1 Community Health Needs Assessment Implementation Plan Advocate South Suburban Hospital () Date Endorsed by Governing Council: March 27, 2014 PRIORITY AREA: Target Population: GOAL: Childhood Asthma Children ages 5-17 that reside in the primary service area of Advocate South Suburban Hospital () which includes but is not limited to the following zip codes: Calumet City, Chicago Heights/Ford Heights, Dolton, Flossmoor, Frankfort, Glenwood, Harvey, Markham, Hazel Crest, Homewood, Lansing, Matteson, Midlothian, Oak Forest, Olympia Fields, Park Forest, Richton Park, and South Holland. To decrease asthma related Emergency Department visits and/or hospitalizations of children ages 5-17 that reside within Advocate South Suburban Hospital s primary service area by developing and implementing programs to increase the knowledge base of the pediatric asthma community that include asthma management skills, trigger identification, and therapeutic and social interventions that will reduce asthma related complications while establishing self-advocacy skills and ultimately creating a better quality of life for those we serve. Healthy People 2020 Alignment: RD-2.2 Reduce hospitalizations for asthma among children and adults aged 5 to 64 years. RD-3.2 Reduce emergency department (ED) visits for asthma among children and adults aged 5 to 64 years. RD-7.1 Increase the proportion of persons with current asthma who receive written asthma management plans from their health care provider according to National Asthma Education and Prevention Program (NAEPP) guidelines. RD-7.3 Increase the proportion of persons with current asthma who receive education about appropriate response to an asthma episode, including recognizing early signs and symptoms of monitoring peak flow results according to the NAEPP guidelines. PERFORMANCE MEASURES How We Will Know We are Making a Difference Short Term Indicators Source Frequency 100% of Pediatric Emergency Department and Inpatient admissions will receive a written asthma action plan with an implementation date of 6/30/2014. Chart Review 100% of Pediatric Emergency Department and Inpatient admissions will receive education on prescribed inhalers prior to discharge with an implementation date of 6/30/2014. Chart Review Increase by 10% of baseline, Pediatric Emergency Department and Inpatient admissions who fill their prescriptions after being discharged within 72 hours with an implementation date of 6/30/2014. Post-Discharge Call Long Term Indicators Source Frequency Reduce pediatric (< 18 yrs) asthma related inpatient hospitalizations to by 10% by 12/31/2015. Midas Quarterly Reduce pediatric (< 18 yrs) asthma related ED visits to by 10% by 12/31/2015. Reduce the proportion of children ages 5-17 within the primary service area who miss school days related to asthma by 10% by 12/31/2016. Midas School nurses Quarterly August-June 1
2 OBJECTIVE #1: Advocate South Suburban Hospital () will increase the proportion of pediatric persons with current asthma who receive appropriate asthma care according to National Asthma Education and Prevention Program (NAEPP) guidelines while an inpatient or during an Emergency Department visit to 100% by June 30, Evidence Base: National Asthma Education and Prevention Program; Guidelines for the Diagnosis and Treatment of Asthma National Heart Lung and Blood Institute: National Asthma Education and Prevention Program (click here) 1. Update the asthma action plan. 3/2014-4/2014 Staff time (, IS, Pediatrics, ED) 2. Acquire training materials (spacer, peak flow meter, education booklet from American Lung Association). 3/2014-4/2014 Staff time $ Hands on demonstration and repeat back 3. Train Emergency Department Physicians, Hospitalists, Pediatricians, Pediatric Nurses, Emergency Department Nurses, Therapists, and Case Management on initiative, tools and their roles. 4/2014-5/2014 Staff time 1 hr per session x 8 sessions of process and available resources 4. Develop a post-discharge call process by an educated Therapist. 7/1/ hours/week incorporated into current roles Assessment of knowledge and increased follow-up compliance 5. Develop community pharmacy partnerships and/or resources to obtain medications and develop a tool to assist with physician and/or clinic access for management. 3/2014-4/2014 Pharmacy, Case Management Access to resources 6. Develop an asthma action plan compliance audit/measurement. 3/2014-4/2014 Staff Time (, ED, Pediatrics) Audit of utilization tools 7. Determine Baseline Inpatient and ED Pediatric Admission/ED visit and readmission rates. 3/31/2014 Quality Resource Time Clinical Data Analyst, Baseline improvement metrics 2
3 OBJECTIVE #2: Advocate South Suburban Hospital () will partner with the community to provide resources within 100% of the middle schools and develop 5 community coaches within our primary service area with asthma education, including how to recognize worsening asthma, administer quick-relief medications, and know how and when to call for emergency services to create ive and safe environments in the school and the community by 6/30/2015. Evidence Base: National Asthma Control Initiative National Heart, Lung, and Blood Institute: National Asthma Control Initiative (click here) IDPH: State Asthma Action Plan (click here) 1. Solidify community relationships with community center leaders, park district representatives, sporting leagues and schools within primary service area. 3/2014-5/2014 Staff time (Community Relations, Pediatrics, Case Management) Relations, base within the community 2. Solicit volunteers with asthma from pool to serve as resource experts within the community to improve knowledge of available resources within the community. 3. Create training program for teachers and community asthma coaches to improve knowledge of asthma triggers, treatment options and resources. 3/2014-6/2014 Volunteer Time Mission and Spiritual Care 3/2014-5/2014 Staff Time 4. Organize quarterly session dates with community schools and leaders. 3/2014-6/2014 Staff Time Relations, 5. Conduct training program for teachers and community asthma coaches. 8/2014-6/ hours/quarter in Therapy time Development of community experts 3
4 OBJECTIVE #3: will implement a school-based asthma education program to be provided to 80% of the middle schools within our primary service area to target as many children who have asthma as possible, to provide an opportunity for them to learn asthma self-management skills and to help provide an asthma-friendly learning environment for students who have asthma. Evidence Base: Kickin Asthma American Lung Association: Kickin Asthma Program (click here) 1. Solidify education dates with school leaders within our primary service area. 4/30/2014 Staff Time Relations, Established community partnerships 2. Educate 6 Therapists to teach the Kickin Asthma program. 3/31/2014 Staff Time (train 6 Therapists) Education dollars x 2 hrs/session 3. Obtain materials from American Lung Association to teach class and materials for hands on demonstration (spacers, peak flow meters). 4. Obtain funding to provide treats, prizes and graduation gifts. 3/31/2014 $30.00 per student VP of Operations, 3/31/2014 $25.00 per student VP of Operations, Return demonstration and compliance Positive reinforcement 5. 5 OBJECTIVE #4: will improve access to resources including additional components, such as self-management training, social, and offer resources to reduce asthma triggers in the home environment by developing a virtual asthma learning center by 12/31/2014. Evidence Base: National Asthma Control Initiative National Heart, Lung, and Blood Institute: National Asthma Control Initiative (click here) 1. Partner with Public Relations and Information Technology to investigate resources and implementation strategies for the virtual asthma learning center. 3/31/2014 Information Technology and Public Relations Identify available technologies Abide by system standards 2. Create a web-based asthma program on the internet page. 6/ /2014 Information Technology and Public Relations Staff Time Interactive web based resource for community 3. Partner with community leaders (schools, libraries, community centers) to promote the usage of the internet resources. 1/2015-3/2015 Staff Time Marketing Relations, Community 4.
