2 PRESIDENT S MESSAGE Dear Colleagues, Join us at the 2016 Rocky Mountain Urological Society meeting at the Embassy Suites Denver Tech Center, April 8 9. Dr. Brian Flynn and the Program Committee have brought together a number of dynamic speakers for our agenda this year. Our main topics will be bladder cancer and the practical management of kidney stones, and we are excited to bring in Dr. Jeffrey Holzbeierlein from the University of Kansas School of Medicine and Dr. Julie Riley from the University of New Mexico School of Medicine to each present several topics. We will also have panel discussions incorporating some of our local experts in clinical and academic practice. Besides our main topics, we will also have a COPIC lecture as well as a health policy presentation in which we will predict the future of American medicine with help from various physicians and administrators representing different areas of practice. We will also have a moderated poster session this year to highlight the impressive efforts of our urology residents. This has proven to be a popular addition to the annual meeting. Jennings, MD President, Rocky Mountain Urological Society EDUCATIONAL NEEDS & OBJECTIVES Educational Needs Bladder Cancer: Bladder cancer is common in industrialized countries. It is the fifth most common type of cancer in the United States. It is the fourth most common in men and the ninth in women. This disease is associated with significant morbidity and death. Urothelial carcinoma (transitional cell carcinoma) is by far the most common type of bladder cancer in the United States. The so-called transitional cells are normal cells that form the innermost lining of the bladder wall. In transitional cell carcinoma, these normal lining cells undergo changes that lead to the uncontrolled cell growth characteristic of cancer. Bladder cancers are classified by how deeply they invade into the bladder wall, which has several layers. Typically we subdivide bladder cancer into superficial and invasive diseases. There is significant variation in workup and treatment of bladder cancer across the country. Technology is rapidly changing, with introduction of numerous new minimally invasive medical and surgical options over the past few years. Given the significant role that community urologists play in diagnosis and treatment of bladder cancer, it is felt that a comprehensive update on diagnosis, workup and treatment of bladder cancer is needed at this meeting. Patients with high risk non-muscle invasive bladder cancer present a unique challenge to the practicing urologist. The opportunity to preserve a patient s bladder must carefully be weighed against the risk of progression. In this session, the speakers will discuss the role of BCG in the management of high grade NMIBC, the risks of progression in these patients. In addition, the speaker will review how to manage side effects of BCG and what to do when patients can t tolerate or receive BCG. There has been a fair bit of debate recently regarding the optimal surgical approach for patients with muscle invasive bladder cancer undergoing cystectomy. The results of the RAZOR trial, a randomized trial of open cystectomy vs. robotic cystectomy, are currently pending. However, in this session, the speaker will review the available date for robotic and open cystectomy and make the case that patient selection is the critical component on which approach to choose. Early recovery pathways are a mainstay of cystectomy. However, there is a critical need to improve nutrition during this time period, which has not been addressed in most ERAS pathways. In this session, the speaker will present data on the baseline malnutrition seen in patients undergoing cystectomy. The speaker will discuss how malnutrition effects outcomes and then describe a pilot study that we have conducted and recently published looking a nutritional supplement that can modulate the immune response after cystectomy. The speaker will present data on preserving weight after cystectomy and decreasing surgical infections using this approach. Robotic Surgery: Robotic-assisted laparoscopic surgery is a minimally invasive way to perform abdominal and pelvic surgery. The Food and Drug Administration approved robotic surgery with the Da Vinci Surgical System in The technique has been rapidly adopted by hospitals in the United States and Europe for use in the treatment in a wide range of conditions. The most
3 widely used clinical robotic surgical system includes a camera arm and mechanical arms with surgical instruments attached to them. The surgeon controls the arms while seated at a computer console near the operating table. The console gives the surgeon a high definition, magnified, 3D view of the surgical site. The surgeon leads other team members who assist during the operation. It is important that urologists specify and formulate the technical aspects and key procedural steps to optimize surgical outcomes. The speaker will give the audience the technical pearls to improve patient care and outcomes. Kidney Stone Disease: Urolithiasis is the term that refers to the presence of stones in the urinary tract, while nephrolithiasis (nephro means kidney, and lithiasis stone) refers to kidney stones and ureterolithiasis refers to stones lodged in the ureter. The size of the stone doesn t matter as much as where it is located and whether it obstructs or prevents urine from draining. Kidney stone disease is the most common problem a urologist will see in their practice. There are a variety of ways treating stone disease including ureteroscpy, shock-wave lithotripsy and percutanous nephrolithotomy. Ureteroscopy is a minimally invasive method of treating stone disease and can be used either as primary therapy or as salvage therapy for residual stones following treatment with other modalities such as extracorporeal shockwave lithotripsy (ESWL) and/or percutaneous nephrolithotomy (PCNL). Compared with ESWL, ureteroscopic lithotripsy achieves a greater stone free state. In select cases, ureteroscopy has been shown to be a viable and effective means of treating stone disease where ESWL may be contraindicated (e.g. pregnant women and pediatric patients). In percutaneous nephrolithotomy or nephrolithotripsy, the surgeon puts a hollow tube into the kidney and either removes (lithotomy) or breaks up and removes (lithotripsy) the stone. This surgery may be used if shock wave lithotripsy or ureteroscopy does not work or if there is a very large stone. Urologists are frequently called upon to educate, diagnose, and treat men with urolithiasis. Given the rapid rise of such therapy, and given conflicting data, it is felt that a thorough discussion of this subject is much needed at this year s meeting. The speaker will give the audience the technical pearls to improve patient care and outcomes when performing ureteroscopy and percutaneous nephrolithotomy. Health Policy and Medico Legal Concerns for Urologists: As part of the rapidly changing health care delivery system in this country, payers both private and public systems are evolving to care for patients. Different systems use different models to care for patients and provide quality of care delivered by physicians. In the socioeconomic hour at this year s meeting, we will explore the changes in practice that impact physicians. We will also provide information on what happens when a patient files a complaint against a urologist and how the provider may respond including the role of arbitration and settlement. Educational Objectives At the conclusion of the RMUS 2016 Annual Spring Meeting, attendees should be able to: 1. Identify the risk factors for bladder cancer. 2. Describe when to preserve the bladder in patients with cancer. 3. Identify when early cystectomy is indicated. 4. Describe the role of BCG in the management of high grade NMIBC. 5. Report the risk factors for bladder cancer progression. 6. Demonstrate how to manage side effects of BCG, and illustrate what to do when patients can t tolerate or receive BCG. 7. Analyze when to perform an open cystectomy and when to perform Robotic cystectomy. 8. Summarize the RAZOR trial, a randomized trial of open cystectomy vs robotic cystectomy. 9. Create factors that are important for patient selection for various approaches the cystectomy. 10. Integrate the role of nutrition in early recovery pathways for cystectomy, and review baseline malnutrition seen in patients undergoing cystectomy. 11. Explain how malnutrition affects cystectomy outcomes, the role of nutritional supplements and how they can modulate the immune response after cystectomy. 12. Indicate how nutrition can preserve weight after cystectomy and how optimal nutrition can decrease surgical infections. 13. Formulate the role of radiation therapy and chemotherapy in the treatment of bladder cancer. 14. Determine how to place trocars and dock the robot for robotic surgery, optimize patient positioning in robotic surgery. 15. Explain how to improve surgical techniques in robotic surgery and ureteroscopy. 16. Describe surgical techniques for placing a ureteral access sheath and ureteral stent and baskets. 17. Describe surgical techniques for pecutaneous stone surgery. 18. Describe the laser settings for lithotripsy. 19. Identify how to gain access for percutaneous nephrolithotomy. 20. Explain procedure selection for urolithiasis. 21. Describe how value based purchasing affects the business of medicine. 22. Explain the legal process if you are named in a patient complaint or lawsuit, arbitration, and/or settlement in a medical malpractice claim.
