1 Unoccupied Commercial Property Owners Insurance Schedule Please check this document. This Insurance is subject to the information detailed in this document. The Insured and their insurance intermediary (agent) should carefully review the contents of the Certificate (including its attached schedule, endorsements and proposal/statement of facts). If any of the information set out therein is incorrect, the Insured or their insurance intermediary must notify the Insurers. Failure to do so may invalidate the Insurance provided. Should at any time prior to the expiry date of this insurance any of the information change, then the Insured has a duty to notify the Insurers of changes as soon as reasonably possible. 2 Endorsement(s) CL007 - Portable heating Warranty It is a condition precedent to the liability of the Underwriters that there will be no use of or storage on the premises of any form of portable heating unless specifically agreed. CL009 - Waste Warranty Warranted combustible storage/waste in the open to be at least 20 ft from building when premises are closed for business. CL015 - Renovation/Refurbishment Exclusion Excluding losses arising from building works, renovation or refurbishment unless otherwise agreed and endorsed by the Insurers. CL019 - Survey Clause Cover hereon is strictly subject to receipt by Underwriters of a satisfactory survey carried out by an authorised surveyor during currency of this insurance or by a date to be agreed by the Leading Underwriter only and advised to the Insured. Underwriters reserve the right to cancel this insurance with immediate effect or impose special terms and conditions and/or revise the premium rate upon receipt of the survey. The Insured shall implement the survey requirement(s) or as otherwise agreed by Underwriters within a period to be agreed by Underwriters and advised to the Insured. If the Insured fails to implement the requirement(s) within the period agreed by Underwriters then all coverage shall terminate at the end of said period. CL020 - Minimum Protections Warranty It is warranted that all final exit doors have a minimum 5 lever mortice deadlocks fitted with box steel striking plates at least 7. Where locks are to be replaced or fitted these should conform to BS3621. Alternatively a 5 lever close shackle padlock with substantial locking bar. All accessible windows to be barred or fitted with key operated window locks or fixed permanently shut. CL023 - Fire Extinguishing Appliances Maintenance Clause It is warranted that the Assured will maintain all the Fire Extinguishing Appliances contained in the premises in full working order during the currency of this Insurance, Nevertheless this Certificate / Policy shall not be invalidated by any defect in the Appliances due to circumstances unknown to or beyond the control of the Assured. CMP002 - Protection Maintenance Clause It is a condition precedent to the liability of the Insurers hereunder that: (a) t h e whole of the protections provided for the safety of the insured property shall be maintained in good order throughout the currency of this Insurance and that they shall be in full and effective operation at all times when the Insureds premises are closed for business and at all other appropriate times, including when the said premises are left unattended, and that such protections shall not be withdrawn or varied to the detriment of the interest of the Company without its consent. (b) all key and duplicate keys relative to the above protections are removed from the premises when the premises are closed for business, and at all other appropriate times, including when the said premises are left unattended. All defects occurring in any protections must be promptly remedied. CMP005 - Unoccupied Property Warranty It is a condition precedent to the liability of the Insurers hereunder that: Page 2 of 6

3 (a) immediate notice is given to the Insurers if the Premises become unoccupied or if there is any change in occupation of the said premises. (b) the Premises are made secure against illegal entry. (c) the Premises are in good condition at inception and will be so maintained and are to be inspected at least once a week and any defect found is repaired immediately. (d) all water is drained from tanks, apparatus and pipes and water turned off and the mains and all other services to be disconnected from the mains. (e) Warranted all letter boxes shall be sealed to prevent insertion of material. It is further understood and agreed that: (i) the Insurers liability hereunder is limited to loss or damage caused by FIRE, LIGHTNING, EXPLOSION, EARTHQUAKE, AIRCRAFT damage only. (ii) the Insurers will not be liable for any loss arising from or caused by building operations CMP012 - Boarding Warranty twithstanding anything contained herein to the contrary, in addition to locks, it is warranted that all accessible openings are secured with professional strength boarding ply and secured with non returnable screws. 5 Proposal / Statement of Fact Disclosure The following proposal/statement of fact, together with any information supplied to the Insurers must not be misleading or incomplete. It will form the basis of the contract with the Insurers and will be incorporated therein. Any non-disclosure or misrepresentation of a material fact could invalidate all or part of the Insurance contract. A material fact is a fact likely to influence insurers assessment of the risk. If you are in any doubt as to what constitutes a material fact you should consult your insurance intermediary. Has the Proposer, any Director or Manager of the Proposer (if a company), or any partner (if a firm); a) ever been convicted of, or charged (but not yet tried) with, a criminal offence (other than motoring offences)? b) ever been prosecuted (whether or not convicted) under the Health & Safety at Work Act 1974, or under any similar statute or regulation? c) ever been declared bankrupt, gone into insolvent liquidation, or been subject to a receivership or an administration order? d) had any Insurer decline insurance, refuse renewal terms, cancel cover, or impose special terms? (b) ever suffered from and / or been monitored for subsidence, landslip or ground heave (such as internal or external cracks or bulging) and / or are they in an area troubled of subsidence, landslip or ground heave? (c) ever had a survey carried out that reported settlement or movement of the buildings? Security (a) Is there an alarm system protecting the premises? (b) Do the locations to be insured meet with the following minimum physical security requirement? Is there any additional material information which should be disclosed in respect of above questions that may influence insurers decision?
