The chapter describes historic and contemporary roles of professional social workers in public welfare and social services, including work in income maintenance, adult protective services, special housing programs, HIV/ AIDS services, kinship care, and domestic violence programs. It describes the dilemmas and tensions inherent in these roles. Geriatric Care Management Settings Carmen Morano and Barbara Morano acprof:oso/ This chapter describes social work practice in geriatric care management settings. It considers contemporary challenges that arise in these settings, including definitional issues around what constitutes care management as opposed to case management, the need for culturally relevant services, and developing professional credentials and identification in a relatively new area of practice. It begins by providing both historical and current perspectives on hospice care, including a differentiation between end-of-life, palliative, and hospice care, and it integrates current utilization and length-of-stay information from the National Hospice and Palliative Care Organization (NHPCO). The chapter's three main sections discuss (1) regulations and policies that guide hospice social work; (2) hospice care environments; and (3) hospice social work with older adults and their families. Although people of all ages enroll in hospices, the chapter focuses on older patients and their families. Community Mental Health Centers Sherry M. Cummings and Kimberly M. Cassie acprof:oso/ This chapter focuses on community mental health centers (CMHCs), which are traditionally underutilized by seniors. Tracing the background of social work practice in community mental health centers, the impact of deinstitutionalization and devolution is considered. The chapter identifies the emerging focus on behavioral health care, and a role for CMHCs in managed care. It cites recent studies suggesting that nearly all CMHCs provide services to older adults, and that the majority of centers now offer specialized geriatric services. Nonetheless, mental illness among older adults continues to be undertreated, and seniors continue to be underrepresented in these care settings. The increased CMHC use by seniors is expected in coming years, and the challenges this will pose for the setting itself and the professionals involved are outlined. Page 2 of 5

It offers an overview of managed care in the U.S., before presenting major debates about the impact of this care model on clients and professionals. It outlines important ethical dilemmas that confront social workers in managed care, with a focus on retaining the core values of the profession while developing the business acumen necessary to communicate their effectiveness in this industry. Social Service and Health Planning Agencies Robert B. Hudson acprof:oso/ This chapter examines the role of planning agencies at state and substate levels in improving the quality of life for older adults. Gates acprof:oso/ This chapter reviews the demographics and trends that are making the role of caregiver to the aged central to the lives of many, with consequences for the well-being of workers and their families, and for the effectiveness of their employing organizations. It explores what workplaces and society have done in response and suggests additional possibilities for both prevention and intervention, and the role social workers can play in achieving their implementation. Substance Abuse Networks Connie Corley, Muriel Gray, and Richard Yakimo acprof:oso/ This chapter summarizes epidemiological studies on the prevalence of alcoholism in older adults, as well as the diagnostic challenges presented by this population. It describes the service delivery network, and considers the role of Employee Assistance Programs (EAPs). It argues that the service delivery system is ill-equipped to meet the needs of older adults who suffer from substance use disorders and calls for professional education to alert social workers in all settings to the signs of these problems. This chapter provides a detailed look at case management as a venue for social work practice. It discusses the national demonstration projects that have served as the proving grounds for case managers, and highlights the tension over who owns case management.
