1 Approved Contractor Application Form Dumfries and Galloway Housing Partnership (DGHP) DGHP maintains an approved list for contractors and consultants involved in repairs and maintenance works and services that are not covered by our principle contractors. Please complete the details below and return the requested information to Ms Paula Todd, DGHP Ltd., Grierson House, The Crichton, Bankend Road, Dumfries, DG1 4ZS. Should you wish an electronic version of this form please and one will be sent to you. It is the responsibility of the applicant to provide all documentation. To aid with this a checklist has been included at the end of the form. Full name/title of company Registered Address, including telephone number and address. 2 Nature of works that you can provide. Ensure that all works that you can carry out are included as if successful you will only be contacted in relation to the works listed. If you have certificates relating to the ability to carry out these works these should also be provided: e.g. SEPA certificates for the safety of waste disposal (continue on a separate sheet if necessary) Please circle the areas in which you are able to carry out these works: Nithsdale, Stewartry, Annandale and Eskdale, Wigtown Do you provide emergency cover including a 24 hour service and/or Christmas/Bank holiday cover.
