We need more information about your or your partner s earnings so we can make a decision about your claim. You must complete this form promptly and sign the declaration on page 6. If you have just started a new business within the last 13 weeks please complete part 1 with the weeks you have been in business for and then complete part 2 (page 7) with an estimation of what you will earn in the next 13 weeks. Please ensure that you sign both declarations.
