2 a message from our president The HCA North Florida Division remains a significant force in the North Florida and South Georgia communities we serve, with an economic impact exceeding $1,200,000,000 annually. Our 17,000 employees, physicians and volunteers provide vital healthcare services through our 14 hospitals, five surgery centers, five cancer centers, and five imaging centers. Our efforts to improve operations in our emergency departments continued this year, resulting in record low wait times for the 550,000 patients who received emergency care in our facilities. While our ER volumes are growing, so is the number of patients without adequate health insurance or the financial resources to pay for this care. Last year, our hospitals treated more than 141,000 uninsured patients. Additionally, we treated more than 500,000 patients covered through the Medicaid program. The cost of charity and uncompensated care provided by our hospitals totaled more than $135,000,000 in Despite these challenges, we continue to expand to meet the growing demand for our services. In addition to announcing major expansion projects at Gulf Coast Medical Center and Capital Regional Medical Center, we opened pediatric emergency rooms at both Doctors Hospital of Augusta and Capital Regional. West Florida Hospital s Cancer Center opened in 2011, enhancing cancer care treatment options in Pensacola with its affiliation with the Sarah Cannon Research Institute. And lastly, North Florida Regional Medical Center completed Phase III of its multi-phase campus expansion with the opening of its new cardiovascular intensive care unit and interconnected coronary care unit. In late 2011, we opened the One Step Patient Transfer Center, which serves both the North Florida and South Atlantic Divisions. We anticipate the Transfer Center will prove beneficial for continued future growth as we serve the rural communities surrounding our hospitals and facilitate expedient transfers for patients needing more advanced or specialized care. Throughout the year, we continued our emphasis on providing the highest quality patient care possible. Each of our facilities earned accreditations and recognition of the hard work they do each day to ensure our patients receive the excellent care they deserve. Many of our hospitals offer Accredited Chest Pain Centers, Joint Commission Certified Primary Stroke Centers, and Cancer Centers accredited by the American College of Surgeons Committee on Cancer. These certifications represent the gold standard of care today, awarded by independent accrediting organizations. Community Support The affiliates of HCA North Florida and their employees are committed to improving the health and well-being of those in the communities we serve. They do this not only through the quality of care they provide, but also through the financial and volunteer support they provide to countless community organizations, schools, colleges and non-profit and charitable organizations in their communities. Indigent Patient Education HCA North Florida Hospitals are committed to helping uninsured patients qualify for local and state health benefit programs. Many of our hospitals offer on site benefit education and Medicaid enrollment assistance. Step Up for Students Since 2005, HCA has redirected corporate income tax to the Step Up for Students program. Through this gift, low income students receive a K-12 scholarship enabling them to receive a quality education tailored for their specific needs. It is well documented that a one size fits all education is not the best approach for every student. Families with the means may choose the perfect school for their child but that s simply not an option for many people. Through this program children that qualify for the National School Lunch Program can receive scholarships. Qualifying parents can use these grants to send their child to the public or private school that best meets their child s needs. 7 Accredited Chest Pain Center Accredited Chest Pain Center w/pci (Heart Attack) Center Primary Stroke Center Awards & Recognition Numerous awards and recognitions are bestowed on our facilities and management teams, acknowledging not only the outstanding medical care and services we provide, but also the leadership role we play in our communities. All of our facilities are Joint Commission (JC) Accredited, with nine JC Accredited Primary Stroke Centers, and JC Accreditation for Spinal Care at Gulf Coast Medical Center and Doctors Hospital of Augusta. This year, 10 of our facilities were recognized by The Joint Commission as one of the Top Performers on Key Quality Measures. This first-ever recognition was focused on accredited hospitals that attained and sustained excellence in accountability measure performance during calendar year As Top Performers on Key Quality Measures, our 10 facilities were among only 405 hospitals being recognized for 2010, which represents the top 14 percent of Joint Commission accredited hospitals (of those that report core measure performance data). Ten of our facilities offer Certified Chest Pain Centers, recognizing their consistent high quality care and expertise in dealing with patients suffering with chest pain. Six have been designated by Blue Cross Blue Shield as a Blue Distinction Center for Spine Surgery, and five for Hip and Knee Replacement Surgery. During 2011, Gulf Coast Medical Center became the first Florida hospital to become an affiliate of the University of Alabama at Birmingham (UAB) Cancer Care Network, and is the only hospital in Northwest Florida to earn National Accreditation as a Breast Center by the National Accreditation Program for Breast Centers (NAPBC). West Florida Hospital s Breast Center was awarded Breast Imaging Center of Excellence status from the American College of Radiology. Volunteering One of the most important contributions our facilities make is through the many caring hearts and helping hands of our HCA North Florida Division family members. Last year, over 208,000 hours of volunteering were performed by our staff, volunteers and physicians giving back to our communities and beyond, both in and outside of our facilities. North Florida Regional Medical Center received numerous accolades from HealthGrades, including Best Rated in Region for Overall Cardiac Services, and Gastrointestinal Medical Services in Further, HealthGrades recognized the facility as the only hospital in the region to receive a 5 Star rating for Chronic Heart Failure in (Region is Gainesville, FL, as defined by the Federal Government s Office of Management and Budget.) Coliseum Medical Centers and Coliseum Northside Hospital were named to the Georgia Hospital Association s Partnership for Health and Accountability Quality Chairman s Honor Roll, two of only 10 hospitals statewide. Fairview Park is one of 19 hospitals in Georgia to be placed in the Trustee category, based on clinical data provided by the Federal Centers for Medicaid & Medicare Services (CMS), which administers the nation s Medicare and Medicaid programs. The data was collected from July 2010 to June Twin Cities Hospital was selected as a recipient of the Hospital of Choice Award from The American Alliance of Healthcare Providers (AAHCP). For the third consecutive year, Twin Cities Hospital proudly shares this recognition with only 50 other hospitals in the nation. And lastly, the Joseph M. Still Burn Center at Doctors Hospital of Augusta earned Burn Verification, a rigorous review program administered by the American Burn Association and the American College of Surgeons that ensures the Burn Center has the resources required to provide optimal care to burn patients from the time of injury through rehabilitation. Consult-A-Nurse Free physician referral, health information and class registration are available 24/7 by calling HCA-DOCS. Registered Nurses are available for consultation through this multi-lingual community service, sponsored by the HCA hospitals of North Florida. Our Commitment to Excellence 5

8 quality & patient safety Despite our achievements, HCA North Florida recognizes the importance of patient safety and continues to make it paramount. Our initiatives ensure the safety and quality of care given to our patients each day. InterMedHx (RxHx) Providers are routinely tasked with obtaining a list of a patient s current home medication a valuable part in the diagnosis and treatment of their patients. RxHx is designed to function as step one in this process by querying participating source medication databases, including pharmacies, payers, and pharmacy benefit managers. RxHx extracts available medication data, such as drug names, dosage, route, and frequency, and prints a personalized medication report all within minutes of the initial registration. The attending practitioner can then easily review the report with the patient to verify the content and check for any OTC medications, physician samples, and health supplements taken. With today s growing nursing and pharmacist shortages, RxHx can enable a provider to reallocate valuable professional time to other tasks and functions which, in turn, can further reduce medical errors. Capsule Vitals Now Vitals Now speeds the access of clinical information to caregivers by capturing and recording patient vital signs at the point of care. This technology helps caregivers capture vital signs for multiple patients, validate these clinical measurements at the bedside and automatically send them to the hospital EMR using wireless technology. By improving connectivity between patient monitors, biomedical devices and hospital information systems, data can flow directly into the hospital electronic medical record (EMR), thereby reducing documentation errors and improving patient safety. Severe Sepsis Program Severe sepsis is the most common cause of death in the ICU nationally. Healthcare costs to care for patients with severe sepsis are nearly twice as high as those for other patients, and mortality rates are much higher as well. The Sepsis Program includes a four-tiered process and use of evidence-based bundles along with a designated sepsis coordinator. Team efforts are initially focused in the ED and ICU, with plans to go house wide. 6 HCA North Florida

9 AirStrip CARDIOLOGY AirStrip CARDIOLOGY allows clinicians anytime/ anywhere access to live and historical ECG data on their ipad and iphone. It delivers a 20-fold improvement in ECG visual display over traditional paper tracings and PDFs, enabling clinicians to make faster, more informed diagnosis and treatment decisions for their cardiac patients. The ECG is among the most vital tools that clinicians use to diagnose a heart attack, and the use of AirStrip CARDIOLOGY can shave precious minutes off the process of diagnosis and treatment decision-making. When treating cardiac patients, those extra minutes can help patients recover more quickly while preventing heart damage. With regard to its use in the Emergency Department setting, AirStrip CARDIOLOGY allows for unprecedented collaboration between EMS, ED, and cardiology teams especially when transporting potential AMI and STEMI patients. AirStrip CARDIOLOGY receives information wirelessly from EMS transport, which the physician can then access from anywhere and review on his/her mobile device via an AirStrip application. The ECGs are displayed with the highest possible resolution no more faxes, s or PDFs. The physician can then communicate immediately with EMS to provide the clinical assistance needed to properly care for the patient while en route. The ED and/or cath lab can also prepare for the arrival of the patient and facilitate the most appropriate and timely intervention. AirStrip OB AirStrip OB delivers vital patient waveform data including fetal heartbeat and maternal contraction patterns directly from the labor and delivery unit to a medical professional s mobile wireless device similar to how they appear on a hospital-based monitor. Physicians can closely monitor patients 24/7 when the demands of their day necessitate their periodic absence from the Labor and Delivery Unit. Built on the latest mobility platforms, the AirStrip service is FDA-cleared on virtually all mobile devices and HIPAA compliant with a single-user signon component. AirStrip OB incorporates robust security measures to ensure confidential patient data is fully protected. In today s healthcare environment, improving patient safety and reducing risk are of paramount concern. The Joint Commission has identified ineffective communication as one of the leading causes of medical errors leading to patient injury. By bringing the right data to the right doctor at the right time, AirStrip OB helps our hospitals deliver excellent patient care. Our Commitment to Excellence 7

10 specialized care Emergency Excellence The combination of our highly skilled emergency physicians and nurses, along with best practices and the latest technology, benefitted more than half a million North Florida and South Georgia residents who sought emergency care in our facilities last year. Protocol driven care that begins in the field through direct communications with local EMS teams, and meeting stringent standards to earn Chest Pain Center Accreditation and Primary Stroke Center Certification by the Joint Commission are all part of our commitment to improve quality in our busy emergency departments. Our strong focus on enhancing care and improving ER processes has also dramatically reduced wait times below national averages. The average ER wait times for each of our hospitals are posted on their websites as well as digital outdoor boards in many of the communities we serve, and are also available by text messaging ER to Smart phone users are able to access average wait times via itriage, an app that helps locate providers for emergency care, while also serving many other medical needs. Pediatric Emergency Rooms were opened at Doctors Hospital of Augusta and Capital Regional Medical Center, joining Fort Walton Beach Medical Center and Gulf Coast Medical Center in offering specialized care for our youngest patients. In February, Capital Regional Medical Center launched one of two pilots throughout the country of itriage s pre-notification functionality. Branded as inotify, this function allows smart phone users to notify the ER that they are on their way, along with symptoms and other information to help the ER team prepare for their arrival. The pilot was successful and inotify will be launched in several other North Florida Division markets during HCA North Florida The newly-renovated ER at Doctors Hospital of Augusta.

11 Imaging Services The HCA North Florida network provides the most comprehensive array of outpatient imaging services in North Florida and South Georgia. Our facilities are equipped with a broad range of diagnostic technology and most offer extended hours. Mobile units bring needed imaging services out into the communities we serve. We are committed to offering imaging services needed across the full continuum of a patient s care. Fairview Park Hospital made upgrades to its imaging services, including the addition of coils that allow for advanced orthopedic and gastroenterology procedures. Heart & Vascular Network Our cardiovascular network ensures that lifesaving technology and the latest minimally invasive techniques are available in our communities. Several of our facilities offer accredited Chest Pain Centers: Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Fairview Park Hospital, Fort Walton Beach Medical Center, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Health System and West Florida Hospital. All of our facilities are beating the national STEMI benchmark of 90 minutes or less, dramatically improving the lives of our patients experiencing a heart attack. Only 40 percent of hospitals in the country have been able to consistently accomplish this, and similar to our average ER wait times, we post our average STEMI times on our hospital websites. The latest surgical techniques for minimally invasive mitral valve and atrial fibrillation repair provide a safer alternative for patients suffering from these conditions, improving their quality of life and reducing their risk for other serious complications. Our Cardiac Catheterization and EP Labs offer stateof-the-art technology and treatments for invasive and non-invasive cardiac procedures. Orthopedic and Spine Surgery Patients needing advanced treatments for orthopedic and spine issues can find high quality care at our North Florida Division hospitals. Advanced surgical procedures including Minimally Invasive Hip Replacement, Anterior Approach Hip Replacement, Custom Fit Knee Replacement, Gender Specific Knee Replacement, Knee and Hip Resurfacing, Partial Hip Resurfacing, and Partial Knee Replacement are available to relieve pain and help patients return to normal activities. Our surgeons are committed to performing the latest innovations in their field, and patients benefit from the experience of a panel of orthopedic experts, individualized treatment plans and a high caliber of care. Robotic & Minimally Invasive Surgical Services Our network offers some of the most efficient and well equipped operating rooms in the area. Advanced systems from scheduling to pre-op and discharge planning ensure that patients receive the care they need. One of the most exciting technologies today is the da Vinci Robotic Surgical System. Through the use of robotics, surgeons are able to perform complex and delicate procedures through tiny incisions, with unmatched precision. For patients, this means less blood loss and scarring, and a quicker recovery and return to normal daily activities. Seven of our facilities offer this option for appropriate surgical procedures. The Orthopedic and Spine Centers of North Florida and South Georgia provide a panel of experts who collaborate on treatment plans for patients. The teams at each Center include pain management professionals, diagnostic-imaging specialists, orthopedic surgeons, physical medicine and rehabilitation specialists, neurosurgeons, and physical therapists. Coliseum Medical Centers and North Florida Regional Medical Center offer the O-arm Multidimensional Surgical Imaging System technology. Neurosurgeons are utilizing this revolutionary technology for spine surgery, including lumbar, cervical and thoracic procedures. The O-arm System creates detailed two- and three dimensional images, which helps streamline the process during surgery. This can mean less time in the operating room, and make all the difference in the patient s surgical experience. 13 Women s Services Obstetrics services are offered at eight of our facilities, with a range of amenities that include private family waiting areas and beautiful home-like surroundings, along with state of the art equipment to help make the birthing process go as smoothly as possible. Our commitment to providing the best in women s services, from digital mammography, bone density ultrasound, to stereotactic breast biopsy, is unsurpassed. Capital Regional Medical Center and Coliseum Medical Centers offer Comprehensive Breast Centers, providing women access to advanced breast imaging technology in a warm, spa-like environment. Patient navigators at several of our facilities work with women who have potentially concerning findings to help them understand the process from detection, to diagnosis, to treatment. Our facilities ensure that patients receive the most advanced diagnostic technology and the highest quality of healthcare available in North Florida and South Georgia. NeuroSciences and Stroke Care Nine of our hospitals have earned Joint Commission Primary Stroke Center designation, the gold standard for ensuring the highest level of care is immediately available. Additionally, Fort Walton Beach Medical Center has been named the #1 Spine Surgery Program in the state of Florida by HealthGrades. HCA hospitals also offer surgical and non-surgical treatment options for various neurological disorders. The focus has expanded to include patients living with Parkinson s disease and other movement disorders that could benefit from Deep Brain Stimulation surgery. Our facilities utilize the team concept, incorporating therapists, dietitians, neurologists and case managers to diagnose and treat the needs of patients. Joseph M. Still Burn Center Ranked among the world s leading burn facilities, the self-contained Joseph M. Still Burn Center at Doctors Hospital of Augusta treats an estimated 7% of burn patients nationwide (3,000 patients each year) about one-third of them children. Renowned for its multi-disciplinary approach to acute burn care, it averages an impressive 97% survival rate annually. Its team of specialists consists of plastic surgeons, general surgeons, maxillofacial surgeons, critical care intensivists, pediatricians, psychiatrists, nurses, physical and occupational therapists, dietitians, and social workers. In addition, its Advanced Wound and Burn Clinic is an outpatient clinic specializing in burn treatment follow-up and other serious skin-related conditions. The Burn Center is the largest in the country with 70 beds, 4 surgical suites, a dedicated ambulance entrance, and 20 outpatient treatment rooms in addition to hyperbaric oxygen chambers and hydrotherapy. 14 specialized care Bariatric Surgery The goal at our Bariatric Centers is to improve the health and lives of those in our communities suffering from morbid obesity. With more than 60 million adults considered obese, the Centers offer specialized weight-loss treatment options to those who need help with their condition. The services offered include behavioral, nutritional, and exercise intervention, as well as surgical options that consist of gastric banding, gastric bypass and gastric sleeve. After a complete medical evaluation, a customized program is developed for each patient. Six of our hospitals offer bariatric surgery programs, two of which are designated Bariatric Surgery Centers of Excellence by the American Society of Metabolic and Bariatric Surgery - Ocala Regional Medical Center and North Florida Regional Medical Center. Comprehensive Cancer Care Cancer is the second leading cause of death in this country and our facilities are committed to meet and exceed the many standards required by the American College of Surgeons Commission on Cancer (ACoS CoC). To become accredited, our hospitals commit to regular meetings of the cancer team, which include surgeons, medical oncologists, radiation oncologists, pathologists, radiologists, and hospital clinical and support staff to proactively review patient cases. North Florida Regional s Cyberknife provides advanced treatment options for patients We are also committed to bringing the latest in technology to the communities we serve, and our Cancer Centers are no exception. The CyberKnife System, the world s first robotic radiosurgery system, is available at the Cancer Center at North Florida Regional Medical Center. West Florida Hospital s Cancer Center was opened during 2011 along with an affiliation with the Sarah Cannon Research Institute. We also offer advanced cancer treatment at our other three cancer centers in Tallahassee, FL, Lake City, FL and Augusta, GA. Before surgery, I weighed 276 pounds. I m now down to 148 pounds. I m able to go the amusement park with my students and ride the rides with them rather than sit on the sidelines. I run up and down the stairs, and our school has made me the official fitness coordinator for the whole staff. I am an advocate for movement now instead of being a fan of my couch. But the team of doctors and nurses at Capital Regional Medical Center were there to ease my fears and make my time in the hospital less stressful for my family and me.
