Failure to successfully complete all requirements prior to the beginning of PT9583 will result in cancellation of the students PT9583 clinical placement 1 COURSE DESCRIPTION Full time clinical experience in a designated clinical placement setting hours 40/week; 6 weeks Full course This Intermediate-Senior Clinical Experience placement will provide the opportunity for year two students to further integrate the skills, behaviors, knowledge and clinical reasoning learned in the Year 1 and Year 2 Advanced Practice portion of the MPT program, while under the supervision of a registered Physical Therapist in approved clinical facilities. Students will have the opportunity to gain experience in Physical Therapy practice - assessment, differential diagnoses, treatment and evidence based practice incorporated in a variety of clinical practice settings. This Intermediate-Senior clinical placement experience will provide the opportunity for students to be introduced to more complex and challenging cases emphasizing the skills and clinical reasoning required to assess, find and integrate information, select and perform treatment skills under the supervision of a registered Physical Therapist. During the placement it is anticipated that increased professional responsibilities will be added and supervision decreased. Students completing clinical experience placements will wear nametags that clearly identify themselves as a physical therapy student. All client/patient health records and related documentation must be co-signed by the supervising physical therapist or designate.
2 Language proficiency Each student granted admission to Western must be proficient in spoken and written English. Students must demonstrate the ability to write clearly and correctly. Work which lacks a proficiency in English (unacceptable for academic credit) will either be failed or, at the discretion of the instructor, returned to the student for revision to a literate level. UWO Senate regulations require all instructors to take proficiency in English into account in the assignment of grades. Attendance 100% attendance for all clinical experience placements is mandatory, as it involves the clinical care of patients, and is therefore a professional responsibility. See student handbook for further information regarding attendance. 2. EXPECTED CLINICAL LEARNING OUTCOMES for STUDENTS Based on the Essential Competency Profile for Physiotherapists in Canada (2009), by the end of the MPT Senior Clinical experience placement (PT9584), the student should be able to meet the following criteria by the end of the clinical placement: Expert 1. Consistently practice a safe manner that minimizes risk to patient, self and others. 2. Consult with the client to obtain accurate information about his/her health associated history, previous health interventions and associated outcomes 3. Demonstrate the ability to collect accurate information and perform an accurate assessment/reassessment of patient status (physical, functional) in any of four Settings (Acute Care, Rehabilitation, Clinics, Community) for straightforward and some complete clinical cases relevant to the clinical placement setting with CI consultation. 4. Demonstrate the ability to analyze patient needs based on subjective and objective assessment findings with CI cueing and consultation. 5. Establish physical therapy diagnosis/prognosis, with some CI cueing and consultation 6. Develop an intervention/treatment plan and D/C plan that is client centered, outcome focused, evidence informed and realistic with some CI cueing and consultation 7. Perform effective treatment procedures such as therapeutic exercise, soft tissue/ manual therapy techniques, electro-physical agents and mechanical modalities, functional activity training (e.g. gait training, transfers, mobility, and wheelchair management, instruction in the appropriate use of assistive devices and gait aids, and effective use of cardio-respiratory techniques) with minimal CI supervision. 8. Evaluate the effectiveness of treatment intervention with minimal CI consultation. 9. Understand issues related to impairment, activity and participation within the clinical setting Communicator 10. Communicate verbally and nonverbally, in an effective, professional and timely manner that is congruent to the situational needs. 11. In a timely manner produce accurate and comprehensive documentation to support physical therapy assessment, analysis and treatment/intervention provided. 12. Effectively educate others (patient, family, caregivers, staff, students, and most other health care providers) regarding physical therapy care of the patient/client condition and health/wellness, using relevant and effective teaching methods.
3 Collaborator 13. Work collaboratively and effectively within interprofessional health care team to achieve patient goals most of the time. 14. Demonstrates basic collaborative skills in interprofessional (IP) role clarification, communication, client/family/community centered care and Team Functioning, most of the time. Manager 15. Participates in safe and effective physical therapy practice. Consistently recognizes and prevents hazards and injury. 16. Manage resources (e.g. time, space, equipment, work load statistics, scheduling patients) to achieve goal of practice setting with some assistance by CI. 17. Works with support personal involved in the delivery of patient care and physiotherapy services some of the time under CI direction. 18. Manage approximately 50%-60% graduate entry to practice PT caseload relative to clinical setting Advocate 19. Works collaboratively to identify, respond to and promote the health needs and concerns of individual clients, populations and communities with substantial assistance from CI Scholarly Practioner 20. Implements a self-directed plan for professional development and lifelong learning 21. Effectively utilizes self-reflection and feedback from clients, families, clinical preceptor and other health care providers. 22. Uses principles of research and evidence informed practice in the delivery of physical therapy care with simple and some complete clients with CI consultation. 23. Record a relevant clinical question, review the literature, select and review 3 significant research articles related to the clinical question, and complete a Clinically Appraised Topic (CAT) for each of the 3 articles Professional 24. Understands the role of Physical Therapy in the clinical setting. 25. Dresses and acts in a professional manner at all times. 26. Provides services within scope of practice, personal competence, standards of practice and relevant legislation (i.e. confidentiality and patient consent) with CI consultation.
4 3. EVALUATION a. Clinical Performance Instrument (CPI) The Clinical Performance Instrument (CPI) developed by the American Physical Therapy Association will be used for evaluation of performance during each clinical placement. For all clinical placements, MPT students are graded on a pass/fail basis, based on the learning outcomes outlined in this course outline and those outlined in the MPT PT9581-PT9585 Expected Clinical Performance Learning Outcomes Table. At midterm and final evaluation, the CI will provide formative written comments and verbal feedback to the student on individual CPI items as well as identify areas of strength, areas needing improvement and comments on the student s overall performance. Students are required to discuss their CPI self-assessment with the supervising therapist. b. Significant Clinical Performance Concerns 1. A student with significant concerns as flagged on the CPI for any of the 24 performance criteria will be drawn to the attention of the ACCE by the midterm evaluation date, or as soon as they occur, such that a remedial learning plan can be put into place. 2. Should there be any concerns by the CI whether the student should pass or fail the placement, the ACCE, CI and CCCE together will make a collaborative decision related to a pass or fail mark for the students placement, considering the clinical placement expectations and the students clinical performance. 3. The ACCE (or designate) reviews all completed CPI s, and signs the evaluation as part of the students formal academic record. c. Clinical Instructor and Student Obligations Considering the five performance dimensions, Quality of care, Supervision/guidance required, Consistency of performance, Complexity of tasks/environment and Efficiency of performance, the Instructor (CI) will: 1. Using the CPI, complete a written and verbal midterm and final evaluation of the student overall clinical performance, providing feedback that identifies clinical areas of strength and areas needing improvement. 2. Sign the CPI for midterm and final evaluation. 3. Send the CPI to the SPT by the CI / CCCE for inclusion in the student s MPT academic record. The Student will: 1. Complete a self-evaluate at midterm and final evaluation using the CPI and discuss their self-assessment with the supervising therapist. 2. Sign the CPI for the midterm and final evaluation for the CPI to be included as a part of the student s academic record. 3. Complete the on-line NACEP Site and Clinical Instructor Evaluation and review with the CI at both midterm and final evaluation
5 4. Complete the on-line MPT Setting & Area of Practice Summary form at the end of placement and submit to SPT. 4.
