1 Joel Allison Chief Executive Officer Baylor Scott & White Health Joel T. Allison is chief executive officer of Baylor Scott & White Health. The largest not-forprofit health care system in Texas, and one of the largest in the United States, Baylor Scott & White Health was born from the 2013 merger of two exemplary systems. Scott & White Healthcare was established in Temple in Baylor Health Care System was founded in Dallas in Both grew into systems respected nationally for patient care, medical research and education. Combined, they comprise 43 hospitals, more than 500 patient care sites, more than 6,000 affiliated physicians, 34,000 employees and a health plan. Joel s career includes more than three decades in health care management. He joined Baylor Health Care System in 1993 and served as senior executive vice president and chief operating officer before being promoted to president and CEO in Joel is a Fellow of the American College of Healthcare Executives. Nationally, he serves on the Healthcare Leadership Council, the United Surgical Partners International board and the board of commissioners for the Joint Commission. Governor Rick Perry recently appointed Joel to serve on the Texas Institute of Health Care Quality and Efficiency board and is involved in many other state and local organizations, including the Texas Association of Voluntary Hospitals, Healthcare Coalition of Texas, Baylor University Board of Regents, Dallas Citizens Council, Dallas Education Foundation and the Denison Forum on Truth and Culture. During his career, Joel has received numerous recognitions for his outstanding leadership and commitment to the health care field. He received a bachelor s degree in journalism and religion from Baylor University in 1970, a master s degree in health care administration from Trinity University in 1973 and is a graduate of the Harvard Business School Advanced Management Program. Today, through his leadership role with Baylor, Joel continues to fulfill his lifelong mission to help lead an organization that combines ministry and health care in its mission statement and day-to-day operations. Joel and his wife Diane have a daughter, two sons and six grandchildren.

2 Michael J. Mack, MD Medical Director of Cardiovascular Surgery Baylor Health Care System Michael Mack, MD has practiced cardiothoracic surgery in Dallas, Texas since He is board certified in internal medicine, general surgery and thoracic surgery. Currently, Dr. Mack is the medical director of cardiovascular surgery for Baylor Health Care System, chair of the Baylor Health Care System Cardiovascular Governance Council and director of cardiovascular research at The Heart Hospital Baylor Plano. He also co-founded and is chair of the Cardiopulmonary Research Science and Technology Institute (CRSTI). He has more than 400 peer-reviewed medical publications. Dr. Mack is past president of the Society of Thoracic Surgeons (STS), as well as past president of the Thoracic Surgery Foundation for Research and Education ( ), the Southern Thoracic Surgical Association (2009) and the International Society for Minimally Invasive Cardiothoracic Surgery (2000). He has served on the board of directors of the STS and is currently on the board of CTSNet. Dr. Mack was recently appointed to the American College of Cardiology (ACC) board of trustees and is a member of the ACC Interventional Scientific Council. He is an honorary member of the German Society for Thoracic and Cardiovascular Surgery. He also is chair of the STS/ACC National Transcatheter Valve Therapy Registry Steering Committee and is a member of the MDEpiNet Advisory Committee.

3 Guy M. Masters Senior Vice President The Camden Group Guy Masters is a senior vice president with The Camden Group. With 25 years of health care experience, Mr. Masters focuses on strategic, business and service line planning, business advisory and competitive positioning strategies for hospitals/health systems, physicians and payers. He has developed health maintenance organizations, preferred provider organizations, independent practice associations, management service organizations, physician-hospital organizations, medical groups and direct contract relationships with employers. He is an experienced facilitator of board retreats, strategic planning processes and business planning for health care services. Mr. Masters has written more than 80 articles on health care trends, strategic and business planning, financial forecasting, managed care strategy and physician-hospital integration. He is co-author of the Health Care Capitation & Risk Contracting Manual, (Thompson Publishing, New York, 1995) and a frequent speaker at industry conferences, professional associations and other meetings nationwide. He is an advisor to and is on the faculties of The Governance Institute and The Medical Leadership Forum. Prior to joining The Camden Group, Mr. Masters was with Ernst & Young s Los Angeles West Region Health Care Consulting Group, and served on their National Physician Services Advisory Committee. He holds a bachelor s degree in business from Iowa Wesleyan College, and a bachelor s degree in political science as well as an MPA from Brigham Young University.

4 Jean A. Moore, FSA, MAAA Senior Consultant and Actuary Towers Watson Jean Moore is a senior consultant and actuary in the Towers Watson Denver office. She is the West Division Health and Group Benefits Practice Leader and is responsible for all Health and Group Benefits activities for the West Region, which includes California, Texas, Arizona, Colorado, Washington, Oregon, Wyoming, Montana, Nevada and Utah. Previously, Jean spent four years as the chief Health and Welfare actuary for Towers Perrin. She has been with the firm since Jean spent more than 11 years with a major consulting firm in Denver, before joining Towers Watson. With more than 25 years in the employee benefits consulting industry, Jean has considerable experience with the ongoing maintenance and overall strategic evaluation of health and welfare benefit plans. Some examples of her experience include: Developing health care strategies that are sustainable and cost-effective in conjunction with health care reform requirements Designing benefit plans to support corporate culture and business objectives Assisting with collective bargaining processes, including the development of interactive cost models Evaluating quality and cost criteria for health plans to determine health plan program structure Performing statistical evaluations of health plan performance to gauge the relevance of any variances in the results Negotiating with carriers during RFP and renewal process Performing postretirement medical valuations to determine current and future liabilities in accordance with FAS (Financial Accounting Services) 106 and FAS 112 Education and Credentials Jean is a Fellow of the Society of Actuaries and a member of the American Academy of Actuaries. She graduated summa cum laude from Drake University with a bachelor s degree in actuarial science.

5 Kevin Holloran Director, Co-Analytical Leader Healthcare Group and Analytical Manager Standard & Poor s Ratings Services Kevin Holloran, director of Corporate & Government Ratings, joined Standard & Poor s Rating Services in Kevin is the co-analytical leader and an analytical manager in the not-for-profit health care group. He follows a portfolio of high-profile acute care health care credits around the country. Before joining Standard & Poor s in 2003, Kevin spent seven years as a consultant at Ernst & Young, LLP and Cap Gemini Ernst & Young, focusing on post-merger integration projects within the health care space. Kevin holds a master s of health services administration from the University of Michigan and a bachelor s degree in business from the University of Notre Dame.

6 Michelle Berndt Mrs. Texas International Breast Cancer Survivor On Feb. 14, 2012 I was diagnosed with invasive breast cancer. This was not a surprise to me as I am a fourth generation BRCA 2 gene carrier and had recently watched my mom and grandma suffer from cancer. On Feb. 24, 2012, I had a bilateral mastectomy and the cancer was removed. However, the journey had only begun. Shortly after followed eight rounds of chemo, eight steroid shots, an emergency three-day stay at the hospital and 33 radiation treatments. I believe God chose this path for me to help empower women to embrace their inner beauty even when challenges are thrown their way. At a time when I had no eyebrows, eyelashes or hair, I was most positive, strong and confident about my future. I am proud to say I am a one-year survivor and just finished the final chapter in March 2013 with my reconstructive surgery and hysterectomy.
