1 Oregon Physical Therapy Association 2013 Spring Business Meeting April 13, :00 1:30 pm Sheraton Portland Airport Hotel Portland, Oregon Election Candidates Secretary, Treasurer, Member at Large #1, #2, and #3 and Nominating Committee Member You will be asked to vote for one candidate for each position at the Spring Business Meeting. The Member at Large positions and the secretary and treasurer positions are all two-year terms; and the Nominating Committee Member position is a three-year term. Candidates are listed in alphabetical order by last name by category. Nominations will be accepted from the floor at the Business Meeting. If you are unable to attend the Spring Business Meeting, you can request an absentee ballot by contacting Kristie at the OPTA office at or or via at kvolk@opta.org. Or you can download one at Absentee ballots are due at the OPTA office by April 11, 2013 in order to be counted. Secretary Mary Hlady, PT, DPT, OCS Employment History: Gateway Rehab Clinic Lead PT, Providence Health & Services: orthopedics and chronic pain 2009-present, Escarpment Sports Medicine staff physical therapist. OPTA Activities: Public Relations committee chair 2010-present, Public Relations committee 2009/2010. Treasurer Kirstin Hamilton Pauken, PT Employment History: I have been in the field of Physical Therapy for 20 years and worked in variety of roles. Currently, I work for Kaiser Permanente as a home health physical therapist with my practice interest in geriatrics. Prior to my current position, my role was more administrative in nature, working as the Director of Rehabilitation also at Kaiser Permanente for over 5 years. I oversaw the budget reporting and ensured the financial health of the department. I have also worked in Inpatient Rehab, Outpatient Orthopedics, Acute Care, and even volunteered with Medical Teams International in Haiti after the earthquake. My diverse clinical background along with my administrative skills would allow me to represent and support the needs for the position at treasurer. OPTA Activities: I have recently begun to be more involved with OPTA activities, attending various business meetings, conferences, and PAC fundraisers over the past several years. I supported the previous Treasurer, on the Financial Investment committee and with some auditing tasks. I am excited to becoming more involved with OPTA activities
2 Pauken Continued APTA Activities: I have attended several Combined Sections Conferences. I have advocated for several of the house and senate bills in OR and WA, supported by APTA, promoting the scope of Physical Therapy. Comments: I am very excited to be considered for this role as OPTA treasurer. My combination of administrative skills, clinical experience, and exposure to the OPTA workings have prepared me well for this role. There are many changes going on in the field of Physical Therapy and Health Care locally and nationally. I look forward to expanding into this next professional opportunity where I can support OPTA in being financially sound so that we can best support our members' interest and voice. Justin Weatherford, PT, DPT Employment History: I have worked in Skilled Nursing, Outpatient, Acute Care, Inpatient Rehab, and now starting Home Health. I have served as staff therapist, single site director, multi-site director, and hospital department director. Completing patient care through high level strategic planning and budgeting. Clinically, I most enjoy working with neurologically impaired patients through the continuum of care. Managerially I most enjoy planning and executing strategic plans to position ourselves in the best spot to thrive with the ever changing environments of healthcare. Comments: I would be honored to serve in this position to help represent and further the priorities of the OPTA in the grand scheme of changing healthcare. My demonstrated toolsets and experience align greatly with achieving success going into the future of healthcare. Member at Large #1 - PT Licensing Board Liaison Joan Brassfield, PT, MS, CEEAA Employment History: 1998 until present: Infinity Rehab PT for Geriatrics in SNFs; Teacher of Restorative Aide Class; CCC for 3 years; DOR; Telehealth therapist; Sundance Rehab in SNFs with emphasis on geriatrics; Rehab Network in SNFs with emphasis on geriatrics; Good Samaritan Hospital in PHX AZ in acute care with emphasis on orthopedics; Gompers Rehab in PHX AZ working in the schools with pediatric population; Maricopa Medical Center in PHX AZ working in burn unit and acute care; St. Joseph's Hospital in PHX AZ in acute care with emphasis on neurological and orthopedic patients OPTA Activities: Alternate Delegate; Active Delegate; Nominating Committee. APTA Activities: Task force for Determining Minimal Education Requirements for PTA Education FSBPT; 2012 NPTE-PTA Standard Setting Task Force. Comments: I love being a Physical Therapist and learning new things. I have a broad base of experience and feel I could represent the OPTA's interest as a representative on the PT Licensing Board. I welcome the chance to serve and hope you will elect me to this position
3 Richard Jones, PT, MPT Employment History: Area Director and PT in SNF, Assisted Living, and Outpatient settings. OPTA Activities: Recently involved in OPTA get togethers, such as pub night, and gatherings at APTA CSM. APTA Activities: LAMP Leadership Development. Attended multiple Combined Sections Meetings in San Diego, Las Vegas, and New Orleans. Publications/Research/Awards: Research in Master Program Land versus Pool in Rehabilitation status post ACL knee surgery. Currently working on LAMP Leadership Development Program. Comments: I am very interested and involved in the ongoing Medicare regulations and reimbursement as it applies to the delivery of our services. It is important that we are an advocate for our profession and those we treat as we deal with the changing world of regulations and insurance plans. My involvement to learn more regarding policies and regulations started in 1999 with the phasing in of PPS/MDS and that desire continues today as Insurances, HMO's, and Medicare continues to evolve as it relates to our field. Paul Shew, PT, DPT Employment History: George Fox University- March 2011 to present; Providence Med Center 2011 to present (on Call Home Health); Providence Med Center 1981 to APTA Activities: Education Section Research Section. Publications/Research/Awards: Oasis inter-rater reliability and reimbursement: a study of inter-rater reliability of the Outcome and Assessment Information Set (OASIS); its effects on the Home Health Resource Group (HHRG) and reimbursement. Member at Large #2 Membership Debbie Van Dover, PT Employment History: PT since Primary clinical setting practice acute care. Currently program director of PTA program and full-time faculty since OPTA Activities: Active member since moving to Oregon in Previous member of Scholarship Committee and Membership Committee member before serving as committee chair since March APTA Activities: Attended APTA National Membership Chair Conference July Recently completed CCIP Trainer Course in order to conduct APTA workshops for CI credentialing courses. Publications/Research/Awards: Assisted multiple publishers in review of PTA education textbooks. Comments: I have enjoyed serving as the OPTA Membership Chair these past couple years and working with our current OPTA Board. Really appreciated the feedback in our recent member survey and looking forward to following up with activities for our new and potential members. Comments: I feel that my effectiveness as a physical therapist is enhanced by synthesizing different views and disciplines. (For example, my first career was as an engineer for Intel Corp. in the fast-paced and innovative high tech industry, followed by working as a Licensed Massage Therapist in a spa setting and independently.) I similarly feel that our profession is enhanced by bringing together all physical therapists in a common organization where we can synthesize all our varying viewpoints and experiences to promote our profession the OPTA/APTA. I serve on the OPTA Membership Committee because I believe that increasing our membership is a fundamental part of strengthening our profession by having all perspectives participating in the OPTA/APTA. I would like to serve on the nominating committee because I want to bring the best perspectives that we have within our community to leading positions within the OPTA/APTA. 5 Jane Montgomery, PT Employment History: For the past 21 years I have been the outpatient PT supervisor at Providence Portland. This involved supervising 60+ PT/PTA/Aide staff. I was in charge of all aspects of daily operation including staffing, hiring, performance management and leadership mentoring while focusing my patient care time primarily on spine and oncology patients. Additionally, I coordinated a special regional (five Providence area hospitals) team in charge of Rehab Human Resources which involved documentation of all aspects of rehabilitation HR. I also coordinated another regional team focusing on the needs of regional Rehab Oncology. This involved staff education, marketing and representing rehab at many stakeholder tables regionally. OPTA Activities: OPTA Board of Directors 2010 to present. Legislative Day in 2005 and Coordinate meeting with PT and PTA schools to promote the APTA and the Emerging Student Leader scholarship. APTA Activities: Oregon Chief Delegate to APTA House of Delegates 2010 to present. Active Delegate for six years prior to being Chief Delegate. March on the Capital 2011 and Currently I am serving on an APTA Work Group for Chief Delegates focusing on Delegate Education. Comments: It has been a privilege to serve with our dedicated Delegate Team. Since I recently retired from Providence, and am not currently employed, I have more available time to serve. I hope to use my skills to contribute to the fine work of the Nominating Committee
