedule s sent to the LHP at certain intervals: Daily, Weekly, and Bi Weekly. These messages can be turned on or off at the LHP s discretion. 18

20 Using the Licensed Healthcare Messaging (Send High Priority Message) The High Priority Message option allows the LHP to set up a message to be sent immediately to a patient s message queue. These messages are posted to the patient s message queue and will be delivered to the patient s Telcare BGM following the next reading. A message can be sent to an individual patient, or to all patients that are registered with the LHP. 19
