1 About Catherine Kasper Life Center Executive Director's Message Since 1971, the Catherine Kasper Life Center (CKLC), a designated Continuing Care Retirement Community (CCRC) has excelled at providing customized care options that include independent-living retirement apartments and a skilled nursing care facility. We have excellent rehabilitation and physical therapy services, and a spectrum of educational and enrichment activities and programs that include our MoonTree weaving and painting classes, Earthworks gardening, craft, and environmental sustainability courses, fishing excursions, lectures at Ancilla College, and concerts. Within the next year, we will begin to create an assisted living facility to provide supervised residential services for you or your family. Our dedicated staff is an integral part of our success; they provide a high level of consistency in care to our residents. This level of excellence is why my own mother resides happily at CKLC. Who We Are The Catherine Kasper Life Center Inc. (CKLC) is a ministry of the Poor Handmaids of Jesus Christ (PHJC). Located in rural Donaldson, IN, since 1922 and originally the site of the convent of the religious congregation of PHJC, the PHJC Ministry Center was created in the mid 1980s out of the women religious recognition that they no longer required all the facilities for their own purposes. A decision was made to create an ecumenical space of welcome and ministry so that others could also benefit from and use their facilities. A variety of ministries were created; Catherine Kasper Life Center being one of those ministries. What We Offer The Catherine Kasper Life Center campus is nestled in a picturesque landscape overlooking Lake Gilbraith. The Catherine Kasper Life Center is a community dedicated to creating a joy-filled, safe, healthy environment where residents are assured that Christian values exist. It is composed of Maria Center independent senior living and Catherine Kasper Home, a licensed nursing facility. It also offers a variety of intergenerational experiences and activities, which promote physical, mental and spiritual well-being. 2 Maria Center: Independent Living Maria Center features 27 beautiful, unfurnished efficiency and one- and two-bedroom apartments each equipped with a private bathroom and shower, a kitchen with stove and refrigerator, window coverings and carpeting. It s comfortable gracious living at an affordable price for independent mature individuals and married couples. Overview of Maria Center Services: Pastoral Care: To promote the dignity of residents, Pastoral Care Services ministers in a holistic manner. Pastoral support is available to residents, family and friends. Pastoral Services are provided through daily Mass services and the Sacraments, non-denominational services and small group Scripture reflection. Housekeeping Services: Bi-weekly dusting, vacuuming, cleaning bathroom(s), annual top to bottom cleaning of apartment Laundry Facilities: Located on each floor Dietary Services: Dietary Services is dedicated to presenting attractive, delicious and nutritious meals for residents and their guests in a pleasant setting. Parking: Free reserved parking and snow removal for residents vehicles 24-hour Emergency Alert and Response System Rehabilitation Services: Specialize in treatment of: orthopedic, (hip replacement, shoulder replacement), stroke rehabilitation, brain injury, neurological cases, Parkinson s, lupus, pneumonia, balance issues, back pain, pain management, tendonitis, knee pain, ankle pain, wound care, and many other ailments. Therapy: Therapy Services utilizes qualified professionals to serve the occupational, physical and speech therapy needs. Each resident is assessed by a team of therapists to determine a personalized treatment program. 3 Maria Center: Rates Prices are subject to change. Please contact us for in-depth financial information. Entrance Fee: $5,000 individual/$7, 500 couple The purpose of the Entrance Fee is to generate income to contribute to the operating income of Catherine Kasper Life Center and to help pay for operating and capital costs. As such, income earned after the Occupancy Date from the investment of the Entrance Fee will be paid to Catherine Kasper Life Center for the benefit of CKLC. In addition, at CKLC's sole discretion, Entrance Fees may also be used to pay for project development costs, start-up deficits, interest expense, debt retirement, costs of future expansions, and other purposes Catherine Kasper Life Center deems appropriate. No reserve funding will be specifically established pertaining to the Entrance Fee. A resident may request assistance from the Finance Assistance Fund to pay part or all of the Entrance Fee, if needed, based on information in the Financial Disclosure form. CKLC will refund 2/3 of the Entrance Fee up to one (1) year and 1/3 of the Entrance Fee up to the second (2 nd ) year, without interest, on the later of: (1) the effective date of termination of this Agreement; or (2) the date a new Entrance Fee and executed Agreement have been received from a new resident and the new resident has taken occupancy of Resident s apartment. The Entrance Fee is non-refundable as of the first (1 st ) day of third (3 rd ) year of occupancy. Security/Apartment Maintenance and Renewal Fee: $1,000 CKLC will refund the Security/Apartment fee when the apartment has been completely vacated and if the apartment is left in usual wear and tear. Months Rent: $ [refer to chart below for rates] Rental costs are based on the square footage of the apartment chosen. See Rent Rate sheet. In addition to the monthly fee of $20, the business office bills resident for all long distance phone calls. 6 Assisted Living: Coming Soon Catherine Kasper Life Center (CKLC) is a continuing care retirement community sponsored by the Poor Handmaids of Jesus Christ. It includes Maria Center with its 27 apartments for independent seniors, and Catherine Kasper Home, an 81-bed facility for both skilled and rehabilitative care. This facility has consistently received the top five-star ranking among skilled nursing facilities in the nation for its quality care. Poor Handmaid Values of Community and Dignity and Respect for all focus the purpose of the Catherine Kasper Life Center to provide high quality yet affordable housing and healthcare for seniors. The Catherine Kasper Life Center Mission Statement affirms that the residents and co-workers form a community in which all persons are equally listened to, cared for, appreciated and respected. This community commits itself to the comfort, dignity and well being of all residents as they continue on their journey through life. CKLC leadership listened to the needs of seniors in Marshall and Starke Counties. They heard a need for an assisted level of care between independent living and skilled living. After careful strategic planning it was determined a new Licensed Residential Facility is needed to help meet the demands for assisted living in the surrounding area. A Market Assessment for the Catherine Kasper Life Center was prepared by Tim Stair of the Mennonite Health Services in Goshen, Indiana. It confirmed a need for assisted living in this locality. CKLC is one of only two facilities in the Marshall/Starke area that offers any kind of continuum of care. If CKLC were to add assisted living it would be the only non-profit continuum of care provided in the area. As currently envisioned, the Licensed Residential Facility would be a three-story building with approximately 70,000 + square footage containing 56 apartments. These square foot apartments would be divided into four neighborhoods providing a sense of community. Cost for such a Licensed Residential Facility is projected at approximately $5 million. 7 Catherine Kasper Home: Nursing Care Located in rural Donaldson, IN, Catherine Kasper Home and the surrounding campus is nestled in a picturesque landscape overlooking Lake Gilbraith. Catherine Kasper Home (CKH) is a non-profit care facility with a holistic approach. Our spacious, pristine, 81-bed facility is part of Catherine Kasper Life Center, a faith-based community providing a wide range of care services from independent living at the Maria Center apartments to skilled nursing care at Catherine Kasper. Catherine Kasper Home has recently received the highest survey rating available for nursing homes participating in the Medicare and Medicaid programs. We pride ourselves on maintaining a high quality of life for our residents. To that end, we have dedicated staffers administering to several different types of private living quarters: Medicare/Skilled Nursing Facility, Memory Unit, Nursing Facility, and Non-Skilled Nursing Facility rooms. Every room at Catherine Kasper Home includes, at no extra charge: Pastoral Care, Dietary Services, Nursing Care, Therapy, Dietary Services, Activity Services, Art Classes, Monthly Lecture Series, Social Services, Environmental Services, Personal Laundry Services, Hair Care, Cable TV Service, and Mobile Care. 10 Rehabilitation Services and Restorative Care Therapy Services utilizes qualified professionals to serve the occupational, physical and speech therapy. Each resident is assessed by a team of therapists to determine a personalized treatment program. Restorative care assists residents in meeting individual goals and maintaining independence and 90-95% percent of the patients in the skilled unit are eventually able to return to a home environment. Overview of Rehabilitation Services: Specialized treatment of: orthopedic, (hip replacement, shoulder replacement), stroke rehabilitation, brain injury, neurological cases, Parkinson s, lupus, pneumonia, balance issues, back pain, pain management, tendonitis, knee pain, ankle pain, wound care, and many other ailments. Equipment and therapies utilized include: Aqua Therapy, Electrotherapy (Electrical stimulation, ultrasound therapy), treadmill, and recumbent bikes Maynerd Utayde, our trained Physical Therapist, has a 15-year background in hospitals and nursing homes. Other staffers include committed occupational therapists and occupational pathologists. Weekend appointments are available, which means 7 days of therapy are potentially available to patients. 11 Intergenerational Living & Learning Programs At Catherine Kasper Life Center, we pride ourselves on enhancing our residents quality of life with living and learning programs, which accommodate all age groups. Our retirement community is located on grounds that include a college, greenhouses, art studios, and a lake to ensure our residents have access to a range of activities within footsteps of your home. o o o o Ancilla College, Ancilla College is a 2-year liberal arts college sponsored by the Poor Handmaids of Jesus Christ, which offers 17 academic programs for traditional and non-traditional students. The college is located on the grounds of the PHJC Ministry Center in Donaldson, Indiana. Earthworks, Earthworks is an environmental education resource for local communities sponsored by the Poor Handmaids of Jesus Christ. Dedicated to demonstrating and teaching how all species on our fragile Earth are connected, Earthworks offers a place of education, beauty and demonstration. The Earthworks campus is rich in natural ecosystems including wetlands, a forest environment, riparian zone, and restored prairie areas. In addition, Earthworks has herb and vegetable gardens, a large teaching greenhouse and domestic farm animals. Lindenwood Lindenwood is an ecumenical retreat and conference center located at the PHJC Ministry Center in Donaldson, IN. Nestled among rural woodlands, it offers a peaceful and spiritual atmosphere for churches, non-profit organizations, businesses and individuals to address issues of personal growth and development. The Center provides space for reflection and meetings in a peaceful atmosphere. A gothic-style chapel, prayer garden, labyrinths, and comprehensive library offer ideal settings for relaxing. In addition, Lindenwood sponsors many retreats and workshop opportunities such as 12-step, women s and men s spirituality, and quiet days of reflection. MoonTree, The ministry of MoonTree Community works to encourage the artistic movements in each individual in an ongoing attempt to help earth heal herself and unite all with the larger whole.
