1 SNYDERVILLE BASIN and EASTERN SUMMIT COUNTY CHECKLIST AND REVIEW PROCEDURE FOR SPECIAL EVENT PERMIT SUBMISSION REQUIREMENTS o Application form: Completed and signed by the property owner(s). o Approval of the property owner(s) if different from the applicant: The property owner(s) must sign the back of the application, or submit a letter indicating their ownership and authorization for the submittal of the application. o Proof of insurance: a certificate of broad liability insurance from an A+12 rated or better insurance company, naming Summit County as additional insured, with the following minimum limits: $1,000,000 for events involving less than 5,000 people $2,000,000 for events involving 5,001-10,000 people $5,000,000 for events involving 10,001 or more people o Application fees: $250 for one-day single location events, $400 for multiday/mobile/multi-location events of up to 5,000 people, or $1,000 for multiday/mobile/multi-location events exceeding 5,000 people. o Clean up deposit: Minimum $1,000 dollar refundable cash deposit to cover any cost incurred in cleaning up after the event. o Event summary: a written summary of the event, detailing the following: dates and duration of event number of expected participants, spectators, volunteers/employees whether alcohol will be sold or provided whether food will be sold or provided solid waste management traffic control and management medical plan personnel names/qualifications, type(s)of medical equipment, etc. security plan fire protection plan sound plan (music, amplifiers, etc.) o Site plan(s) / route map: a detailed site plan (and route map for mobile events) must be provided, which may consist of a single or multiple plans, showing in clear detail the following: vicinity / location map parking plan (location, access, capacity, personnel, etc.) location of emergency personnel

2 location of medical tent/structure/supplies bathrooms and hand wash station - location and type any existing or proposed fencing alcohol containment plan if alcohol will be provided lighting and illumination plan location of telephone service camping facilities, if included location of security personnel detailed route map roads / trails / any requested road closures / intersections / volunteer location / signage location / etc. signage plan including the dimensions, colors, content, number, and location of signs o Digital copy: Electronic copy of everything in the application. o ADDITIONAL INFORMATION MAY BE REQUIRED APPLICATION DEADLINES o Non-roadway involving events: Minor events of people: 45 days prior to event Major events of people: 90 days prior to event Major events with more than 5000 people: 120 days prior to event o Event occurring on roadways: Minor events of people: 45 days prior to event Major events of people: 90 days prior to event Major events with more than 5000 people: 120 days prior to event o Regularly established ongoing series (i.e. concerts): 90 days prior to initial event o Applications submitted after these deadlines will be charged double fees. o Applications submitted less than 30 days prior to event will not be accepted. 4 o Special Considerations Ensure that adequate parking is provided. Off-site parking may be considered if transportation to and from the parking area is provided, and staffed with parking attendants. Unpaved parking shall be watered to prevent dust. Typically onstreet parking is not permitted. Temporary signage is limited to two (2) 50 sq. ft. banners, and if necessary, directional signs. Examples of permissible directional signs may include race route signs and bike route signs. Where an event is reasonably anticipated to increase traffic or otherwise impact residents, notification signage and / or door hangars may be required in advance of the event. Road closures, especially in residential areas, require that neighborhoods be appropriately alerted. Medical standby is required when there will be more than 300 participants / spectators, and may be required (determined by Fire District) for athletic events. Off-duty Sheriff s Deputies may be required for events with over 500 people. Back-to-back subsequent events and same-day events may only be approved where it is demonstrated that such events will not be injurious to the health, safety, and general welfare of residents of the area. Events occurring 100% on trails are not required to get a Planning permit, and instead are required to get a SBSRD permit. DECISION o Minor Event: A final decision shall be issued no less than fourteen (14) days prior to the event. o Major Event: A final decision shall be issued no less than twenty one (21) days prior to the event.

8 EVENT PARKING In addition to answering the questions below, the applicant shall attach a parking plan for the event, including either text and/or maps of parking areas, shuttle stops, and pedestrian circulation, parking personnel, and private property owner(s) permission (if applicable). If so, please describe: NOTE: Street and/or lane closures shall be requested to and approved by the Summit County Engineering Department, 60 North Main Street, Coalville, Utah Phone (435) SANITARY SERVICES NOTES: In addition to answering the questions below, the applicant shall attach a site plan showing existing and portable restroom locations, water station locations, solid waste container locations, and first aid locations. Also attached shall be locations identified for directional signs, including first aid, restrooms, and drinking water stations. Summit County encourages sustainable efforts in waste management including recycling of as much event waste as possible. Please contact either the Health Department or Planning Department Staff for a list of local recyclers to find out how they may assist you. Solid waste must be removed from the site within 24 hours of end of the event. Have you read and understand the notes Will there be recycling at the event? Drinking water must be provided free of charge. How will this be done? A garbage can must be provided next to the drinking station. Depending on the size of the event you may be required to provide additional stations. What is the source of water supply? 12 TEMPORARY SIGNS NOTE: The applicant shall provide a sign plan identifying sign descriptions, sizes, and locations of the on-site signage. Signs identifying first aid, restrooms, drinking facilities, and exits are required to be on site during events. Will temporary outdoor signs be used for the event? Two (2) on premise signs fifty (50) square foot signs/banners may be permitted per event. Directional signs may be erected as long as they are posted only for the duration of the event and do not exceed six (6) square feet. No signs are allowed within the State right-of-way without approval from UDOT. INSURANCE Are there insurance policies covering this event? Yes / No NOTE: Insurance policies are required for most events. Summit County reserves the right to require insurance based on the nature and location of the event, the number of people attending the event, and other relevant factors. Standard amounts are as follows: * Events with 5,000 persons or less: minimum policy limit of $1,000,000 * Events with 5,001 10,000 persons: minimum policy limit of $2,000,000 * Events with 10,001 persons or more: minimum policy limit of $5,000,000 Also required is Worker's Compensation with a minimum policy amount of $1,000,000 Summit County shall be listed as an additional insured on all policies.
