2 s Welcome to the Dental Services in Shropshire The provides specialist dental services, emergency dental services and routine dental services to patients and local communities in Shropshire. This leaflet contains information about services we provide, how to make an appointment and contact the clinic. Services on offer are: Children s and Special Care Dentistry Emergency Dental Service Emergency Dental Service - Out of Hours service - Dental Practices

3 Services Provided Children s and Special Care Dentistry Full range of dental care provided to children and adults who need to access special care dental services. Domiciliary service (home visits by the dentist) for patients unable to attend the dental clinic due to medical/health problems. Referrals are made by Healthcare Professionals, Residential Care Home workers or family members. General Anaesthesia and Conscious Sedation By referral the service offers out-patient dental treatment under general anaesthesia for children and adults at the Royal Shrewsbury Hospital, Princess Royal Hospital and Robert Jones & Agnes Hunt Orthopaedic Hospital. The service also provides dental treatment under inhalation sedation (laughing gas) and intravenous sedation at the Special Care Dental Centres. Emergency Dental Service Emergency Dental Service Access Centres provide emergency dental treatment to the general public in Shropshire. Emergency Dental Service - Out of Hours If you have a serious dental problem that cannot safely wait until normal working hours then contact the Emergency Dental Line on Dental Practices There are two NHS Dental Practices in Shropshire: Craven Arms Market Drayton

7 Patient Information Making an appointment If you ask us for an appointment, we will try and offer one as soon as possible. You can make an appointment by either phoning, in person, fax or . Reminders and recalls At the end of your course of treatment, your dentist will discuss with you when you will need to see a dentist again. This means you can attend the dentist as often as needed to keep your teeth and gums healthy. Missed Appointments and Cancellations Missed appointments waste NHS time and resources. If you are unable to keep your appointment please let us know as soon as possible. If you miss two appointments and give less than 24 hours notice for a cancellation, we may not be able to offer you NHS dental care in the future. For more information on Missed Appointments please refer to the procedure in the waiting room or ask reception. Your Dental Records Your dental records will remain confidential and secure. However from time to time we may need to release information to Shropshire Community Health NHS Trust and/or the NHS Business Service Authority as part of our ongoing clinical review process. We will try to contact you before releasing your dental records to another organisation.

8 Your Rights and Responsibilities You are entitled to: A thorough examination of your mouth, teeth and gums. A full explanation of your treatment options. A written treatment plan (including costs). Information about NHS Charges displayed in the waiting room. Advice on how to keep your teeth and gums healthy. Information about this clinic/practice and services available. A care and treatment summary if you decide to transfer to another dentist. Make a complaint if you are not happy with your treatment and care, ask reception for details.

9 Your Rights and Responsibilities You are responsible for: Giving at least 24 hours notice if you have to cancel or change an appointment. We will not charge you for missed appointments but if you miss two appointments we may no longer be able to offer you treatment. Following your dentist s advice to prevent tooth decay and gum disease. Paying your bill promptly at the time of your appointment. For payment plan information, please ask reception for details. Bringing proof of patient charge exemptions when attending for NHS Dental Care. Patients have the responsibility to bring evidence with them of patient charge exemptions. Benefit eligibility checks are carried out by the NHSDS, if the patient is found not to be entitled to free dental care then they will be asked to pay the NHS charges and may also receive a further penalty charge. Treating our staff with courtesy and respect. Patients found to have incorrectly claimed help with the cost of NHS dental treatment may receive a penalty charge.
