1 Healthy Living Presentation Outline 5 Minutes Objective: To effectively communicate the nature and structure of a Healthy Living Workshop and provide potential participants and/or host agencies with a clear understanding of the commitment involved Length of Presentation: Presentation length varies based on the time allotted. The following document includes outlines for a 5, 15, and 30 minute presentation. Instructions: This document is designed to provide a basic outline for a verbal presentation. It is important that the key elements outlined are included in any presentation so potential participants or host agencies have a clear understanding about the program. Presentations do not need to be word for word, paraphrasing is encouraged. Brief personal stories or testimonials from past participants allow the presenter to connect with the audience. Five Minute Outline Materials needed: Fact sheet (to be handed out to participants prior to presentation) Workshop flyer Copy of Living a Healthy Life with Chronic Conditions Sign-up sheet. Introduce yourself State your name and the agency you represent I m here to talk to you about the Healthy Living: Self-Management of Chronic Conditions Workshop. This workshop is designed to deal with the problems caused by chronic conditions like diabetes, arthritis, COPD, heart disease and many more. Participants may have a chronic condition themselves or may have a close friend or family member with one. It is helpful to include an example of how the workshop relates to you personally or provide the testimonial of a former participant. Two examples: I know first-hand how chronic conditions affect a person s life. Recently my husband was diagnosed with hypertension. He is now taking medication and needs to change his eating habits, which directly affects me since I m the cook. I also worry about him and what problems we could face if he doesn t get his blood pressure under control. Betty, a participant in one of our recent workshops experienced a lot of pain and fatigue due to her condition and wanted to learn about some ways that she could deal with those symptoms. Transition: We can t change the fact that we or our loved one have been diagnosed with a chronic condition, but we can change the way we manage the problems associated with it, that is where this workshop can help.
2 Basic Structure of the course (what is it) Workshop meets once a week for 2 ½ hours for 6 weeks Facilitated by two trained leaders, one or both have a chronic condition This is a self-management workshop, we don t tell you what to do but help you discover how to. Not a lecture course, but an interactive, supportive environment Topics covered include, problem solving, dealing with difficult emotions, healthy eating, exercise for fun and fitness, medications, and communicating with health professionals and loved ones. Focused on action plan and problem solving (briefly describe the action planning and problem solving process) Workbooks to guide participants (State you agency s book policy, donation, lending library, etc.) If you have time tie in the testimonial you gave at the beginning to how this workshop benefits the individual.
