1 Member Medical History Plus: How MMH+ Can Help You Transform Your Practice
2 MMH+ is designed around you MMH+ provides patient-based personal health information online MMH+ is easy to use: Practices don t need any special hardware or software. One of our provider representatives will set up external access for any participating practice that wants to use the program. 2
3 MMH+ allows for secure information exchange MMH+ is secure: It meets all HIPAA security requirements. When you re ready to get started, you can access MMH+ via a secure URL, using an individual user ID and password. Via the Communication tab inside MMH+, we will alert users to planned outages, enhancements or login changes. 3
4 MMH+ at your fingertips MMH+ is flexible: Patient data can be exported into Excel for inclusion in patient charts. Primary access is via internet URL. MMH+ is free: There is no charge for providers to use MMH+. MMH+ is fast: A recent rewrite of the application included a move to a true transactional database platform. It typically takes only a few seconds to retrieve a patient s record. 4

7 How MMH+ serves patients: Susan s Story Susan is a new patient starting in the practice. She filled out her initial assessment several days before her appointment via a patient portal. The afternoon before Susan s appointment, Janice, the practice s medical assistant reviews the patient list for the next morning. She notices Susan s coverage history and looks up her record in MMH+. 7
8 How MMH+ serves patients: Susan s Story Janice prints out the patient summary and provides it to Dr. Jones and the care team to review during the morning huddle. Susan has diabetes, which she reports has been a struggle to keep managed. 8

MMH+ has a record of her medication history and labs for the past two years. Dr. Jones and his team are able to use this information, along with Susan s reports of her eating and lifestyle habits, to create a care plan to help get her diabetes under control. 99
