General Instructions: Complete all sections of the Application Form. Please print all information clearly. Incomplete applications cannot be processed and will delay processing. The application fee will not be refunded. The College recommends that you make a copy of the completed Application Form and supporting documents before mailing the Application Form package to the College. Any original documents submitted may not be returned. The College does not accept faxed/ ed copies of the Application Form. Mail the original, completed, dated and signed Application Form and supporting documents, with your application and payments to: College of Early Childhood Educators 438 University Avenue, Suite 1900, Toronto ON M5G 2K8 Additional information and documentation may be required for the College to assess your qualifications for registration. The College will send confirmation of receipt of your Application Form by or mail (if is unavailable). Please note this is not proof of membership to the College. You may also download and print the Application Form and Application Guide from the website Please note: Your application will be valid for two years. If the College has not received all the documents required within two years, the College will evaluate your application based on the available information. Educational Requirements for Entry: To become a member of the College, you must meet the College s entry requirements. If you check at least one of the categories listed below, you may continue to complete the corresponding Application Form. I am an applicant in my last semester of the ECE diploma program offered by an Ontario College of Applied Arts and (OCAAT) or a program on the Approved Post Secondary Programs List (see Page 6-7). I am a graduate of a diploma program in early childhood education offered by an Ontario College of Applied Arts and (OCAAT) (see Page 6). I am a graduate of an Ontario program on the Approved Post Secondary Programs List (see Page 6-7). I am a graduate of a program in Canada (outside of Ontario) on the Approved Post Secondary Programs List (see Pages 8-12). I hold a licence or certificate granted by a regulatory authority of another Canadian province or territory which is considered a match to the Certificate of Registration issued by the College (see Page 5). I have received a Recognition of Equivalency Certificate or a Letter of Equivalency Certificate from the Association of Early Childhood Educators Ontario (AECEO) or the Association francophone à l éducation des services à l enfance de l Ontario (Aféseo) prior to February 23, College of Early Childhood Educators - Application Guide (October 2015) Page 1 of 12

2 Confidentiality: Persons engaged in the administration of the Early Childhood Educators Act, 2007 are required to comply with section 51 of the Act to keep confidential all information that comes to their knowledge in the course of their duties, subject to certain exceptions. Breaching the provisions of section 51 can lead to the imposition of a fine as set out in the Early Childhood Educators Act. Section 1 Personal Information Last name Enter your last name as it appears on your current identification. First name Enter your first name as it appears on your current identification. Middle name(s) Enter your middle name(s) as it appears on your current identification. Common first name Enter the first name that you are known by (for example: if you use your middle name as your first name, please provide it as your common name.) Note: If the name(s) on your documents is different from your current name, you must provide proof of name change. If you have changed your name when you married, please provide a photocopy of the marriage certificate. If you have legally changed your name, please send a photocopy of either the change of name certificate or the court order that changed your name. Please note that both your previous name(s) and your new name must be referenced in the certificate or the court order. Employment Information Full-time means your official status with your employer is full time or equivalent or your usual hours of work are equal to or greater than 30 hours per week. Part-time means your official status with your employer is part-time or your usual hours of work are less than 30 hours per week. Casual means your status with your employer is on an as-needed basis, with employment that is not characterized by a guaranteed fixed number of hours per week. Place of employment address Enter the name and the address of your current place of employment (not the supervisor name). If your business address is the same as your home address, please enter your home address again in this field. Please note that the College has authority to designate certain information, such as a member s business address, as forming part of the public register of members. Communications from the College Preferred mailing address for communications from the College The College will mail you newsletters and other information items to your preferred mailing address. If you do not check either box, the College will send it to your home address as the default. If you would like the information sent to your business address, check the business address box. Personal address for communications from the College is increasingly important as a means of communication. You may indicate a personal address where you prefer to receive information from the College. Preferred language for communications from the College Check the box of the preferred language in which you would like the College to communicate with you. 3 Section 2 Educational Qualifications The Application Form provides options for the type of document(s) you must provide as evidence, depending on the educational qualifications you possess. Please read each section carefully. Effective February 14, 2011, the College implemented the following registration practices: a) an applicant who is required to satisfy the requirements of the Registration Regulation must provide an official transcript issued by the educational institution at which the applicant obtained his or her academic credential, bearing the institution s seal and signature of the registrar or equivalent official; b) an applicant who has received a Recognition of Equivalency Certificate or a Letter of Equivalency Certificate issued by the Association of Early Childhood Educators Ontario (AECEO) or the Association francophone à l éducation des services à l enfance de l Ontario (Aféseo) must provide to the College an original letter* on letterhead of the AECEO or the Aféseo, signed by a senior official of the AECEO or the Aféseo. The letter must confirm the issuance of a Recognition of Equivalency Certificate or Letter of Equivalency Certificate to the applicant, the date on which the applicant applied to the AECEO or the Aféseo for the certificate and the date on which the certificate was issued. c) the transcript described in subparagraph (a) must be mailed directly from the educational institution to the College of Early Childhood Educators. Note: If you hold a licence or certificate granted by a Canadian regulatory authority of another province or territory which is considered a match to the Certificate of Registration issued by the College of Early Childhood Educators, you are not required to provide the above evidence. Please complete the Application Form for Individuals who hold a licence or certificate granted by a Canadian regulatory authority of another province or territory which is considered a match to the Certificate of Registration issued by the College. See page 4 of this guide for the list of match certificates and licences. *For the Recognition of Equivalency Certificates or Letter of Equivalency Certificates issued by the AECEO prior to April 2011, a verification letter issued directly to the College may be requested. Section 3 Language Fluency You must be able to speak and write either English or French with reasonable fluency. Please check at least one of the statements on the Application Form that applies to you. You may be requested to provide additional information. Section 4 Citizenship/Immigration Status You are required to be a Canadian citizen, a permanent resident of Canada or be authorized under the Immigration and Refugee Protection Act (Canada) to engage in employment within the practice of the profession. Please answer the questions on the Application Form as they apply to you and attach the required evidence as applicable: Canadian birth certificate, valid Canadian passport, Certificate of Canadian Citizenship, Canadian citizenship card, Certificate of Indian status, Record of Landing, Confirmation of Permanent Residence, Permanent Resident card, valid work permit which provides authorization to engage in employment within the practice of the profession for at least one year period, etc. College of Early Childhood Educators - Application Guide (October 2015) Page 3 of 12

4 Section 5 Declaration of Conduct Answer the four questions listed which relate to your conduct. If you answer yes to any of these questions, please provide a detailed explanation on a separate piece of paper, which you may place in a sealed envelope labelled confidential. The College will consider and assess whether the conduct is significant. A yes response does not necessarily make you ineligible for registration in the College. The College reserves the right to decide on an individual basis as to the possible impact of the conduct on the practice of the profession. Note: You are required to immediately inform the Registrar if, after having applied for but before being issued a Certificate of Registration, you become the subject of a finding or proceeding described in Section 5 of the Application Form or you are found guilty of an offence described in Section 5 of the Application Form. Supporting documentation may be requested such as: additional information from the Children s Aid Society, letters of reference from past or current employers, etc. Section 6 Application & Registration Fees The application fee is for the processing of your application and is non-refundable, regardless of the outcome of the application process. The one-time fee is $75. The fee for registration as a member is $150. The fee will be returned if your application is not approved for registration. An annual fee of $150 is payable on the anniversary of the date on which your Certificate of Registration is issued. Please note: Acceptable methods of payment for fees are: Cheques in Canadian funds and from a Canadian bank Money Orders in Canadian funds and from a Canadian issuer Bank Drafts in Canadian funds Visa or MasterCard or Visa debit Please print your name on the cheque or money order, for a total of $225, made out to the College of Early Childhood Educators. Post-dated cheques are not accepted. American Express and pre-paid credit cards are not accepted. Cash is not accepted and the College is not responsible for lost cash. A processing fee of $30 will be charged for any cheque that is not honoured. Bulk payments for multiple applications are not accepted. U.S. and other international cheques are not accepted. For international payment, please pay in Canadian dollars by bank draft or credit card. If paying by credit card, the application fee and registration fee will appear on your bank statement as two separate charges. For confidentiality and safety, the College does not accept credit card information over the phone. To protect the confidentiality of your information, credit card information will be blacked out after it is no longer needed. The application fee is non-refundable. Signed Confirmation All applicants are required to sign and date the Application Form to indicate that all the information in the Application Form and related documents is true. List of Certificates or Licences Considered to match the College of Early Childhood Educators Certificate of Registration from Canadian provinces and territories other than Ontario Alberta Child Development Supervisor (formerly Level 3 - Advanced Qualification Certificate) British Columbia Manitoba Newfoundland and Labrador Nova Scotia Prince Edward Island Saskatchewan Yukon Territory Infant Toddler Educator or Special Needs Educator Early Childhood Educator Level II or III Child Care Services Levels 2, 3, 4 Level 2 or Level 3 Classification Early Childhood Supervisor Early Childhood Educator Level III Child Care Worker Level III The Ontario Labour Mobility Act came into force on December 15, The purpose of this Act is to eliminate or reduce measures that restrict the ability of an individual to obtain a Certificate of Registration in Ontario when the individual is already certified in the same occupation by a regulatory authority of another Canadian province or territory. If you hold one or more of these licences or certificates granted by a regulatory authority of one of the provinces or territories indicated above, please indicate this information on the Application Form under Section 2 d). The licence or certificate must be valid. Once in receipt of your application, the College will contact you regarding any additional documentation required for registration.
