3 Leaders Guide EURAMAX PROCEDURE REFERENCE: I-2.0: Accident Investigation and Reporting MEETING OBJECTIVE: Making sure that accidents and near misses are reported is an important part of our overall safety program. If you don t know about workplace hazards, you can t control or eliminate them. The purpose of this meeting is to teach your employees the importance of reporting all accidents however minor as well as near misses. Employees will learn correct reporting procedure, what to do and what not to do when they witness an accident, and how to present the facts in a way that will help in the investigation that follows. MEETING PREPARATION: Read the Euramax procedure, understand the contents, and ensure compliance. Make copies of the Euramax accident report form to distribute to employees. This handout will help them better understand exactly what to report about an accident. Think of an accident or a near-miss that occurred in your facility that had a positive outcome one where a hazard was controlled or eliminated as a result. Choose an accident that was relatively minor where no one was seriously hurt, or where no one was hurt one that might have gone unreported. Be prepared to discuss this accident and the positive changes that resulted. Review the employee handout to see if there are any other materials you wish to bring to the meeting. Use a flip chart during the discussion to write key points and employee responses. This technique visually reinforces your instruction. 4 Leaders Guide MEETING INTRODUCTION When an accident happens, it s important for witnesses or victims to report the incident and cooperate in the investigation. Often people think that an accident is too minor to report or that they ll get a co-worker in trouble if the report it. The purpose of this meeting is to set the record straight on accident reporting. The only way we can prevent accidents from happening is if we learn as much as possible about the ones that have already occurred. That way we can prevent them from happening again. When you report accidents and cooperate in the investigation, you help protect yourself and your co-workers form future harm. Accidents claims thousands of lives and injure many more workers around the country every year. Injured workers lose time from work and frequently must endure physical pain and suffering. Some are disabled for life. The sad part is that most accidents can be prevented. If all workplace accidents and near-misses were reported, the accident rate would decline significantly. Why is it important to report all accidents? It s Euramax policy to report all accidents. Someone could be more seriously injured than they realize. The information you provide can help uncover the cause of the accident, which will help prevent similar accidents from happening in the future. What if no one got hurt or it was only a minor injury? Do you need to report it then? Absolutely. It doesn t matter whether anybody got hurt or whether it was just a scratch. An accident is an accident. If it goes unreported, next time the outcome might be worse. Page 2

5 Leaders Guide What about near-misses? Should you report them? After all, nobody was hurt. Near-misses should be reported just the same as accidents. A near-miss is an accident waiting to happen. Describe the accident that led to improved safety conditions at your facility. What might have happened if no one had reported this accident? Positive improvements in our safety conditions might not have been made. Another accident involving the same conditions might have occurred. This time someone could have been seriously hurt. Discuss the importance of the first few minutes after an accident. Explain that quick action can make the difference between life and death for a seriously injured victim. What should you do if someone is injured in an accident? Assess the situation and determine if it s safe for you to approach the victim. If it is safe to assist, help the victim while someone else goes for help. Do no further harm. Don t move an injured person unless there s a danger of fire, explosion, or some other life-threatening situation. Administer first aid if you know how. After an accident occurs, it s important not to disturb the area because you could hamper the accident investigation. What steps should you take to make sure the area remains undisturbed? Don t move anything or destroy evidence. Stop others from moving or touching anything. Page 3

6 Leaders Guide Don t begin cleanup until told. Block off the area to keep others away. Explain that when you are involved in an accident as a victim or a witness, every thing you see is vital information that can help determine why the accident occurred and what must be done to prevent this type of accident from happening again. Your role is like that of a news photographer and reporter. You should try to take mental snapshots of the incident and later make notes of what you saw. What should you ask yourself when taking mental snapshots of an accident? Who was involved? What actually happened? Where did the accident happen? When did it happen? Why did it happen? Why is it best not to discuss an accident with co-workers until you have written down your notes and given a full report to the appropriate person? Talking about the event with others might change your account of the accident. You might remember what other people said about it, rather than what you actually saw. Distribute copies of the Euramax accident investigation form. Discuss the details of the form with your group. This will help your employees better understand the kind of information they should be prepared to report after the accident. SUMMARY: Let s hope that none of you is ever involved in an accident on the job. But if you are, remember what we ve discussed today. Your role in accident reporting can make Euramax a safer place for everybody, including you. And remember that what we have said about reporting accidents applies to near misses, too. A near miss is an accident waiting to happen. If you report a near miss, you are helping prevent accidents. Page 4

8 Employee Handout Accidents are the result of unsafe acts or unsafe conditions, or both whatever the reasons, we want to eliminate them to keep you safe and you can help! Unsafe conditions (10%) are physical hazards such as missing machine/protective guards, exposed electrical circuits, damaged equipment, slippery floors, improper storage of material, lack of supervision, and inadequate training. Unsafe Acts (90%) are the things people do that are obviously just not safe. Some examples are: Horseplay Not using PPE Running Using damaged tools Not lifting properly Violating safety rules Accidents can result in injury or death to you or another employee. That s why it s important to immediately report any unsafe conditions to your supervisor. Excuses some people use That s the way I always do it I can take shortcut because I m experienced I was just trying to fix it I thought I knew how I was in a hurry. Doing it safely takes too much time I didn t know it was loaded ALL Accidents are preventable!
