1 Injury Illness Prevention Program Policy Policy Statement It is University policy to conduct operations in conformance with all applicable laws, regulations, relevant published standards and practices for environmental, health, and safety protection. Purpose Overview Scope The University is committed to maintaining a safe workplace environment. The Injury and Illness Prevention Program (IIPP) complies with the Cal/OSHA requirement to provide a safe and healthful workplace for all employees (California Code of Regulations (CCR) Title 8, Section 3203). The IIPP is a comprehensive plan demonstrating the integration of the California Code of Regulations (CCR), Title 8, section 3203 regulation requirements of ongoing health and safety activities on campus. It establishes methods for identifying and correcting workplace hazards, providing employee safety training, communicating safety information, and ensuring compliance with safety programs. It is reviewed and updated annually to reflect any changes in regulations, personnel or procedures. This policy describes specific requirements for program responsibility, compliance, communication, hazard assessment, accident/exposure investigations, hazard correction, training, and recordkeeping. Applicability This program applies to all University employees (staff, faculty, and student employees). 2 Responsibility Definitions The ultimate responsibility for Pacific s IIPP rests with the Program Administrator. Program Administrator has the authority and responsibility for implementing provisions of this program for the University. Responsibilities include: Advising senior management on safety and health issues. Working with senior management to develop safety and health guidelines and policies. Preparing and distributing the University's guidelines, policies and procedures on safety and health issues. Reviewing, updating and evaluating the overall effectiveness of the IIPP. Maintaining current information on local, state and federal safety and health regulations, serving as the liaison with governmental agencies. Planning, organizing and coordinating safety trainings. Arranging for safety and health inspections and follow up to ensure necessary corrective action is completed. Establishing, conducting and maintaining an injury/illness/accident report and investigation procedure. Coordinating with Human Resources on maintaining injury and illness records. Reviewing injury and illness trends. Establishing a system for maintaining the records of inspection, hazard abatement and training. Environmental Health and Safety (EH&S) is responsible for the development and administration of the IIPP, this includes: Providing training and support to managers and supervisors on our IIPP. Assistance with conducting workplace hazard assessments to identify, evaluates, and corrects hazards. Evaluating the adequacy and consistency of our training. Manager and Supervisors are responsible for implementing Pacific s IIPP, which includes: Ensuring that workplaces and equipment are safe, well maintained, and in good working order. Ensuring that employees are properly trained to complete all assigned tasks. Ensuring that workplace safety and health practices and procedures are clearly communicated and understood by employees. Enforcing health and safety rules fairly and uniformly. Evaluating employees on compliance with safe work practices. Acknowledging employees who make a significant contribution to maintenance of a safe workplace and disciplining employees who fail to follow safe work practices. Encouraging employees to report health and safety issues without fear of reprisal. Ensuring periodic inspections of workspaces that are under their authority are conducted. Reporting workplace incidents (injuries, exposures, or illnesses) and ensuring they are investigated and corrective action is taken promptly. Revised: March 9, 2016 Page 2 of 7

3 Responsibility Definitions, continued Compliance Safety Communication Employees are responsible for the safety of their own actions and for monitoring safety of the operations around them. This includes: Being familiar with emergency and evacuation procedures. Maintaining personal work areas in accordance with specific safety requirements and wear proper personal protective equipment PPE (when required). Participating in training programs as required. Requesting job safety information when needed. Adhering to healthy and safe practices in their workplace. Promptly reporting any accidents, unsafe conditions or acts to their supervisor. Cooperating with incident investigation when necessary. Suggesting solutions to improve safety. All employees, including managers and supervisors are responsible for complying with safe work practices, following all safety policies and operating procedures, and assisting with maintaining a safe and healthful work environment. To ensure compliance, the University: Informs employees of the provisions of our IIPP; Evaluates the safety performance of all employees; Recognizes employees who perform safe and healthful work practices. This recognition is accomplished by: informal recognition of safety practice, formal recognition of individuals or groups for safety performance, and recognition during annual reviews; Provides additional training to employees whose safety performance may be deficient; Takes disciplinary action for employees who fail to comply with safe and healthful work practices. This process includes a series of write-ups for unsafe work practices on an escalating scale, from warnings to termination. Provides safety training that specifically highlights IIPP in laboratory safety, hazard communication, and new hire orientation The IIPP components are communicated and discussed to facilitate a continuous two-way flow of safety and health information between administration, managers and supervisors, and employees by: Training and discussion during new hire orientations. Workplace-specific safety and health training and discussions regarding job-specific policies; Monitoring by supervision to ensure effectiveness of initial training and providing re-training if necessary; Effectively communicating safety information and concerns through a variety of means such as: /text messages, posts to the University website, University publication and distributing materials to employees, (all which include language translation when appropriate); Safety meetings during staff/faculty meetings to provide information regarding any safety issues. (More frequently as deemed necessary if hazards or occurrences of injuries and illnesses arise); Providing employees an anonymous resource that encourages communication to administration regarding workplace hazards without fear of reprisal. 4 Identifying Hazards Safety inspections identify and evaluate workplace hazards and conditions that may result in illness, injury, or property damage. Managers and Supervisors must ensure that safety inspections are conducted on a regularly scheduled basis. An initial inspection was performed when our IIPP was first established. Additional inspections must be performed by (EH&S) as follows: When new substances, processes, procedures or equipment that present potential new hazards are introduced into our workplace; When new, previously unidentified hazards are recognized; When occupational injuries and illnesses occur; When we hire and/or reassign permanent or intermittent employees to processes, operations, or tasks for which a hazard evaluation has not been previously conducted; Whenever workplace conditions warrant an inspection. Periodic inspections consist of identification and evaluation of workplace hazards utilizing applicable sections of the Hazard Assessment Checklist, and any other effective methods to identify and evaluate workplace hazards. Accident/ Exposure Investigations The University s EH&S department is the only designated department to perform investigation of workplace accidents, hazardous substance exposures, and nearaccidents, which include: Visiting the scene as soon as possible; Interviewing affected employees and witnesses; Examining the workplace for factors associated with the accident/exposure/near-accident; Determining the causes of the accident/exposure/near-accident; Taking corrective action to prevent the accident/exposure/near-accident from reoccurring; and Recording the findings and corrective actions taken within the Enterprise Risk Management information system. Hazard Correction Hazards, including unsafe or unhealthy work conditions, practices or procedures on University property identified as a result of inspections and/or assessments, or at any time, must be corrected in a timely manner based on the severity of the hazard(s), and according to the following procedures: When observed or discovered; When an imminent hazard exists, which cannot be abated without endangering employee(s) and/or property, all exposed employees will be removed from the area, except those necessary to correct the existing condition. Only employees wearing protective equipment and trained to correct the hazard will be admitted to the affected area or participate in the correction/safeguarding. All such actions taken and dates they are completed must be documented. EH &S coordinates the associated corrective actions and interim protection, if needed, with the responsible departments and effected employees as appropriate. Revised: March 9, 2016 Page 4 of 7

5 Hazard Training EH&S provides training support and resources for IIPP requirements. EH&S reviews and approves all safety training programs and informative materials. Training will be provided to all employees: When the IIPP is first established; When new employees are hired; When an employee is given a new job assignment, which training has not been previously provided; Whenever a new substance, process, procedure or piece of equipment are introduced on campus and represent a new hazard; Whenever we become aware of a new or previously unrecognized hazard. Who supervises others (or when an employee becomes a supervisor) to familiarize them with the safety and health hazards to which employees under their immediate direction and control may be exposed; and With respect to hazards specific to each employee's job assignment. This training will include (but is not limited to): Explanation of our IIPP. Emergency action and fire prevention plan. Measures for reporting any unsafe conditions, work practices, injuries and when additional instruction is needed. Provisions for medical services and first aid, including emergency procedures. Proper housekeeping, such as keeping stairways and isles clear, work areas neat and orderly, and promptly cleaning up spills. Prohibiting horseplay, scuffling, or other acts that adversely influence safety. Proper storage to prevent: o Stacking goods in an unstable manner o Storing materials and good against doors, exits, for extinguishing equipment and electrical panels. Where applicable our training may also include: Prevention of musculoskeletal disorders, including proper lifting techniques. Use of appropriate clothing, including gloves, footwear, and personal protective equipment. Information about chemical hazards to which employees could be exposed and other hazard communication program information. Proper food and beverage storage to prevent them from becoming contaminated. In addition, EH&S will provide specific instructions to all employees regarding hazards unique to their job assignment, to the extent that such information was not already covered in other trainings. 7 Record Keeping and Documentation Requirements Our organization has ten or more employees and keeps records as follows: Safety Inspection Reports include: 1. Records of scheduled and periodic inspections 2. Person(s) conducting the inspection. 3. Any identified workplace hazards (i.e., unsafe conditions and work practices) 4. Corrective action(s) taken to remedy the identified unsafe conditions and work practices Campus Training Record Location Stockton Training records (including department/group safety meetings) are retained by EH&S. Sacramento Training records (including department/group safety meetings) are retained by Human Resources San Francisco Training records (including department/group safety meetings) are retained by department * Records are to be kept for no less than a one year period. Documentation of health and safety training for each employee includes: 1. Employee name or University ID 2. Training date(s) 3. Type(s) of training 4.
