1 McCloud Healthcare Clinic 2011 Accomplishments McCloud Healthcare Clinic s Mission Statement: to offer access to a full scope of high quality healthcare to everyone without regard to their ability to pay. McCloud Healthcare Clinic s Vision Statement: Our goal is to provide a conservative, affordable approach to all families health needs. The 2011 year proved to be an important year for the McCloud Healthcare Clinic in terms of sustainability, quality of care and commitment to mission. The state of the environment of healthcare in 2011 was faced with funding cuts, state budget reductions, increased service requirements without compensation all complicated by a financially stressed patient population. The MHC Board of Directors along with the clinic administrator has been keenly aware of these challenges and has looked for innovative and effective avenues to take as to keep our clinic viable in the future. Not only have we increased patient volume in 2011 but we have added a psychiatric PA-C, a Marriage and Family Therapist and maintained our podiatric and chiropractic services. As we enter 2012 we do so with opportunity and options to ensure sustained healthcare for our community. In the face of past, present and future challenges the board, staff and administration has and will work tirelessly to overcome and excel in not only continued sustainability but in offering a diverse scope of quality care. In the midst of governmental chaos we are determined and positioned to be here for years to come. Thank you all for supporting the McCloud Healthcare Clinic in 2011 and being an important part of why we can continue to offer our services to you and your loved ones. 3 McCloud Healthcare Clinic 2011 Accomplishments Organizational Milestones Secured a contract with a superior Information Technology Company We received our FQHC recertification to continue through 2013 A registered nurse is available during all clinic hours We secured a part time internal medicine provider, Dr. Mirtha Balcazar McCloud Healthcare Clinic submitted a New Access Point grant through Health Services Corp to become a 330 FQHC which would provide grant funding for increased services. Funding possible 06/2012 Members of the California Primary Care Association Our PA-C Denis Hentrich completed a psychiatric PA-C internship and returned to our clinic Began the process to become a Patient Centered Medical Health Home Hired a Shantel Joling, PA-C 2 new front office staff Selected our Electronic Health Records System Entered into a contract with CalHipso Secured Video Store for expansion possibilities Initiated e-prescribing Initiated Northstate Online Referral System Initiated UC Davis Online Referral System Began credentialing/admitting process for all MHC primary care providers Grants 2011 Grants and Donations Hearst Foundation 2 nd year of $150,000 grant - $75,000. Catholic Healthcare West (CHW) Diabetes Grant $11,067 Blue Shield Indigent Care Grant - $20,000 McConnel Foundation Dental Equipment Grant-$42,300 Health Alliance of Northern California Indigent Care Grant - $5,000 County Medical Services Program (CMSP) Pilot Project o We completed our last year of the CMSP Behavioral Health Pilot Project that partners with Siskiyou County Behavioral Health and local area therapists to help meet the behavioral needs of our county. o We are currently the only CMSP Behavioral Health Providers in Siskiyou

McCloud Alumni Association - $1,000 Grants Secured for 2012 Dignity Health Diabetes Grant - $16,000 Blue Shield Indigent Care Grant - $20,000 Siskiyou County Behavior Health Partnership - $85,000 California Telehealth Network Grant/College of the Siskiyous for Telemedicine Equipment - $45,000 McCloud Alumni Association - $1,000 Grants Written and Pending Announcement for FQHC New Access Point Grant (Health Resources and Services Administration) o $650,000 per year for 2 years. Renewing every 3-5 years thereafter. This grant would provide increased services in McCloud including a dental clinic This grant would establish a primary care clinic in Dunsmuir 2011 Donations Over $20,000 was raised at this year s McCloud Healthcare Clinic Golf Tournament Private donations for 2011 reached over $8,111. We received a $5,000 private donation this year. We competed in a matching fundraising campaign with the Campbell Group and together we raised over $4,000. Grants and donations allow our clinic to provide services above and beyond what we could offer if we only ran by our fees for service alone. These funds allow us to purchase life-saving and/or treatment enhancement equipment like the EKG machine, our generator and new exam tables Program Highlights Began preparing for and adopting Patient Centered Health Home philosophy Community Garden Project (Thank you, Denis) o This project created the opportunity for our community members to exercise, set goals, be creative, work independently and as a team, and to grow, eat and learn about healthy foods. Pain Management Guidelines Adopted o These guidelines have been in development for over a year. They are the product of our area practitioners wanting to give the best care to our patients in pain. Free A1C Diabetic Screening in clinic

Smoking Cessation/Hypno-therapy Diabetes Program o This is a multi-faceted program that ranges from individual monthly monitoring, self management, weekly support meetings to weekly exercise classes.

A goal of this meeting is to work as a team, include the patient and create a plan of care.

Participated in McCloud Heritage Days Diabetes information booth at the Siskiyou County Golden Fair Booth at Harvest Festival Senior Fair Facilitated and participated in the Siskiyou County Dental Van Project. Dental van was in McCloud for 2 full days.
