MISSION OF THE COLLEGE DeWolff Institute has, as its principal mission, the training of qualified Cosmologists and Cosmetology Instructors. To prepare them to pass the State Board Examinations; render good service to patrons; to instruct students effectively and prepare them for entry level employment in their chosen field; Presenting an organized approach to students and patrons by constantly endeavoring to improve the operation by encouraging staff to attend clinics, seminars, etc., to keep abreast with the ever-changing developments and techniques in all phases of our industry. ADMISSIONS REQUIREMENTS DeWolff Institute does not discriminate in its employment, admission, instruction, or graduation policies on the basis of creed, religion, race, color, ethnic, origin, religion, ancestry, national origin, residence, age, nondisqualifying disability, gender, financial status, sexual orientation, marital status, or veteran s nor does the

5 institute actively recruit students already enrolled in or attending another institution offering similar programs. DeWolff Institute admits as regular students the following. To be eligible for Admissions to the institute a student must: Be at least 17 years of age or older (proof by a State issued Id) o License o Passport o Id o Birth Certificate All applicants must be physically able to complete the course Be able to speak, read, and write fluently in English (all classes are taught in English) $25.00 money order payable to NM State Board of Barber & Cosmetology One of the following is accepted for high school education: High school graduates Certificate of Attainment A High School Graduation Equivalency Certificates (GED) If a transcript is used for proof of education, it must have a graduation date. Home schooling (state approved may need to be verified) A Student Instructor applicant must meet all of the above requirements and: hold a current license in the field they wish to teach complete and instructor in training application to be forwarded to the State Board of Cosmetology (as necessary) DeWolff Institute does not accept diplomas or GED s completed online or the ability to benefit test. If proof of education is questionable it will be checked with the New Mexico Office Higher of Education and approved by the institute. All diploma s or GED s must be taken in an approved testing facility. 6 INSTITUTE CALENDAR The Institute is in continuous operation year round, classes start the first Tuesday of each month. All of the following holidays are observed Memorial Day, Fourth of July, Labor Day, Thanksgiving Day(at noon the day before-thursday-friday 2and1/2 days) Winter Break (at noon on Christmas Eve re-turning on the day after New Year s day) and Good Friday. Additional days are published well in advance. Students are requested to listen to local TV (Channels 4 or 7) to obtain pertinent information regarding unexpected school closures due to extenuating circumstances. All of the above scheduled days off are included in the contract end dates. See the institute office for exact starting dates. CLASS SCHEDULE Full Time Schedule: 7:30a.m. - 5:30p.m. Tuesday through Friday Full time students are scheduled to attend an average of 35clock hours per week. Half Time Schedule: 8:30a.m.-1:30p.m. Tuesday through Friday (Flexible per student) Half time students are scheduled to attend an average of clock hours per week JOB DEMAND SURVEY The following information was obtained from a National Survey conducted in May 2010 by the Occupational Outlook Handbook. From the Bureau of Labor Statistics in 2010 barbers, cosmetologists, and hairdressers held about 712,200 jobs. Overall employment of barbers, hairdressers, and cosmetologists is expected to grow 14% (100,900) from 2010 to 2020, as fast as the average for all occupations. Physical stamina is important, because workers are on their feet for most of their shift. Many work part time, and nearly half were self-employed in The 2010 Median pay was $22, per year which is approximately $10.82 per hour. As of January 2010, there were 14,538 professionals employed in New Mexico's 2,187 salons.

6 7 COMPLETION RATE JANUARY 1, DECEMBER 31, Students were scheduled to graduate During this period there was a completion rate of 61.11% Licensure rate of 95.83% Employment rate of 90.48% STATE LICENSURE REQUIREMENTS Any person is eligible to receive a license who submits proof that he/she is at least 17 years of age: has an education equivalent to the completion of the second year of high school; has completed the required clock hours for the applicable course; has paid the required fees; and has passed the examination conducted by the Board to determine his/she fitness to receive a license. PROGRAMS OF STUDY All courses are taught in English. De Wolff Institute offers programs in the following areas of Cosmetology in a normal time period as noted below: Preparation for licensure as a professional, Cosmetologist clock hours 46 weeks Preparation for licensure as a professional, Instructor clock hours - 29 weeks CAREER OPPORTUNITIES Welcome to the world of cosmetology! The field of cosmetology as whole (including barbers, salon workers, nail technicians and estheticians) is projected to grow 20 percent between 2008 and 2018, which according to the US Bureau of Labor Statistics, is much faster than average. When our graduates succeed, we succeed!

7 8 POLICY REGARDING METHODS OF PAYMENT The student or parent of a dependent student that has entered into an agreement with DeWolff Institute regarding a Title IV, HEA loan has been informed that the loan will be submitted to the National Student Loan Data System (NSLDS), and will be accessible by guaranty agencies, lenders, and schools determined to be authorized users of the data system. OPTION I: CASH OPTION II: DEFERRED PAYMENT PLAN $ down payment, balance to be paid at the rate of $ month. No interest is charged under this plan. OPTION III: FINANCIAL AID PROGRAMS Student agrees that they are primarily responsible for paying expenses for programs selected, and that they will make the application for Financial Aid assistance. However, should a student become ineligible for Financial Aid assistance and / or not be able to cover full cost by way of assistance, the student will be referred to Option I or Option II to cover the remaining balance of their tuition. Student agrees that, under this option, funds will be disbursed according to policies set forth by the United States Department of Education and this Institute. Student agrees to provide all information required completing the application for aid, including copies of income tax and any other documents may be required. THE FOLLOWING APPLIES TO ALL OPTIONS LISTED UNDER METHOD OF PAYMENT: 1. All moneys due to the institute must be paid in full before report of program hours, transcripts of grades, and / or practical reports will be released. A charge of $20.00 will be incurred for transcripts sent to other schools or to students. REFUND POLICY A. An applicant, not accepted for enrollment by the institute, shall receive a refund of all monies paid, if any B. If a student (or in case of a student under legal age their parent or guardian) cancels their enrollment and demands their money back in writing, within three (3) business days of the signing of the enrollment agreement, all monies collected by the school shall be refunded. The postmark on written notification will determine the cancellation date, or the date said information is delivered to the school administrator/owner in person. Formal termination shall occur within 30days of determination by the institution that the student has withdrawn without notifying the institution. C. If a student cancels his/her enrollment after three (3) business days following the signing of this contract but prior to entering classes, they will entitled to a refund of all monies paid to the college, less a registration fee of $ plus an administrative fee of 5% or $100, whichever is less. Institutions refund policy The policy follows NM State and NACCAS Total tuition DeWolff Institute shall receive or retain: % of Scheduled Time to Total Time of program=% of total tuition owed to DeWolff Institute 01% to 4.9% = 20% 5% to 9.9% = 30% 10% to 14.9% = 40% 15% to 24.9% = 45% 25% to 49.9% = 70% 50% or over = 100% Enrollment time is defined as the time elapsed between the students start date and the last physical date of attendance at the school. Refunds will be calculated on scheduled hours. Any monies due the applicant or student shall be refunded within 45 days of enrollment time is defined as the time elapsed between the actual starting date and the date of the student s last day of physical attendance in the college or in the case of a leave of absence the documented date of return. In the case of disabling illness or injury, death in the student s immediate family or other documented mitigating circumstances, a reasonable and fair refund settlement will be made.

8 9 If permanently closed or no longer offering instruction after a student has enrolled, the institute will provide a prorate refund of tuition to the student within 30 days. If the course is canceled subsequent to student s enrollment, the institute will either provide a full refund of all monies paid or completion of the course at a mutually agreeable location. Books and equipment kit are not returnable for sanitary reasons. It is understood that sufficient time is allowed within the term of this contract. This contract will be changed only for the following documented reasons: 1. Student s hospitalization 2. Time loss due to death in student s immediate family 3. Court appearance 4. Approved leave of absence 5. Professional judgment. The listed extensions will be made at no charge to the student. Other reasons are accepted and could result in charges. GRADUATION REQUIRMENTS DeWolff Institute will grant a certificate of completion, an Official Transcript of Hours for the applicable course when the student has successfully completed all phases of study, required tests, practical assignments; passes a final comprehensive written and practical examination with a score of 80% or better; completed the program of study according to state requirements; complete all exit paperwork; and made satisfactory arrangements for payments of all debts owed to the Institute. LEAVE OF ABSENCE A leave of absence will be requested in writing from the student to the administration office. Only in an emergency can a student be given a leave by telephone. In these cases, a written statement must be given to the Institute office upon the students return. A leave of absence will be granted for the following reasons 1) medical 2) immediate family emergency 3) death of immediate family member. The maximum time frame for a leave of absence is 180 days. The DeWolff Institute permits more than one leave of absence in a 12-month period provided that the total numbers of days do not exceed 180 days. The leave of absent will extend students contract and maximum time frame by the same number of days taken in the leave of absence. During any leave of absence all financial aid is suspended. The student will return to school in the same progress status held prior to LOA. REENTRY STUDENTS/INTERRUPTIONS A determination of Satisfactory Academic Progress will be made and documented at the time of withdrawal or beginning of a Leave of Absence. That determination of status will apply to students at the time they return to school. The student may appeal a negative Satisfactory Academic Progress determination of status will apply to students at the time they return to school. Elapsed time during a Leave of Absence does not affect Satisfactory Academic Progress and will extend the contact period by the same number of days in the Leave there will be no additional charges to a student for the time off when they are on an approved leave of absence. Students reentering after exiting the school will not be evaluated as new students and consideration will be given to the student s progress status at the time of previous withdrawal. Students will enter in the same progress status as when they left. LEAVES OF ABSENCE FOR MILITARY Leaves of absence, including military leaves, shall be reasonable in duration, not to exceed that period stated in the approved catalog, and shall be for specific and acceptable purposes. DeWolff Institute attendance records will clearly show leave of absences granted. A written request for leaves of absences properly dated and signed by both the student and the Institute director must be placed in the individual student file prior to the effective date of such leaves of absence. The school certifying official is responsible for reporting leaves of absence to the Department of Veterans Affairs

9 Regional Office using the Notice of Change in Student Status (VA Form b). If the student fails to return from leave, he or she will be automatically terminated, and a refund will be totally consummated within forty (40) days. 10 GRADING SYSTEM The student will be evaluated by written and oral testing at the end of each unit of study covered by the instructor. The theory grade will be a numerical grade between 75%-100% with 75% being the minimum allowable grade for satisfactory progress. The instructor will inform the student at the beginning of each unit of study when they will hold their test. The student s practical grade will be determined by a weekly project grade in the clinic, or if a freshman, in the classroom. The student will be given a project on Friday which will be given a numerical grade by the classroom instructor, with 75% being the minimum passing grade. The instructor will inform the student before testing of what they will be grading them on in the project. The hair design, perm wraps, color project, will be performed on a mannequin, with a grade in hair sculpture, nails, etc., being performed on the patron, (or if in the classroom on the mannequin). At the end of the month the weekly grades will be averaged to determine the students practical grade for the month. The theory grades for the month will also be averaged and the student will receive a grade in both practical and theory, and also an average of the two. The students will receive a monthly report to inform them of their progress. Practical and Theory: 100=A+ Outstanding 99 to 91=A Above Average 90 to 81=B Average 80 to 75=C Slightly Below Average 74 to 0 =F Unsatisfactory and Failing POLICY REGARDING MISSED THEORY WORK Any student who is behind in theory subjects, due to being absent, may make up theory work under the supervision of a licensed instructor. The student has one week, after return from being absent, to make up theory test missed. It is the responsibility of the student to contact their theory instructor and make arrangements to take the test they missed. All missed theory test are to be made-up with in the week of the test date. Failure to comply will be considered unsatisfactory academic progress and will result in a financial aid warning. INTERNAL GRIEVANCE POLICY A student with a complaint or question concerning their educational program will first discuss the situation with their instructor. If they need further clarification or explanation, the student may pick up a complaint form in the office and make an appointment with school owners to discuss the written complaint. If the student is not satisfied with the decision of the owners, they may file a complaint with the New Mexico State Board of Barber & Cosmetologist PO Box Santa Fe, NM (505) or website If there are still concerns, the student may file a complaint with the National Accrediting Commission of Career Arts and Sciences (NACCAS)4401 Ford Ave. Suite 1300 Alexandria, VA (703) or website and New Mexico Higher Education Department (NMHED) 2048 Galisteo St. Santa Fe, NM (505) or website The student has the right to refer the complaint to the Attorney General of New Mexico. The student will not be singled out by staff, students or administration for filing any complaint. State Board Grievance is as follows: Each school licensed by the Board must have an internal grievance policy for students to seek solutions to any student complaint or concern that is not frivolous or without merit. Evidence of final resolution of each complaint will be retained in a schools file in order to determine the frequency, nature, and patterns of complaints for the institution. Verification that the procedure is in place and being followed will occur during routine inspections of schools. Formal complaints can be filed with the Board pursuant to NMAC of these rules. Schools may not retaliate against students who file complaints with the Board.

10 11 EXCUSED AND UNEXCUSED ABSENCES DeWolff Institute follows the New Mexico State Board guidelines for excused and unexcused absences. A student must complete the course scheduled hours for completion and met the minimum attendance standard (or 90%) of the established course of student and all other academic and evaluation factors established by the school. Therefore, DeWolff Institute grants students 10% excused hours per month of possible hours available. The following reasons will be accepted for excused hours: 1) medical 2) immediate family emergency 3) death of immediate member family. These reasons must include documentation that is kept in student file. All other reason will not be accepted and student will not be given credit which will be considered unexcused absences. TARDINESS POLICY I FULL TIME- COSMETOLOGY FULL TIME- INSTUCTOR 450 clock hours and 13 weeks 500 clock hours and weeks 900 clock hours and 26 weeks 1250 clock hours and 39 weeks HALF TIME-COSMETOLOGY HALF TIME-INSTRUCTOR 450 clock hours and 26 weeks 500 clock hours and 29 weeks 900 clock hours and 52 weeks 1250 clock hours and 78 weeks SATISFACTORY ACADEMIC PROGRESS POLICY DeWolff Institute s Satisfactory Academic Progress (SAP) standards measure each student quantitative (clock hour completion) and qualitative (cumulative grade point average) towards the completion of the students program of study. The SAP standards are used primarily to determine a student s eligibility to receive federal financial aid under Title IV Higher Education Act; however, the SAP standards are applied to all students and represent a minimum standard of academic achievement by DeWolff Institute for continued enrollment. All students are required to adhere to the same SAP standards whether or not they are receiving financial aid. Monthly evaluations will help determine if the student is on track to meet the minimum requirements for SAP by the next payment period. The frequency of evaluations ensured that students have ample opportunity to meet both the attendance and academic progress requirement and if not then allow those students to receive guidance in time to correct their attendance or academic progress. Students meeting the minimum requirements for academics and attendance at the end of each evaluation period (end of payment period) will be considered making SAP until the next scheduled evaluation. Students will receive a hard-copy of their SAP determination at the time of each of the evaluations. Students are evaluated for both academics and attendance at the time they have actual completed their Title IV evaluation periods (450 hrs. 900hrs, 1250hrs). Students attending the Instructor program are evaluated at the time they have actual completed their Title IV evaluation periods (500 hrs.). Financial Aid and Title IV funds will be disbursed for Cosmetology - 0, 450, 900, &1250 hours, Cosmetology Instructor - 0, 500. SAP is figured by number of hours successfully completed divided by the number of hours scheduled. Students must attend a minimum of 67% of the cumulative scheduled hours and a minimum of 75% in academic grades to maintain Satisfactory Academic Progress and complete the Cosmetology and/or Cosmetology Instructor program within the maximum time allowed. 11 12 MAXIMUM TIME FRAME The maximum time frame is equal to 1.5 times the published length of the Cosmetology Program (2400 scheduled hours) Instructor program (1500 scheduled hours). Time off for authorized leaves of absence will be added to the maximum time frame. During the maximum time frame, on the average a student would have to complete the following hours during the following months to remain in Satisfactory Academic Progress. The maximum time (which does not exceed 1.50 times of the program length) allowed for students to complete each program length at Satisfactory Academic Progress is stated below: PROGRAM MAXIMUM TIME ALLOWED Weeks Scheduled Hours Cosmetology 69 weeks 2400 hrs. Instructor 43 weeks 1500 hrs. If a student does not complete their program in the maximum time frame, they will lose all Title IV funding. ACADEMIC PROGRESS Students are expected to attend classes as per their enrollment agreement. Students are responsible to clock in and put appropriately to document their hours: the only documentation accepted for student hours is the time clock system. The student is responsible for class material and/or tests missed while absent. Students who miss 10 consecutive school days (14 calendar days) without communication with the School Director/designee may be terminated on the 11 th consecutive school day as an unofficial withdrawal. Students who have excessive absences may be terminated: re-enrollment is at the discretion of school Students are assigned theory study and practical assignments. Theory is evaluated by written exams given after each unit of study. Practical assignments are evaluated as completed and counted toward program completion ONLY when rated as satisfactory or better. Practical skills are evaluated according to textbook procedures and set forth in practical skills evaluation criteria adopted by the school. Students are evaluated quantitative (hours) and qualitative (GPA) at each evaluation period ( ). Students must maintain a cumulative theory grade average of 75% and pass a FINAL written with an average of 80%, and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered to the following scale: Practical and Theory: 100=A+ Outstanding 99 to 91=A Above Average 90 to 81=B Average 80 to 75=C Slightly Below Average 74 to 0 =F Unsatisfactory and Failing DETERMINATION OF PROGRESS Students meeting requirements at the end of each evaluation period will be considered making SAP. In order for a student to be considered making SAP the student must meet the required clock hours, the required number of weeks, attendance and academic minimum requirements at the end of each evaluation period, to be considered eligible for the next payment period. All students must complete their educational program in no longer than 150% of the published length of the program. A leave of absence will extend the student s contract period, and maximum time frame, by the same number of days in the leave of absence. A leave of absent does not affect the cost of the program. *VA students are evaluated on a monthly base. VA students must maintain academic progress at all times. VA students have 46 weeks to complete the cosmetology program.

12 13 FINANCIAL AID WARNING Students who fail to meet minimum requirements for attendance and academic progress (67% cumulative attendances and a cumulative theory grad average of 75%, C or 2.0GPA) at the end of evaluation period will be placed on a Title IV Financial Aid Warning the first time. Any student on Financial Aid Warning may continue to receive assistance under Title IV, HEA programs for one payment period only. At the end of the payment period if the student has met the minimum grade and attendance requirement the student is considered to be meeting SAP. If a student is making SAP at the end of the Financial Aid Warning, they shall be returned to normal SAP status with no loss of Title IV eligibility and no further action will be taken. If the student is not meeting SAP at the end of the Financial Aid Warning Period: there will be a loss of Title IV eligibility: with the right to appeal. The student will be placed on a Title IV Academic development Status, with a loss of Title IV,HEA funding and will be required to meet specific criteria of an improvement plan to assist them in regaining SAP and Title IV, HEA eligibility. During this period the students will not be eligible to receive Title IV, HES funds but he/she may continue on a cash pay basis with an approved payment plan. APPEAL POLICY A student who losses their financial aid eligibility due to not making SAP at the end of a financial aid warning has the right to file an appeal regarding their SAP Evaluations. A student, who wishes to appeal Title IV, HEA Academic development Status and loss of Title IV eligibility, must submit a written request to the Dean of the Institute within 5 school days of being notified that they are in a non-satisfactory progress status. The student must describe any unusual circumstance(s) that the student believes as to why they deserve special consideration. The basis on which a student may file an appeal: death of a relative, an injury, or illness of the student or other special circumstance(s). The student must provide information as to why they did not make SAP and what has changed that will allow them to make SAP by the next evaluation point. The written appeal will be reviewed within 5 school days by the Dean of the Institute and the student will be notified of the results in writing. The results of the appeal review are final and not appealable. If the student prevails on appeal they will be placed on Title IV, HEA Financial Aid Probation, which is a status assigned by the Federal regulation to a student who fails to make SAP, who has appealed and has had their Title IV eligibility for aid reinstated for one payment period only. FINANCIAL AID PROBATION Any student that prevails upon the appeal process shall be placed on Financial Aid Probation, and will be eligible to receive Title IV, HEA during this period. A student placed on Financial Aid Probation may receive Title IV program funds for one payment period only, While on Title IV Financial Aid Probation, the student must meet the institutions SAP standards and complete the requirement of the Academic Improvement Plan, which was developed by the institutions to assist the student in regaining their Title IV, HEA eligibility. Those who are not making SAP at the end of the Title IV Financial Aid Probation period will be ineligible to receive Title IV funds for the following payment periods. A Student must meet SAP prior to having eligibility reinstated. Requirements for the Academic Improvement Plan: 1)Maintain an attendance percentage that will ensure the student will meet SAP by the next payment period 2)Maintain a 75% grade point average. (The higher grade point is required to raise the standard to help the student make up for areas that may have been previous lacking). 3) Attend theory class regularly (student must attend an average of 4 out of the e hours required for theory class each week). 4) Complete all required test and projects assigned (test and projects must be completed and turned in when requested). 5) The academic improvement plan will be monitored by the School Director (or designate). 6) The school will notify the student each month on their academic improvement status during the monthly progress report/advising session. Status of the students report will be written on the progress report form for the student to sign. 7) Students choosing to remain in school while requesting an appeal will be responsible for charges accrued whether or not the appeal is granted.

13 REESTABLISHMENT OF TITLE IV HEA FUNDING If a student that is NOT making Satisfactory Academic Progress, he/she may reinstate Satisfactory Academic Progress by: 1) Making up missed tests and assignments and increasing cumulative grade point average to 75% 2) Increasing cumulative attendance to 67% REINSTATEMENT OF TIVE IV HEA FINDANCIAL AID Title IV, HEA aid will be reinstated to students who have prevailed upon appeal regarding the status of SAP or who have reestablished SAP. Students on suspension of will be monitored daily to determine when they reestablish SAP. REINSTATEMENT OF STATUS 14 A student that is NOT making Satisfactory Academic Progress may reinstate Satisfactory Academic Progress by: 1) Making up missed tests and assignments and increasing cumulative grade point average to 75% 2) Increasing cumulative attendance to 67% If a student clock s in late, the student will only receive the hour s actual clock-in. As long as, the student maintains 67% of attendance per month and 75% of theory work per month, they will have no adverse actions taken against them, although continued tardiness will eventually affect students progress. GENERAL FACILITIES & EQUIPMENT DeWolff Institute is located in the northeast quadrant of Albuquerque. The Princess Jeanne Shopping Center which is well maintained, well lighted and has ample parking available for patron and students. DeWolff Institute facilities are well lighted, air-conditioned and heated for comfortable year round operation. It is well equipped for student and instructor usage in the classrooms and clinic area. The DeWolff Institute administration offices are centrally located in the facility. ACADEMIC YEAR DEFINITION DeWolff Institute academic year is defined as 900 clock hours and 26 weeks (full time) or 900 clock hours and 52 weeks (half time) for title IV purposes. For Title IV payments the student must meet both clock hours and weeks of instruction as well as complying with all standards for Satisfactory Academic Progress before they receive Title IV payments. Students are evaluated for both academics and attendance at the time they actual complete. They will be checked at the following time: TRANSFER POLICY All students that transfer into DeWolff Institute with hours from another school will be evaluated at the same schedule as any other new student. Until, DeWolff Institute sends an official request of hours with an official transcript from the college the student is transferring from for approval to New Mexico State Board of Barber and Cosmetologist. The Board will then approve or decline any hours from the request; at that point they will return the request with approved hours, the college will then adjust the student s contract and clock hours. The evaluation periods may have to be adjusted for transfer students, depending on the amount of hours transferred into the College. Evaluation period 1 will require a SAP only if student is bringing in hours from another school and SAP can be verified from that school. All clock hours at the Institute and transfer hours or hours completed at another institution must be counted toward the 150% eligibility whether a student received Title IV, HEA federal student aid or not will affect the students Title IV, HEA eligibility, and will be counted also in order to graduate within the maximum time frame of 150%. EXTERNSHIP PROGRAM The institute encourages qualified students to obtain salon sponsors for participation in the state approved student externship program. It allows students who have completed 75 %( 1200) of the required hour to participate in onthe-job training up to eight hours per week in an area salon (not to exceed 10% of course = 160 clock hours for cosmetology). The student must be maintaining Satisfactory Academic Progress According to the college s policy, be current on all written examinations. The student must apply for and post a duplicate student permit in

14 the establishment while training in the establishment. The Institute must accept the training certified by the establishment and include it on the official transcript of training for state board. 15 MINI-BOARD PREP CLASS AND TEST In each program of study at DeWolff Institute, a Mini-Board Prep Class is given when a student is nearing graduation. Students must pass the Mini-Board test as a graduation requirement. There are two parts to this test, a practical and a written. A Mini-Board prep class and test is designed and administered to prepare students graduating from DeWolff Institute to take their State Board Examination with confidence. Before a student is able to attend this class all theory tests and workbook must be completed. Tuition, and all other charges; if any, must be paid in full. PREVIOUS CREDIT EVALUATION DeWolff Institute maintains a written record of the previous education and training of each student. The record indicates that appropriate credit has been given. The student and any other appropriate agency will be notified of this evaluation. The institute allows students in good standing to re-enter without loss of credit. POLICY REGARDING STUDENTS WITH DISABILITIES APPLICANTS Before admittance, all students with disabilities applicants must be interviewed to determine their ability to benefit from the training offered. The school does not discriminate in its admissions practices or other policies against students with disabilities persons. The licensing requirements for courses offered at the school may restrict some applicants. The institute director may answer questions regarding licensing requirements and the physical demands of the industry. All entrances and exits at this facility allow access to all persons in wheel chairs and/or crutches. All applicants that believe they need to receive reasonable accommodations must submit their request in writing and it will be evaluated. PLACEMENT SERVICES De Wolff Institute has a placement binder in the administrative offices where students are able to look at the job opportunities available. A staff member is available to assist students, and keep records of where students can find employment. Students are instructed on how to make resumes in the Salon Management portion of their theory classes and are given samples of various types of resumes and letters of recommendation. If students so desire, they can request assistance in making appointments for interviews. The institute has a variety of salon owners and representatives come to the school and present to the students what their salon or spa can offer them. Job placement is offered but not guaranteed. 15 16 GOALS AND OBJECTIVES The goal of the Cosmetology program at DeWolff Institute is to prepare the student to successfully pass the practical and written examinations required by the State of New Mexico to be licensed. Just as important, the goal of DeWolff Institute is to prepare the student to successfully find employment in the field and to be able to compete in the challenging career of Cosmetology. The objective of this program is as follows: At the end of the program, and before a certificate is issued, the student will be able to successfully pass the institute s final written examinations with at least a 75% score and to successfully demonstrate entry level job skills by passing the college s final practical examinations, which involve testing in all units of the course. At that time the student will receive a certificate with an official transcript of completed clock hours. COSMETOLOGY PROGRAM FORMAT The program includes lectures, demonstrations, and hands on instruction. All three types of instruction input (visual, oral, and physical) are used throughout the program. The students are continually encouraged to prepare the physical skills they are instructed in, and to apply the theoretical knowledge they learn to solve problems presented by the clients in the Institute Clinic. Examples of employment opportunities are: cosmetician, platform artist, salon manager, salon owner, manufacturer s representative, or traveling stylist. The cost per hour for over contract is $ TUITION WAIVER PROGRAM DeWolff Institute has created a waiver program to help provide assistance to students who demonstrate the qualities needed to be successful in our industry. The waiver program is only available to students who maintain a 90% attendance and 90% grade average daily. The student will have $2, posted to their account at the fourth payment period which is 1250 clock hours. The student must maintain the 90% throughout the 1600 hour program. No leaves of absent will be granted in order students to qualify for this waiver. (December 17, 2014). On July1, 2014, DeWolff Institute will make available $20, in tuition waiver. Each year there may be a different amount of a waiver. The amount a student can receive will be $2, The waiver is on a first come first served basis as long as the student maintains the required SAP (90% attendance and grades). All students who enroll in DeWolff Institute will automatically be enrolled in the waiver program. INSTRUCTOR PREREQUISITE Applicants for the Instructor s Program must submit proof they have completed at least an approved four year high school program of study or its equivalent as determined by the State Board, and is a licensed cosmetologist in New Mexico. Written proof of such background must be submitted prior to enrollment in the Cosmetology Instructor Course. PROGRAM DESCRIPTION To be an Instructor means to share, dispense and divulge knowledge and/or skills. The Instructor Program is devoted to acquiring the teaching skills needed so that an experienced Cosmetologist can more readily share, dispense, and divulge their cosmetology knowledge and skills in a logical and organized manner to students just entering the career field. The total program is 1,000 clock hours in length. Total $ 7, The cost per hour for over contract is $7.60 COUNSELING POLICY DeWolff Institute shall counsel each student at the end of each month in regard to their progress in theory and practical subjects. Counseling services are available to every student, at any time, in regard to a student s education. Financial aid counseling is available for all students who are having difficulties meeting their financial responsibilities to the institute. Clinical counseling is available through several offices that are listed in the Cleary Act. ABOUT THE WRITTEN AND PRACTICAL EXAMINATION FOR THE NEW MEXICO STATE BOARD OF BARBERS AND COSMETOLOGISTS A grade of 75% must be made in each subject in the written examination. The practical portion of the examination is determined by Pass/Fail grade. Professional Credential Services examination dates will be published yearly and exam dates will be posted on the student bulletin board in the college. Registration deadlines for the examinations are announced and published on the same annual announcement by Professional Credential Services (PCS) All students must register with PCS for licensure examination upon completion of their course requirements. COPYRIGHT INFRINGEMENT DeWolff Institute prohibits copyright infringement. DeWolff Institute will take disciplinary action against any student who distributes unauthorized copyrighted material including peer to peer, file sharing, and the prohibited use of college s information technology system for those activities. Copyright infringement is the act of exercising, without permission or legal authority. Penalties infringement includes civil and criminal penalties. Privacy of Records-family Educational Rights and Privacy Act FERPA Family Policy Compliance Office (FPCO) Home

18 19 The family Educational Rights and Privacy Act (FERPA) (20 U.S.C. 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education. FERPA gives parents certain rights with respect to their children s education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are to the students and parents or guardians of dependent minors. Student and parent or guardians of a dependent minor have the right to inspect and review the student s education records maintained by the school. Schools are not required to provide copies of records unless, for reasons such as great distance, it is impossible for parents or eligible students to review the records. Schools may charge a fee for copies. Student and parent or guardians of a dependent minor have the right to request that a school correct records which they believe to be inaccurate or misleading. If the school decides not to amend the record, the parent or guardians of dependent minors then has the right to a formal hearing. After the hearing, if the school still decides not to amend the record, the parent or eligible student has the right to place a statement with the record setting forth his or her view about the contested information. Disclosures to parents or guardians of dependent minors; 1. A school may disclose information from a student s records to parents in the case of a health or safety emergency that involves the student. 2. A school may let parents of students under the age of 21 know when the student has violated any law or policy concerning the use or possession of alcohol or a controlled substance. 3. A school official may share with parents information that is based on the schools personal knowledge or observation and that is not based on information contained in an educational record. Generally, schools must have written permission each time from the parent or eligible student in order to release any information from a student s education record. However, FERPA allows schools to disclose those records, without consent, to the following parties or under the following conditions (34CFR 99.31): School officials with legitimate educational interest; Other schools to which a student is transferring; Specified officials for audit or evaluation purposes; Appropriate parties in connection with financial aid to a student; Organizations conducting certain studies for or on behalf of the school; Accrediting organizations; To comply with a judicial order of lawfully issued subpoena; Appropriate officials in cases of health and safety emergencies; and State and local authorities, within a juvenile justice system, pursuant to specific State Law. Schools may disclose, without consent, directory information such as a student s name, address, telephone number, date and place of birth, honors and awards, and dates of attendance. However, schools must tell student and parents or guardians of dependent minors about directory information and allow reasonable amount of time to request that the school not disclose directory information about them. Schools must notify student and parents or guardians of dependent minors annually of their rights under FERPA. The actual means of notification (special letter, student handbook, or newspaper article) is left to the discretion of each school. For additional information, you may call USA-LEARN ( )(voice). Individuals who use TDD may call Recordkeeping Source: Federal Student Aid Handbook, Volume II, Privacy of Student Information (FERPA Rules) The Family Educational Rights and Privacy Act (FERPA) sets limits on the disclosure of personally identifiable information form school records, and defines the rights of the student to review the records and request a change to the records.

19 20 With exceptions such as those noted in this section, FERPA generally gives postsecondary students the right: to review their education records, to seek to amend inaccurate information in their records, and to provide consent for the disclosure of their records. These rules apply to all education records the school keeps, including admissions records (only if the student was admitted) and academic records as well as any financial aid records pertaining to the student. Therefore, the financial aid office is not usually the office that develops the school s FERPA policy or the notification to students and parents, although it may have some input. Student s and parents rights to review educational records A school must provide a student with an opportunity to review his or her education records within 45 days of the receipt of a request. A school is required to provide the student with copies of education records, or make other arrangements to provide the student access to the records, if a failure to do so would effectively prevent the student from obtaining access to the records. While the school may not charge a fee for retrieving the records, it may charge a reasonable fee for providing copies of the records, provided that the fee would not prevent access to student records. While the rights under FERPA have transferred from a student s parents to the student when the student attends a postsecondary institution, FERPA does permit a school to disclose a student s education records to his or her parents if the student is a dependent student under IRS laws. Note that the IRS definition of a dependent is quite different from that of a dependent student for FSA purposes. For IRS purposes, students are dependent if they are listed as dependents on their parent s income tax returns. (If the student is a dependent as defined by the IRS, disclosure may be made to either parent, regardless of which parent claims the student as a dependent.) Prior written consent to disclose the student s records Except, under one of the special conditions described in this section, a student must provide written consent before an education agency or school may disclose personally identifiable information from the students education records. The written consent must state the purpose of the disclosure, specify the records that may be disclosed, identify the party or class of parties to whom the disclosure may be made, and, be signed and dated. If consent is given electronically, the consent form must: identify and authenticate a particular person as the source of the electronic consent, and indicate that person s approval of the information contained in the electronic consent. The FERPA regulations include a list of exceptions were the school may disclose personally identifiable information from the student s file without prior written consent. Several of these allowable disclosures are of particular interest to the financial aid office, since they are likely to involve the release of financial aid records. Disclosures to school officials Some of these disclosures may be made to officials at your school or another school who have a legitimate interest in the student s records. Typically these might be admissions records, grades, or financial aid records. Disclosure may be made to: other school officials, including teachers, within the school whom the school has determined to have legitimate educational interests

20 21 to officials of another postsecondary school or school system, where the student receives services or seeks to enroll. If your school routinely discloses information to other schools where the student seeks to enroll, it should include this information in its annual privacy notification to students. If this information is not in the annual notice, the school must make a reasonable attempt to notify the student at the student s last known address. Disclosures to government agencies Disclosures may be made to authorized representatives of the U.S. Department of Education for audit, evaluation, an enforcement purposes. Authorized representatives includes employees of the Department such as employees of the Office of Federal Student Aid, the Office of Postsecondary Education, the Office for Civil Rights, and the National Center for Education Statistics as well as firms that are under contract to the Department to perform certain administrative functions or studies. In addition Disclosure may be made if it is in connection with financial aid that the student has received or applied for such a disclosure may only be made if the student information is needed to determine the amount of the aid, the conditions for the aid, the student s eligibility for the aid, or to enforce the terms or conditions of the aid. A school may release personally identifiable information on an F, J, or M nonimmigrant student to U.S. Immigration and Customs Enforcement (formally the immigration and Naturalization Service.) in compliance with the student exchange visitor information System (SEVIS) program without violating FERPA. Disclosures in response to subpoenas or court orders FERPA permits schools to disclose education records, without the student s consent, in order to comply with a lawfully issued subpoena or court order. In most cases, the school must make a reasonable effort to notify the student who is the subject of the subpoena or court order before complying, so that the student may seek protective action. However, the school does not have to notify the student if the court or issuing agency has prohibited such disclosure. A school may also disclose information from education records, without the consent or knowledge of the student, to representatives of the U.S. Department of Justice in response to an ex parte order issued in connection with the investigation of crimes of terrorism. Documenting the disclosure of information Except as noted below, a school must keep a record of each request for access and each disclosure of personally identifiable student information. The record must identify the parties who requested the information and their legitimate interest in the information. This record must be maintained in the student s file as long as the educational records themselves are kept. FERPA Responsibilities and Student Rights A school is required to: Annually notify students of their rights under FERPA; Include in that notification the procedure for exercising their rights to inspect and review education records; and Maintain a record in a student s file listing to who personally identifiable information was disclosed and the legitimate interests the parties had in obtaining the information (does not apply to school officials with a legitimate educational interest or to directory information).
