1 Brooklyn Campus Admissions Office (718) PharmD Program Brief Overview Arnold & Marie Schwartz College of Pharmacy and Health Sciences The Arnold & Marie Schwartz College of Pharmacy and Health Sciences offers the Doctor of Pharmacy (PharmD) program for students who are interested in becoming licensed pharmacists. The PharmD program is separated into two phases, Pre-professional Phase and Profession al Phase. Pre-professional Phase Offered through the Brooklyn Campus Richard L. Connolly College, the Pre-professional Phase of the program requires completion of two years of foundation courses (total of 67 credits) in the areas of liberal arts & sciences and mathematics. During these two years of study students will strengthen their academic skills and knowledge base in preparation for entry into the Professional Phase. Each fall approximately 250 seats are offered to first time college students (freshmen). Professional Phase The program provides four years of specialized education that is necessary to develop expertise in the field of pharmacy and patient-centered care as well as prepares the student for professional licensure examinations. It consists of didactic and experiential course work in years 3, 4, and 5 followed by 37 ½ weeks of advanced practice experiences in year 6 which are completed in hospital, community and other pharmacy practice settings. Each fall approximately 200 seats are offered to both LIU students and transfer students. Applicants interested in pursuing entry into the Professional Phase of the program must submit a Pharmacy College Application Service (PharmCAS) application and complete the Pharmacy College Admission Test (PCAT) for consideration. Enrollment Applications for the PharmD Program are considered for entry into the fall semester only. Entry into each phase is very competitive and offers no guarantee of admission. An applicant s chance of selection is dependent upon the number of seats available and the number of applications within the application pool. Selection is based on several factors that include, but are not limited to: average (HSA/GPA), test scores (SAT/PCAT), personal statement, letters of recommendation and interview (for Professional Phase only). 1

2 APPLICATION REQUIREMENTS AND INSTRUCTIONS The entry requirements for each phase of the program are listed below. Please read through them carefully. Please note that while first consideration is given to LIU students enrolled in the Preprofessional Phase of the program and other current LIU majors, admission into the Professional Phase of the program is not guaranteed for any group. Transfer students who have not completed the pre-requisites for the Professional Phase but would like to enroll at Long Island University s Brooklyn Campus may apply for admission into an alternate program (i.e. Bachelor of Science in Health Science or other relevant bachelor degree programs) to complete the pre-requisite courses. Students must follow the admissions criteria for the alternate program. Admission, at a later date, to the Professional Phase of the PharmD program is not guaranteed nor will preference be factored into the process. Pre-professional Phase (Years 1-2) Key factors considered in reviewing a freshman application for admission are: rigor of curriculum and appropriate science and mathematics coursework; cumulative high school average; standardized test scores; college-level coursework (if applicable); a personal statement and two letters of recommendation. Admission to the Pre-professional Phase does not guarantee progression to the Professional Phase and will depend upon the requirements set forth by the College of Pharmacy. Requirements and Instructions 1. Complete the Long Island University Brooklyn Campus Undergraduate Application at The PharmD program will consider freshmen (first-time to college) applications for the Preprofessional Phase of the program. Applications will be reviewed and considered for fall semester only. Applications will be reviewed on a continuous basis; however those applications completed by April 15, 2013 will receive priority for consideration of admission. Submission of your application and supporting materials before April 15, 2013 does not guarantee an early decision. Applications received after April 15, 2013 will NOT be given priority for review, thus having an impact on admission. 2. Submit Supporting Credentials to the Admissions Processing Center - Up-to-date high school transcript It should reflect the coursework and grades from the beginning of your secondary education through to the last semester completed. If you have already graduated then it should reflect grades for all years of study and include your graduation date. - Updated transcript Students who are currently completing their secondary education should submit new grades as they become available, i.e. mid-year grades for the 2 nd half of the 12 th grade. 2

3 - Foreign Credential Evaluations Students who have obtained secondary education outside of the U.S. must submit credential evaluations and translations (if not written in English) for review and consideration. Please see the Credential Evaluator s List of acceptable agencies at www2.brooklyn.liu.edu/pharmacy/credential_evaluators_list.pdf. - Test Scores Includes SAT/ACT, Advanced Placement (AP), International Baccalaureate (IB), College Level Examination Program (CLEP), and English proficiency exams such as Test of English as a Foreign Language (TOEFL) or the International English Language Testing System (IELTS). *Please note that SAT/ACT scores are required for admission into all programs at the University* - Recommendations Please submit two letters of recommendation in support of your application. - Personal Statement Your statement can address, for example, a subject that is important or meaningful to you, an issue about which you feel strongly, an experience that has greatly influenced your life or circumstances that you feel are important for the admissions committee to review. - $40 Non-refundable Application Fee Payable to: Long Island University, please mail you re your check or money order to: Long Island University, Office of Admissions, 1 University Plaza, Brooklyn, NY Cash will not be accepted. 3

4 Professional Phase (Years 3-6) Application Deadline: December 3, Applies to both applications, no exceptions. PharmCAS - (Required for all applicants to Professional Phase) Brooklyn Campus Undergraduate Application (Required for transfer students only) It is recommended that applications and transcripts are sent to PharmCAS 5-7 weeks prior to the PharmCAS application deadline Submission of your PharmCAS application and transcripts prior to the deadline does not guarantee admission Application for Re-Admission: Students who have previously attended the LIU-Brooklyn Campus PharmD Program - Professional Phase (years 3, 4, 5, or 6), earned grades, did not attend classes for at least one semester, and did not complete their degree must apply for re-admission. Please contact the Admissions Office at (718) for application information. DO NOT complete the PharmCAS application. Requirements and Instructions 1. Complete and submit the Pharmacy College Application Service (PharmCAS) application. PharmCAS is an electronic application service operated by the American Association of Colleges of Pharmacy (AACP). All applications for the professional program, including external transfer and current LIU students, must be processed through PharmCAS; there are no exceptions. There is a separate non-negotiable fee required; payable to PharmCAS. Listed below are additional requirements for application completion. Personal Statement - Part of the PharmCAS application is a personal essay, approximately one page in length, addressing why you selected Pharmacy as a career and how the Doctor of Pharmacy degree relates to your immediate and long-term professional goals. You should explain why you want to be a pharmacist and support this information with autobiographical details including pharmacy/health care work, volunteer experiences, related extracurricular activities, leadership roles, or any other important information. Letters of Recommendation - Two letters of recommendation are required. These letters may be from an employer, professor, supervisor of a community service project, etc.; however, they may not come from a family member, teaching assistant, co-worker, or friend. You will need to designate your evaluators as part of your PharmCAS application. Electronic submission of letters of reference is preferred but paper references are accepted. Paper references must accompany the PharmCAS reference form (available on the PharmCAS website) in the same envelope. **PharmCAS will forward your letters of recommendation as they are completed; DO NOT wait to submit your application** PharmCAS Academic Update Window PharmCAS provides an opportunity for you to update the coursework section of your application to include grades for newly completed courses as well as edit in-progress/ planned coursework for current and future semesters. Immediately following the close of the fall semester, the Academic Update window will open 4

5 December 14, 2012 and will close February 15, **Carefully read the instructions for the academic update before completion** 2. Complete the Long Island University Brooklyn Campus Undergraduate Application: Transfer students interested in applying to Long Island University must complete the online application. There is a $40 non-refundable fee required; payable to: Long Island University. Please mail your check or money order to: Office of Admissions, 1 University Plaza, Brooklyn, NY Cash will not be accepted. Note: All current LIU students, all campuses, are exempt from this requirement 3. Submit the following supporting credentials: Transcripts Immediately following the completion and submission of your PharmCAS application and, if applicable, the LIU-BC undergraduate application, forward official transcripts and foreign credential evaluations (if attended university outside of the U.S.) to both PharmCAS and the LIU-BC Admissions Processing Center. Also, provide a list of current and future coursework in progress; i.e. fall, spring, and summer semesters (if planned). Foreign Credential Evaluations Students who have completed university education outside of the U.S. must submit credential evaluations and translations for review and consideration. Transfer of course credits will be considered on a case-by-case basis. Note: Current LIU students must submit credentials to PharmCAS only. Pharmacy College Admission Test (PCAT) - The PCAT exam measures the student s verbal ability, reading comprehension, quantitative ability, chemistry, biology and writing skills. The PCAT is required for entry into the Professional Phase of the PharmD program, there are no exceptions. First consideration will be given to those candidates who have completed the test prior to December 3, 2012; however, scores should not be older than three calendar years. Exam scores must be submitted to PharmCAS where they will be matched to your application. *Pre-registration for the PCAT is required. Please find the available testing dates listed on their website at Other Exams Standardized Tests - Transfer credit may be considered for the exams listed below. If taken, please submit the following official score reports to the Admissions Processing Center for consideration: Advanced Placement (AP), International Baccalaureate (IB), or College Level Examination Program (CLEP). English Proficiency - Students for whom English is not their native language must submit evidence of English proficiency. Please submit one of the following English Language score reports to PharmCAS: Test of English as a Foreign Language PBT / ibt or the International English Language Testing System (IELTS). 4. Grade Point Average (GPA) Applicants to the Professional Phase should possess a GPA of at least 3.0 (on a 4.0 scale) for cumulative, science, and mathematics for consideration. However, depending upon the quantity and quality of applications received and because the College of Pharmacy offers only a limited number of seats for the professional program, a more competitive GPA may be required for consideration. 5

6 TRANSFER CREDIT INFORMATION Course Transferability The transferability of course credit depends on, but is not limited to, the following: Courses are taken at a regionally accredited college or university The grade received is C or higher The course is deemed equivalent to a course provided at Long Island University s Brooklyn Campus The course is applicable to a degree offered at the Long Island University s Brooklyn Campus Note: Quarter system units should be taken in sequence (see p.7) for consideration. To determine if courses at your school are transferable or equivalent to courses that are offered at Long Island University s Brooklyn Campus, you must compare the course descriptions from your school s bulletin to the Brooklyn Campus Undergraduate Bulletin. The bulletin can be viewed on the website at Pre-requisite course requirements: Students who have or will have an Associate s or Bachelor s degree from a regionally accredited college or university prior to beginning the Professional Phase of the PharmD program will need all ten required science & math courses plus any 27 credits in social science or humanities (i.e. ENG, PHI, SOC, PSY, HUM, ART, MUS, etc.) with a grade of C or higher to complete the prerequisite course requirements. Applicants who will not have completed a degree from a regionally accredited college/university must adhere to the list of pre-requisites provided in the undergraduate bulletin. Applicants with transcripts from outside the U.S.: Official transcripts from every college/university previously attended, including those from outside the U.S., must be submitted. Also, it is required for all educational records, presented from outside of the U.S, to be evaluated and, if not written in English, translated. Please see the Credential Evaluation list of acceptable agencies for evaluations and translations at www2.brooklyn.liu.edu/pharmacy/credential_evaluators_list.pdf. In some cases this process may take 8 12 weeks so allow enough time for application and program deadlines. Transfer credit will be considered on a case-by-case basis. Applicant may be asked to submit additional supporting documents. Note: Any evaluation and translation submitted from an agency that is not on our list may not be honored. Standardized Tests: Those applicants who have completed the following exams may be considered for advanced standing or transfer credit, official test scores should be sent directly to the Admissions Processing Center for consideration: Advanced Placement (AP), minimum score 3; International Baccalaureate (IB), minimum score HL-4; and College level Examinations Program (CLEP), minimum computer based exam score 50. 6

7 EVALUATING YOUR COLLEGE TRANSCRIPTS Below is a list of all pre-requisite courses required for the PharmD Program. Use it to determine how many pre-requisite courses you have already completed as well as to determine which courses you may be lacking. To determine if a course may be transferable use our Undergraduate Bulletin along with your school s bulletin to compare the course descriptions. Visit our website at to review the Undergraduate Bulletin. Quarter system courses must be taken in sequence. Quarter credits must convert to minimally 8 semester credits for science (courses must include labs) and math courses and 3 semester credits for liberal arts or humanities courses. Note: Transfer applicants who do not hold an undergraduate degree from a regionally accredited college/university are required to complete the entire list of pre-requisite courses on this page, as listed in the Undergraduate Bulletin and the College of Pharmacy Bulletin. These are available online at Regional Institutional Accrediting Organizations Regional accreditation is required for institutional recognition as well as consideration of transfer credits. The acceptable accrediting organizations are: - Middle States Association of College and Schools, Commission on Higher Education (MSA- CHE) - Northwest Commission of Colleges and Universities (NWCCU) - North Central Association of Colleges and Schools, the Higher Learning Commission (NCA- HLC) - New England Association of Schools and Colleges, Commission on Institutions of Higher Education (NEASC-CIHE) - Southern Association of Colleges and Schools, Commission on Colleges (SACS) - Western Association of Schools and Colleges, Accrediting Commission for Senior Colleges and Universities (WASC-ACSCU) - Western Association of Schools and Colleges, Accrediting Commission for Community and Junior Colleges (WASC-ACCJC) Transcripts from outside of the U.S. Regional Accreditation may not apply to transcripts from outside the U.S. Submission of an evaluation and translation of educational records is required for consideration of admission and while transferability of credit may be considered it is not guaranteed. Note: Upon review of your file, you may be asked to submit additional documents to satisfy file completion. Criminal Background Checks and Drug Screening Information such as criminal background checks and drug screening may be required for professional program (year 3) applicants but should NOT be completed until requested by the College of Pharmacy. Test Scores SAT/ACT scores are required for entry into the Pre-professional Phase of the program. PCAT scores are required for entry into the Professional Phase of the program. Proof of English proficiency is required for applicants whose native language is not English. Applicants who have studied where English is the primary language of instruction may submit an SAT Critical Reading score as evidence of proficiency; minimum required score is 480. Otherwise, submit results for one of the tests listed below. 9 Intent to Enroll (Admitted Applicants Only) Pre-professional Phase - $100 non-refundable tuition deposit, deadline set in the letter of admission. Professional Phase - $500 non-refundable tuition deposit, deadline set in the letter of admission. The tuition deposit is required to hold your seat. If the deposit is not received by the listed deadline then the seat will be considered forfeited and granted to a candidate on the wait list; there are no exceptions. Once received, the tuition deposit will be credited to the first semester tuition upon enrollment into the College of Pharmacy. Late deposits will be returned. Note: Current LIU students applying to the Professional Phase of the PharmD program are not required to pay a tuition deposit. Wait List Once the initial class selection is complete all remaining eligible candidates are placed on a waitlist. Selection from the waitlist depends on the number of students who have committed to accept the seats offered. Waitlist selection begins as seats become available. This process is repeated until the class is filled. Status Checks PharmCAS (year 3) applicants should log into their accounts on the PharmCAS website to retrieve status information. Once the university has received the PharmCAS application the status will be changed to Received/Under Review. This status will remain until the admission decision has been made. Decision letters, wait list letters, invitations to interview and requests for additional information will be sent through U.S. Postal Mail and ed to the current address provided on the application. Financial Aid Applicants to the College of Pharmacy who are U.S. citizens or eligible non-citizens should submit the Free Application for Federal Student Aid (FAFSA). For information visit the website at Mailing Address (Transfer Applicants Only) After completion of the Long Island University Brooklyn Campus - undergraduate application, please forward supporting credentials including: official transcripts, foreign credential evaluations, AP/IB/CLEP scores (if taken), etc. to the Admissions Processing Center. Transfer students must complete the Long Island University Brooklyn Campus Undergraduate Application. 1} In addition to transcripts, test scores, personal statements, and letters of recommendation; interviews, drug screening, and background checks may be required. Additional Information Completion of required pre-requisite courses by the end of the spring 2011 semester The PharmCAS application and the Brooklyn Campus Undergraduate Application each require submission of specific supporting documents. 12 Frequently Asked Questions (FAQ s) Q: I took the SAT more than once; which scores will you consider? Will you consider SAT subject tests? While all subjects of the SAT I tests are considered, the most value is placed on the highest Critical Reading and the highest Math scores with less consideration for the Writing. SAT II subject tests are not considered. Q: What will happen if I am admitted and my final average falls below the requirement? Your acceptance is contingent upon successful completion of your current academic plan. If you fail to maintain eligibility, your seat may be withdrawn. Q: I have been admitted to the program and have paid the tuition deposit, if I decide to defer my admission for another year am I guaranteed a seat? Students admitted to the Pre-professional Phase of the program may defer from one admission year to another. However, deferring admission means that you may NOT enroll at another college or University for the term which you have requested deferment. All requests for deferral are required in writing and are subject to approval by the Office of Admissions. Students requesting a deferral must meet all current admission criteria and meet all relevant deadline and deposit dates. Students who attempt to enroll at another college or University may not maintain future enrollment status and will forfeit their seat in the entering Pre-professional class. NOTE: Students admitted to the Professional Phase of the PharmD program may not defer enrollment. Q: Will college level courses completed during high school be considered for transfer credit? Grades of C or better in appropriate coursework will be considered for transfer. Q: Does having a degree give me an advantage? Will it reduce the number of years I have to study? While having a degree is helpful for preparing for the Professional Phase it is not required for entry and therefore does not give you an advantage. An Associate s or Bachelor s degree from a regionally accredited college/ university slightly changes the liberal arts requirements (see Transfer Credit Information). The length of study depends on the number of pre-requisite courses you have completed. The least number of years of study is 4, which is the length of the professional program. Q: Will completion of exams such as Advanced Placement (AP) improve my chances? Will I receive advanced credit? Successful completion of any advanced coursework is considered. Transfer credit for Advanced Placement (AP), International Baccalaureate (IB), and College Level Examinations Program (CLEP) may be considered. Please have official scores sent to the Admissions Processing Center for evaluation. Q: If I have a Master s Degree can any of my credits transfer? Generally, graduate and post-graduate studies cannot be applied to the PharmD program. 12

13 Q: How do I know if the courses I have previously taken will transfer? All courses taken at an acceptable regionally accredited institution, with grades of C or higher, and that can be applied to your plan of study at LIU, are considered for transfer credit. Q: I see that Human Anatomy is required. I have previously taken Human Physiology. Does this satisfy the requirement? Human Physiology does not satisfy the requirement. Human Anatomy cannot be substituted but generally, courses titled Human Anatomy, Vertebrate Anatomy, Cross-sectional Anatomy, Gross Anatomy, Comparative Vertebrate Anatomy or Anatomy & Physiology I or II may satisfy our requirement. Please refer to the undergraduate bulletin for course descriptions and comparisons. Q: I currently attend a college of pharmacy and I want to transfer to Long Island University s Brooklyn Campus. Are my credits transferable? Only pre-requisite courses with grades of C or higher will be considered for transfer. The entire professional portion of the program must be completed at LIU; therefore professional credits are not transferable. Q: Does it matter what college I attended previously? Yes. Your college / university should be accredited by Middle States Association of Colleges and Schools or its regional equivalent [see Additional Information - Regional Institutional Accrediting Organization]. Q: Can I begin the professional program with pre-requisite courses still pending? To begin the Professional Phase, a student must have satisfactorily completed all required science & math courses and be missing no more than 6 credits in the required social science or humanities coursework. Q: I have attended another college and/or university but I don t want to transfer any credits; can I start over as a freshman? Students who have attended a college/university after graduating from secondary school are automatically considered transfer students and are required to submit the transcripts from all colleges/universities attended for consideration. Q: I took one course at another college do I need to submit the transcript? Yes. Submit transcripts from all colleges and universities previously attended even if the credit is not applicable to the PharmD program. Q: What is a cumulative GPA? A cumulative GPA is the result of all of your individual transcripts from each school being combined. We take the total quality points and divide it by the total number of credits to determine the cumulative GPA. All courses, even those that may not be applicable to the PharmD program, are considered in the cumulative GPA. 13

Q: Will a letter of recommendation from my boss help? Yes, a letter of recommendation from your boss and other supporting materials are considered. Q: Does my experience as a Pharmacy Technician increase my chances? While we give consideration for work experience, a strong academic background is the most important factor considered. Q: What is an official transcript? An official transcript is sent directly from your college or university directly to the Office of Admissions. It arrives in a sealed envelope from the school, which must be opened by and Admissions representative only; typically it also has a seal or some other identifying mark on the transcript itself that indicates an official copy. Q: If I don t qualify for the PharmD program at this time can I choose another major, and then apply to change my major at a later date? If qualified, you may begin studies in another department. However, we cannot guarantee that you will be accepted into the PharmD program at a later time. Your academic profile and space availability in the professional program at the time you are ready to change majors will be important factors affecting your chances of admission. Q: I am on the wait list, when will I know if I am accepted? Applications are reviewed on an ongoing basis. As space becomes available, seats will be offered to applicants on the waitlist. This process will continue until the class is filled and may continue throughout the spring and summer months. Q: How can I know if I have completed all of the pre-requisite courses? Please refer to the list of pre-requisite courses and course descriptions provided in the Long Island University Brooklyn Campus undergraduate bulletin. Q: I am currently a Long Island University Pre-professional student; will I be guaranteed a seat in the Professional Phase of the program? There is no guarantee of entry for any level of the PharmD program. It is required that all applicants to the Professional Phase of the program, including current LIU students of all majors, apply through PharmCAS before being granted consideration. Q: I am currently enrolled in a Professional PharmD. program and I want to transfer to Long Island University s Brooklyn Campus. Should I apply through PharmCAS? Students seeking transfer from colleges of pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) should not apply through PharmCAS. Please provide the LIU-BC undergraduate application, official transcripts of all academic work, statement of reasons for seeking transfer and a letter of evaluation/ recommendation from the dean of present college of pharmacy. While the Pre-professional Phase of the program is designed to be completed in four semesters, students requiring proficiency skill courses may have a lengthened course of study. Q: When is the best time to submit the PharmCAS application and supporting credentials? Submit your application and credentials as early as possible. It takes PharmCAS approximately 4-5 weeks to verify grades; therefore, the college receives your application 4-5 weeks after you have submitted it. Update
