4 Description This report shows service delivery sites, with profile and primary member contact information affiliated by Employment Service (ES), Summer Jobs Service (SJS), Literacy and Basic Skills (LBS), or Ontario Self Employment Benefit (OSEB) program type. Access This report is available to all service providers and ministry staff. Format, Schedule and Retention This report is in Excel format to accommodate the ability to adjust font size, column widths, sort and manipulate data, and print based on service provider preferences. It will be replaced daily with no retention on the EOIS SP Reporting Website. Data Filtering Service delivery sites with an active service sometime within the fiscal year with at least one active member will appear on the report. Service delivery sites that do not have a ministry contact are excluded. Data Sources The information on the report is obtained from the service provider records as set up by the ministry. Service Provider Information Service Provider Business Name, Service Delivery Site Name, Service Delivery Site Number, Preferred language, Preferred Communication Method, Designated Francophone, Mailing Address, Physical Address, Primary Phone Number 1, Fax Number, are sourced from the Service Provider and Service Delivery Site Home pages. 1 The report shows a TTY indicator if applicable to the phone number Release 5.2 Version The Queen s Printer Page 4 of 6
5 Service Provider Contact Information Primary Contact Name, Contact -Phone 2, Contact , are sourced from the Service Delivery Site Members page. This section will be blank if there is no main SDS contact information.
